However, new pages are occasionally added to the tariff. When a new page is added between pages already in effect, a decimal is added. For example, a new page added between Page 14 and 15 would be B. Page Revision Numbers - Revision numbers also appear in the upper right corner of each page. These numbers are used to determine the most current page version on file with the Commission. For example, the 4 th revised Page 14 cancels the 3 rd revised Page 14. C. Paragraph Numbering Sequence - There are various levels of paragraph coding. Each level of coding is subservient to its next higher level: D. Check Sheets - When a tariff filing is made with the Commission, an updated Check Sheet accompanies the tariff filing. Application of the Tariff This tariff governs the Carrier s services that originate and terminate in Maryland. Specific services and rates are described elsewhere in this tariff The Company s services are available to residential and business customers The Company s service territory is consistent with the local calling areas and exchange areas listed in Verizon's tariff.

6 PSC MD Tariff No. 2 Original Page Definitions Carrier, Company or Utility refers to Commission means the Maryland Public Service Commission Completed call is a call which the Company s network has determined has been answered by a person, answering machine, fax machine, computer modem device, or other answering device Customer means any person, firm, corporation, or governmental entity who has applied for and is granted service or who is responsible for payment of service Residential customer is a customer who has telephone service at a dwelling and who uses the service primarily for domestic or social purposes. All other customers are nonresidential customers Service means any telecommunications service(s) provided by the Carrier under this tariff Station means a telephone instrument consisting of a connected transmitter, receiver, and associated apparatus to permit sending or receiving telephone messages Time period means the interval of hours that distinguish day, evening, night, and weekend rate periods as indicated in Section 5. The Company charges weekend rates on the following Federal holidays: New Year s Day, Martin Luther King s Birthday, President s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.

7 PSC MD Tariff No. 2 Original Page 7 SECTION 2 - RULES AND REGULATIONS 2.1 Undertaking of the Company The Company offers local exchange and long distance telecommunications services to residential and business customers in the local exchange service areas of Verizon as specified in Verizon s tariff. 2.2 Obligations of the Customer The customer shall be responsible for: The payment of all applicable charges pursuant to this tariff; Reimbursing the company for damage to, or loss of, the company s facilities or equipment caused by the acts or omissions of the customer; or the noncompliance by the customer, with these regulations, or by fire or theft or other casualty on the customer s premises unless caused by the negligence or willful misconduct of the employees or agents of the Company Providing at no charge, as specified from time to time by the Company, any needed space and power to operate the company s facilities and equipment installed on the customer s premises Complying with all laws and regulations regarding the working conditions on the premises at which the Company s employees and agents shall be installing or maintaining the Company s facilities and equipment. The customer may be required to install and maintain the Company s facilities and equipment within a hazardous area if, in the Company s opinion, injury or damage to the Company s employees or property might result from installation or maintenance by the Company. The customer shall be responsible for identifying, monitoring, removing and disposing of any hazardous material prior to any construction or installation work Complying with all laws and regulations applicable to, and obtaining all consents, approvals, licenses and permits as may be required with respect to, the location of the Company s facilities and equipment in any customer premises for the purpose of installing, inspecting, maintaining, repairing, or upon termination of service as stated herein, removing the facilities or equipment of the Company Making Company facilities and equipment available periodically for maintenance purposes at a time agreeable to both the Company and the customer. No allowance for interruptions in service will be made for the period during which service is interrupted for such purposes With respect to any service or facility provided by the Company, the customer shall indemnify, defend and hold harmless the Company from all claims, actions, damages, liabilities, costs and expenses for:

