1 ARRIVAL NOTICE DOCUMENTS The following documents are required for customs clearance. Once prepared, please forward them to the Destination Agent at your earliest convenience by Courier. 1) A photocopy of the following pages of your passport - Photo and signature page - Entry Stamp into Japan (the initial entry stamp and most recent stamp, if different) - Japanese Visa - Visa of country where shipment originated (both valid and expired) if not a citizen of that country. If you entered Japan with a 90-day visa, please prepare an explanation letter as per the sample Appendix 1. If you have both air and sea shipments, please prepare two letters. 2) Unaccompanied Baggage Declaration Form (ORIGINAL). The stamp on the form must match with the entry stamp date on your passport. If you did not obtain the Unaccompanied Baggage Declaration form when you entered Japan you must prepare the statements identified in Appendix 2-A for an AIR shipment or Appendix 2-B for a SEA shipment. If you have an AIR and SEA shipment, both documents must be prepared. 3) Key(s) to any locked items in your shipment. 4) A list of any newly purchased items, if any, indicating price and date of purchase 5) Please fill out the Confirmation Sheet Appendix 3 IMPORTANT NOTICE: Please take the time to read the information on Importation of Prohibited Items into Japan, listed in this document.

2 APPENDIX 1 If you entered Japan on a 90-day visa, please prepare this letter on your company letterhead. However, I am to be assigned to Japan on a long-term contract and am in the process of obtaining proper visa. Since I am to live in Japan for over one year, I have had my household goods and personal effects shipped to Japan. Since the circumstances are as explained above, it is desired that my shipment be allowed to be cleared through the customs on the 90-day visa which will be reissued shortly for a period of one year. 3 APPENDIX 2-A (AIR SHIPMENT ONLY) If you do not have the original declaration form stamped at Narita, please print below on a blank paper and fill out the statement. Because: 1. I forgot to do so. 2. I didn t understand to do so. Now that my unaccompanied articles have arrived, and they are indeed my belongings, I would like your permission to import them into Japan without the unaccompanied declaration form. 5 Importation of Prohibited Items into Japan Please note that Japanese law prohibits importation of pornography, drugs 1, weapons 2 and any restricted items in accordance with CITES - Convention on International Trade in Endangered Species. 1 Drugs can include over-the-counter and prescription medicine, depending on the ingredients please seek guidance prior to including these items in your shipment. 2 Weapons can include swords purchased or received in Japan, that have previously been exported from Japan. Should your shipment contain any of the above, please urgently advise the Destination Agent so that immediate action can be taken to minimize delay in customs clearance. If the prohibited items are found on the day of clearance, please note that Japanese customs will suspend clearance for the entire shipment until an original form of Waiver Agreement is submitted to the Customs Office. This procedure can take 1-2 weeks to complete. If you have any locked items in your shipment, (safe, tool box, desk, etc), please enclose the key or combination and ensure that you report any locked items when sending your original documents for customs clearance. Should customs find any locked items at the time of the customs clearance process, the clearance will be suspended until the key to the locked item is delivered to the customs site. Once the customs officer decides to suspend clearance, the entire shipment will be stored at a bonded warehouse at the customs site, where additional charges will be incurred on a daily basis.