8 PSC MD Tariff No. 2 Original Page Any loss, destruction or damage to property of the Company or any third party, or injury to persons, including, but not limited to, employees or invitees of either the Company or the customer, to the extent caused by or resulting from the negligent or intentional act or omission of the customer, its employees, agents, representatives or invitees; or Any claim, loss, damage, expense or liability for infringement of any copyright, patent, trade secret, or any proprietary infringement of any copyright, patent, trade secret, or any proprietary or intellectual property right of any third party, arising from any act or omission by the customer The customer is responsible for ensuring that customer-provided equipment connected to Company equipment and facilities is compatible with such equipment and facilities. The connection, operation, testing, or maintenance of such equipment shall be such as not to cause damage to the Company provided equipment and facilities or injury to the Company s employees or other persons. Any additional protective equipment required to prevent such damage or injury shall be provided by the Company at the customer s expense The Company s services (as detailed in this tariff) may be connected to the services or facilities or other communications carriers only when authorized by, and in accordance with, the terms and conditions of the tariffs or contracts which are applicable to such connections Upon reasonable notification to the customer, and at a reasonable time, the Company may make such tests and inspections as may be necessary to determine that the customer is complying with the requirements set forth in this tariff for the installation, operation, and maintenance of customer-provided facilities and equipment that is connected to Company-owned facilities and equipment. 2.3 Liability of the Company In view of the fact that the customer has exclusive control over the use of service and facilities furnished by the Company, and because certain errors incident to the services and to the use of such facilities of the Company are unavoidable, services and facilities are furnished by the Company subject to the terms, conditions and limitations herein specified:

9 PSC MD Tariff No. 2 Original Page Service Irregularities The liability of the Company for damages arising out of mistakes, omissions, interruptions, delays, errors or defects in transmission, or failures or defects in facilities furnished by the Company, occurring in the course of furnishing service or other facilities and not caused by the negligence of the customer, shall in no event exceed an amount equivalent to the proportionate charge to the customer for the service or facilities affected during the period such mistake, omission, interruption, delay, error or defect in transmission, or failure or defect in facilities continues after notice and demand to Company The Company shall not be liable for any act or omission of any connecting carrier, underlying carrier or local exchange company except where Company contracts the other carrier; for acts or omission of any other providers of connections, facilities, or service; or for culpable conduct of the customer or failure of equipment, facilities or connection provided by the customer Claims of Misuse of Service The Company shall be indemnified and saved harmless by the customer against claims for libel, slander, fraudulent or misleading advertisements or infringement of copyright arising directly or indirectly from material transmitted over its facilities or the use thereof; against claims for infringement of patents arising from combining or using apparatus and systems of the customer with facilities of the Company; and against all other claims arising out of any act or omission of the customer in connection with the services and facilities provided by the Company The Company does not require indemnification from the customer where the action for which it is seeking indemnification is based on a claim of negligence by the Company Defacement of Premises The Company is not liable for any defacement of, or damage to, the customer s premises resulting from the furnishing of service or the attachment of equipment and facilities furnished by the Company on such premises or by the installation or removal thereof, when such defacement or damage is not the result of negligence of the Company. For the purpose of this paragraph, no agents or employees of the other participating carriers shall be deemed to be agents or employees of the Company except where contracted by the Company.

10 PSC MD Tariff No. 2 Original Page Facilities and Equipment in Explosive Atmosphere, Hazardous or Inaccessible Locations The Company does not guarantee nor make any warranty with respect to installations provided by it for use in an explosive atmosphere. Company shall be indemnified, defended and held harmless by the customer from and against any and all claims, loss, demands, suits, or other action, or any liability whatsoever, whether suffered, made, instituted or asserted by the customer or by any other party, for any personal injury to or death of any person or persons, and for any loss, damage or destruction of any property, including environmental contamination, whether owned by the customer or by any other party, caused or claimed to have been caused directly or indirectly by the installation, operation, failure to operate, maintenance, presence, condition, location, use or removal of any equipment or facilities or the service and not due to the negligence or willful misconduct of the Company Service at Outdoor Locations The Company reserves the right to refuse to provide, maintain or restore service at outdoor locations unless the customer agrees in writing to indemnify and save the Company harmless from and against any and all loss or damage that may result to equipment and facilities furnished by the Company at such locations. The customer shall likewise indemnify and save the Company harmless from and against injury to or death of any person which may result from the location and use of such equipment and facilities Warranties THE COMPANY MAKES NO WARRANTIES OR REPRESENTATIONS, EXPRESS OR IMPLIED EITHER IN FACT OR BY OPERATON OF LAW, STATUTORY OR OTHERWISE, INCLUDING WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR USE, EXEPT THOSE EXPRESSLY SET FOTHER HEREIN Acceptance of the provisions of Section 2.3 by the Commission does not constitute its determination that any limitation of liability imposed by the Company should be upheld in a court of law Nothing in this tariff shall be construed to limit the company s liability in cases of gross negligence or willful misconduct Limitation of Liability Nothing in this tariff shall be construed to limit the Company s liability in cases of gross negligence or willful misconduct.

11 PSC MD Tariff No. 2 Original Page Application for Service Minimum Contract Period: Except as otherwise provided, the minimum contract period is one month for all services furnished. However, if a new residential or single line business customer notifies the Company within twenty days after receipt of the first bill that certain services or equipment are not desired, the Company will delete such services or equipment from the customer s account without a record keeping or service ordering charge. The customer nonetheless shall be responsible for all monthly usage and installation charges incurred for the use of such service and equipment Except as provided in preceding, the length of minimum contract period for directory listings, and for joint user service where the listing actually appears in the directory, is the directory period. The directory period is from the day on which the directory is first distributed to customers to the day the succeeding directory is first distributed to customers The Company may require a minimum contract period longer than one month in connection with special, non-standard types or arrangements of equipment, or for unusual construction, necessary to meet special demands for service Cancellation of Service 2.5 Payment for Service Where the applicant cancels an order for service prior to the start of the installation or special construction of facilities, no charge shall apply, except to the extent the Company incurs a service order or similar charge from a supplying carrier, if any, prior to the construction Where the installation of facilities, other than those provided by special construction, has been started prior to cancellation, the lower of the following charge applies; (a) The total costs of installing and removing such facilities; or (b) The monthly charges for the entire initial contract period of the service ordered by the customer as provided in this tariff plus the full amount of any applicable installation and termination charges Where special construction of facilities has been started prior to the cancellation, and the Company has another requirement for the specially constructed facilities, no charge applies.

12 PSC MD Tariff No. 2 Original Page Service will be billed on a monthly basis and is due and payable upon receipt or as specified on the customer s bill. Service will continue to be provided until canceled by the customer or discontinued by the Company as set forth in Section 2.15 of this tariff The customer is responsible for payment of all charges for service furnished to the customer. Charges based on actual usage during a month will be billed monthly in the month following the month in which the service was used. All fixed monthly and nonrecurring charges for services ordered will be billed monthly in advance The Carrier will provide direct billing to the customer. Customer s may contact the Carrier at,, Boca Raton, FL 33433, or (888) Advanced payments for installation costs or special construction will credited on the first bill in their entirety. 2.6 Customer Deposits The Carrier agrees to abide by the regulations associated with nonresidential customer deposits as specified by Code of Maryland Regulations as amended from time to time In order to establish credit, the carrier may require an applicant for nonresidential service to demonstrate good paying habits by showing that the applicant: Was a customer of a Maryland utility for at least 12 months within the preceding 2 years; Does not currently owe any outstanding bills for utility service to a utility doing business in Maryland; Did not have service discontinued for nonpayment of a utility bill during the last 12 months that service was provided; and Did not fail, on more than two occasions during the last 12 months that service was provided, to pay a utility bill when it became due The Carrier agrees to abide by the regulations associated with residential customer deposits as specified by Code of Maryland Regulations as amended from time to time In order to establish credit, a utility may require an applicant for residential service to demonstrate good paying habits by showing that the applicant: Was a customer of a Maryland utility within the preceding 2 years; Does not currently owe any outstanding bills for utility service to a utility doing business in Maryland; Did not have service discontinued for non-payment of a utility bill during the last 12 months that service was provided; and

13 PSC MD Tariff No. 2 Original Page Did not on more than two occasions during the last 12 months that service was provided, fail to pay a utility bill when it became due Deposits for establishment or reestablishment of credit will not be more than the estimated charge for service for 2 consecutive billing periods or 90 days, whichever is less Customer deposits shall be maintained in a bank located in Maryland. Customers who make a deposit for service will receive interest, at a rate set on such deposit not less than the rate calculated by the method set forth in COMAR (for non-residential customers) or COMAR (for residential customers) as appropriate. 2.7 Late Payment Charges The Carrier agrees to abide by the regulations governing late payment charges as specified by COMAR as amended from time to time Any charges that are disputed by a customer shall not be subject to late payment charges regardless of the outcome of the dispute The company will consider delinquent and apply late payment charges on bills not paid within 20 days of the billing invoice date in the case of residential customers and within 15 days of the billing invoice date in the case of all non-residential customers in accordance with COMAR Sections A and B, respectively Late payment fees will be computed at a rate not to exceed 1.5% per month, for the two nominal billing intervals and may not exceed 5% of the total original unpaid charges in compliance with COMAR A(1). 2.8 Customer Complaints and Billing Disputes Customers may notify the carrier of billing or other disputes either orally or in writing. There is no time limit for submitting disputes.

14 PSC MD Tariff No. 2 Original Page Customer complaints and billing disputes that are not satisfactorily resolved may be presented by the customer to: Office of External Relations Maryland Public Service Commission 6 St. Paul Street Baltimore, MD (888) (Office of External Relations) (Main PSC number) (Toll-free PSC number) The company will not collect attorney fees or court costs from customers. 2.9 Allowance for Interruptions in Service Credit for failure of service or equipment will be allowed only when failure is caused by or occurs in equipment owned, provided, and billed for, by the Company. The Carrier agrees to abide by the regulations associated with interruptions in service as specified by Code of Maryland Regulations as amended from time to time Taxes and Fees All state and local taxes and fees shall be listed as separate line items on the customer s bill If a municipality, other political subdivision or local agency of government, or the Commission imposes and collects from the Company a gross receipts tax, occupation tax, license tax, permit fee, franchise fee, or regulatory fee, such taxes and fees shall, as allowed by law, be billed pro rata to the customer receiving service from the Company within the territorial limits of such municipality, other political subdivision or local agency of government Service shall not be subject to taxes for a given taxing jurisdiction if the customer provides the company with written verification, acceptable to the company and to the relevant taxing jurisdiction, that the customer has been granted a tax exemption Returned Check Charge The charge for a returned check is $25.

15 PSC MD Tariff No. 2 Original Page Directory Assistance Call Allowance Residential customers shall receive six free directory assistance calls per month with two requests per call. Charges will not be levied for Directory Assistance on an individual who suffers from a physical or visual disability that precludes the use of a telephone directory Special Customer Arrangements In cases where a customer requests special or unique arrangements which may include but are not limited to engineering, conditioning, installation, construction, facilities, assembly, purchase or lease of facilities and/or other special services not offered under this tariff, the company, may provide the requested services. Appropriate recurring charges and/or nonrecurring charges and other terms and conditions will be developed for the customer for the provisioning of such arrangements Termination of Service: Denial of Service Without Notice The Company may discontinue service without notice for any of the following reasons: Hazardous Condition. For a condition on the customer s premises determined by the Company to be hazardous Adverse Effect on Service. Customer s use of equipment in such a manner as to adversely affect the Company s equipment or the Company s service to others Tampering With Company Property. Customer s tampering with equipment furnished and owned by the Company Unauthorized Use of Service. Customer s unauthorized use of service by any method which causes hazardous signals over the Company s network Illegal use of Service. Customer s use of service or equipment in a manner to violate the law Denial of Service Requiring Notice The Company may deny service for any of the following reasons provided it has notified the customer of its intent, in writing, to deny service and has allowed the customer a reasonable time of not less than 10 days, or as otherwise specified in this tariff, in which to remove the cause for denial: A Non-compliance with Regulations. For violation of or noncompliance with regulations contained in Code of Maryland Regulations , or for violation of or non-

16 PSC MD Tariff No. 2 Original Page B C D compliance with the Company s tariffs on file with the Commission. Failure on Contractual Obligations. For failure of the customer to fulfill his contractual obligations for service or facilities subject to regulation by the Maryland Public Service Commission. Refusal of Access. For failure of the customer to permit the Company to have reasonable access to its equipment. Non-payment of Bill D D D.3 For non-payment of a bill for service, provided that the Company has made a reasonable attempt to effect collection and has given the customer written notice of its intent to deny service if settlement of his account is not made and provided the customer has at least 5 days, excluding Sundays and holidays in which to make settlement before his service is denied. In cases of bankruptcy, receivership, abandonment of service, or abnormal toll usage not covered adequately by a security deposit, less than 5 days notice may be given if necessary to protect the Company s revenues. Except in cases where a prior promise to pay has not been kept or bankruptcy, receivership, abandoned service, or abnormal toll usage is involved, the Company may not deny service on the day preceding any day on which it is not prepared to accept payment of the amount due and to reconnect service.

For failure of the customer to furnish the service equipment, permits, certificates, or rights-of-way, specified by the Company as a condition to obtaining service, or if the equipment or permissions are withdrawn or terminated. Failure to Comply with Municipal Ordinances. For failure to comply with municipal ordinances or other laws pertaining to telephone service. Failure to Pay Increased Deposit Required. For failure of the customer to pay an increased security deposit when warranted by the Company to protect its revenue in accordance with Code of Maryland Regulations Insufficient Reasons for Denial of Service The following may not constitute cause for refusal of service to a present or prospective customer: A B C D E F Failure of a prior customer to pay for service at the premises to be serviced; Failure to pay for a different class of service for a different entity; Failure to pay the bill of another customer as guarantor of that bill; Failure to pay directory advertising charges; Failure to pay an undercharge as described in the Code of Maryland Regulations D.(2); or Failure to pay an outstanding bill that is over 7 years old, unless the: F F F.3 (i) Customer signed an agreement to pay the outstanding bill before the expiration of this period; Outstanding bill is for service obtained by the customer by means of tampering with equipment furnished and owned by the Company or by unauthorized use of service by any method; or Outstanding bill is for service obtained by the customer by means of an application made: In a fictitious name,

18 PSC MD Tariff No. 2 Original Page 18 (ii) (iii) In the name of an individual who is not an occupant of the dwelling unit, without disclosure of the individual s actual address, In the name of a third party without disclosing that fact or without bonafide authority from the third party, or (iv) Without disclosure of a material fact or by misrepresentations of a material fact This regulation applies to both residential and nonresidential classes of service.

19 PSC MD Tariff No. 2 Original Page 19 PROVISION OF SERVICE AND FACILITIES 2.15 Unlawful Use of Service Service shall not be used for any purpose in violation of law or for any use as to which the customer has not obtained all required governmental approvals, authorizations, licenses, consents, and permits. The company shall refuse to furnish service to an applicant or shall disconnect the service without notice of a customer when: An order shall be issued, signed by a judge finding that probable cause exists to believe that the use made or to be made of the service is prohibited by law, or The Company is notified in writing by a law enforcement agency acting within its jurisdiction that any facility furnished by the Company is being used or will be used for the purpose of transmitting or receiving gambling information in interstate or foreign commerce in violation of the law If service has been physically disconnected by law enforcement officials at the customer s premises and if there is not presented to the Company the written finding of a judge, then upon written or verbal request of the subscriber, and agreement to pay restoral of service charges and other applicable service charges, the Company shall promptly restore such service Interference with or Impairment of Service Service shall not be used in any manner which interferes with other persons in the use of their service, prevents other persons from using their service, or otherwise impairs the quality of service to other customers. The company may require a customer to immediately shut down its transmission of signals if said transmission is causing interference to others or impairing the service of others.

20 PSC MD Tariff No. 2 Original Page Telephone Solicitation by Use of Recorded Messages Service shall not be used for the purpose of solicitation by recorded messages when such solicitation occurs as a result of unrequested or unsolicited calls initiated by the solicitor by means of automatic dialing devices. Such devices, with storage capability of numbers to be called or a random or sequential number generator that produces numbers to be called and having the capability, working alone or in conjunction with other equipment, of disseminating a prerecorded message to the number called and which are calling party or called party controlled, are expressly prohibited Incomplete Calls There shall be no charge for incomplete calls. No charge will be levied for unanswered calls. Customers will receive credit for calls placed to a wrong number if the customer notifies the Company of the error Overcharge/Undercharge Overcharge/undercharge provisions will be in accordance with COMAR When a customer has been overcharged, the amount shall be refunded or credited to the customer.

21 PSC MD Tariff No. 2 Original Page Trial Services SECTION - 3 DESCRIPTION OF SERVICES The Company may offer new services, not otherwise tariffed, from time to time on a trial basis subject to Commission approval. Such trials are limited to a maximum of six months at which time the trial offering must be either withdrawn or made available on permanent basis. 3.2 Promotional Offerings The Company may offer existing services on a promotional basis, subject to Commission approval, that provides special rates, terms, or conditions of service. Promotional offerings are limited to a maximum of six months at which time the promotional offering must be either withdrawn or made available on a permanent basis. All promotions, regardless of whether services are given away for free, are subject to Commission approval. 3.3 Standard Residence Line A Standard Residence Line provides the Customer with a single, analog, voice-grade telephonic communications channel, which can be used to place or receive one call at a time. Standard Residence Lines are provided for the connection of Customer-provided wiring and single station sets or facsimile machines. An optional per line Hunting feature is available for multi-line Customers which routes a call to an idle station line in a prearranged group when the called station line is busy. 3.4 Standard Business Line The Standard Business Line provides a Customer with a single, analog, voice-grade telephonic communications channel, which can be used to place or receive one call at a time. Standard Business Lines are provided for the connection of Customer-provided wiring and single station sets or facsimile machines. An optional per line Hunting feature is available for multi-line Customers which routes a call to an idle station line in a prearranged group when the called station line is busy. 3.5 PBX Trunk Service Basic PBX Trunk Service provides a Customer with a single, voice-grade telephonic communications channel, which can be used to place or receive one call at a time. Basic Trunks are provided for connection of Customer-provided private branch exchanges (PBX) to the public switched telecommunications network. Each Basic PBX Trunk is provided with touch-tone signaling and may be configured into a hunt group at no additional charge with other Company-provided Basic PBX Trunks. The signal is an analog signal at the DS0 level.

22 PSC MD Tariff No. 2 Original Page Direct Inward Dialing (DID) Service Direct Inward Dialing ( DID ) permits calls incoming to a PBX system or other Customer Premises Equipment to be routed to a specific station without the assistance of an attendant. DID calls are routed directly to the station associated with the called number. DID service as offered by the Company provides the necessary trunks, telephone numbers, and out-pulsing of digits to enables DID service at a Customer s location. DID service requires special PBX software and hardware not provided by the Company. Such hardware and software is the responsibility of the Customer. 3.7 Optional Calling Features The features listed in Section are offered by the Company to Residential and Business Customers. Refer to Price Lists in Sections 4 5, 6 and 7 of this tariff for specific features offered with each type of local exchange service Features Descriptions (A) Flexible Call Forwarding: Provides end-user control for call forwarding capabilities via dial-accessed voice prompt menus. Customers may forward calls to a primary local or long distance. The end-user may specify a secondary location for routing of go unanswered at the forward-to location or reach a busy signal. This secondary location may be another telephone number, pager or voice messaging service. Other capabilities included with this feature include: Speed Forwarding; Priority Screening; Ring Control; and Timed Forwarding. It is the responsibility of the Customer to subscribe to the telephone number, pager or voice messaging service used as the secondary location. (B) Flexible Call Forwarding with Audio Calling Name: Provides all of the functionality of Enhanced Call Forwarding. Also permits the end-user to receive the Directory Name of the party's whose call was forwarded to primary number. In some situations, the enduser may hear the calling party's city and state or telephone number, depending on available call data.

23 PSC MD Tariff No. 2 Original Page 23 (C) Flexible Call Forwarding Plus: Provides all of the functionality of Enhanced Call Forwarding. Also includes an additional telephone number with directory listing and distinctive ringing for calls placed to the additional number. Enhanced Call Forwarding Plus allows parties to reach the end-user s location when FCF is active and all calls to the end-users main telephone number would normally forward. Calls to the additional number do not forward even when Enhanced Call Forwarding is active. (D) Flexible Call Forwarding Plus with Audio Calling Name: Provides all of the functionality of Enhanced Call Forwarding Plus including the additional telephone number with listing and distinctive ringing. Also permits the end-user to receive the Directory Name of the party s whose call was forwarded to primary number. In some situations, the end-user may hear the calling party s city and state or telephone number, depending on available call data. (E) Call Forwarding Variable: Permits the end-user to automatically forward (transfer) all incoming calls to another telephone number, and to restore it to normal operation at their discretion. The end-user must dial an activation code from his/her exchange line along with the forward-to number in order to turn the feature on. A separate code is dialed by the end-user to deactivate the feature. (F) Call Forwarding Variable, Remote Access: Permits the end-user to automatically forward (transfer) all incoming calls to another telephone number, and to restore it to normal operation at their discretion. The end-user must dial an activation code along with the forward-to number in order to turn the feature on. A separate code is dialed by the end-user to deactivate the feature. Feature activation may be performed from the end-user s exchange line or remotely from some other line. Remote access requires the end-user to (1) dial a special access number 2) enter their seven-digit telephone number and 3) enter a personal identification number prior to forwarding their calls. (G) Call Forwarding Don t Answer, Basic: Permits the forwarding of incoming calls when the end-user s line remains unanswered after a pre-designated ringing interval. The ringing interval before forwarding and the forward-to number are fixed by the service order. (H) Call Forwarding Don t Answer w/ring Control: Permits the forwarding of incoming calls when the end-user s line remains unanswered after a pre-designated ringing interval. The forward-to number is fixed by the service order. However, the end-user has the ability to change the time interval before forwarding occurs at his/her discretion.

The ringing interval before forwarding and the forward-to number are fixed by the service order. However, the end-user has the ability to turn the feature on or off at his/her discretion. (J) Call Forwarding Busy Line, Basic: Permits the forwarding of incoming calls when the end-user s line is busy. The forwarded number is fixed by the end-user service order. (K) Call Forwarding Busy Line w/customer Control: Permits the forwarding of incoming calls when the end-user s line is busy. The forwarded number is fixed by the end-user service order. However, the end-user has the ability to turn the feature on or off at his/her discretion. (L) Call Waiting Basic: Call Waiting provides a tone signal to indicate to a Customer already engaged in a telephone call that a second caller is attempting to dial in. It permits the Customer to place the first call on hold, answer the second call and then alternate between both callers. Cancel Call Waiting is provided with the feature and allows a Call Waiting end-user to disable the Call Waiting feature for the duration of a single outgoing telephone call. Cancel Call Waiting is activate by dialing a special code prior to placing a call, and is automatically deactivated when the Customer disconnects from the call.

25 PSC MD Tariff No. 2 Original Page 25 (M) Call Waiting -- Deluxe: Allows the end-user to control the treatment applied to incoming calls while the Customer is off-hook on an existing call. This feature includes the capabilities of Call Waiting Basic plus additional call treatment options. Treatment options offered with Call Waiting Deluxe include: Answer the waiting call and placing the first party on hold; Answer the waiting call and disconnecting from the first party; Direct the waiting caller to hold via a recording Forward the waiting caller to another location (e.g., voice mailbox or telephone answering service) Full utilization of Call Waiting Deluxe requires specialized CPE not provided by the Company. It is the responsibility of the Customer to provide the necessary CPE. The end-user must have Caller ID Basic or Deluxe for display of calling party identification information for waiting calls. The end-user must have a Call Forwarding don t Answer feature active in order to forward a waiting call to another location. (N) Call Waiting Deluxe with Conferencing: Provides all of the functionality of Call Waiting Deluxe. Also permits the end-user to conference a waiting call with an existing call (first party) and, if desired, subsequently drop either leg of the conferenced call. (O) Caller ID Basic: Permits the end-user to view a Directory Number of the calling party on incoming telephone calls. Information is displayed on a specialized CPE not provided by the Company. The feature also provides the date and time of each incoming call. It is the responsibility of the Customer to provide the necessary CPE.

26 PSC MD Tariff No. 2 Original Page 26 (P) Caller ID Deluxe: Permits the end-user to view a Directory Name and Directory Number of the calling party on incoming telephone calls. Information is displayed on a specialized CPE not provided by the Company. The feature also provides the date and time of each incoming call. It is the responsibility of the Customer to provide the necessary CPE. In some situations, the calling party s city and state may be displayed rather than a Directory Name, depending on available call data. (Q) Anonymous Call Rejection: Permits the end-user to automatically reject incoming calls when the call originates from a telephone number which has blocked delivery of its calling number (see Calling Number Delivery Blocking). When active, calls from private numbers will be routed to a special announcement then terminated. The feature may be turned on or off by the end-user by dialing the appropriate feature control code. Anonymous Call Rejection is offered as a stand-alone feature or as an add-on to Caller ID Deluxe. (R) Call Block: Allows the end-user to automatically block incoming calls from up to six enduser pre-selected telephone numbers programmed into the feature s screening list. Callers whose numbers have been blocked will hear a recorded message stating that their call has been blocked. The end-user controls when the feature is active, and can add or remove calling numbers from the feature s screening list. (S) Call Return: Allows the Customer to return a call to the last incoming call whether answered or not. Upon activation, it will redial the number automatically and continue to check the number every 45 seconds for up to 30 minutes if the number is busy. The Customer is alerted with a distinctive ringing pattern when the busy number is free. When the Customer answers the ring, the call is then completed. The calling party s number will not be delivered or announced to the call recipient under any circumstances.

27 PSC MD Tariff No. 2 Original Page 27 (T) Call Selector: Allows a Customer to assign a maximum of 15 telephone numbers to a special list. The Customer will hear a distinctive ring when calls are received from telephone numbers on that list. (U) Call Tracing: Allows the tracing of nuisance calls to a specified telephone number suspected of originating from a given local office. The tracing is activated upon entering the specified dial code. The originating telephone number, outgoing trunk number or terminating number, and the time and date are generated for every call to the specified telephone number can then be identified. (V) Calling Number Delivery Blocking: Prevents the delivery, display and announcement of the end-user s Directory Number and Directory Name on all calls dialed from an exchange service equipped with this option. When active, the end-user s telephone name and number will not appear on the called party s Caller ID CPE or be disclosed in another way. The feature is available on a per call or per line basis. With per call Calling Number Delivery Blocking, it is necessary for the end-user to dial an activation code prior to placing the call. With the per line version of the feature, all calls are placed with the end-user s number blocked. Per line end-users must dial an activation code prior to utilization. (W) Message Waiting Indication: Provides the end-user with an audible (stutter dial tone) or visual (lamp or other CPE display) indication that messages are waiting to be retrieved. Message Waiting Indication can only be activated/deactivated by a voice mailbox or other voice messaging service provided by the Company or third party. It is the responsibility of the Customer to subscribe to a compatible voice messaging service. Visual Message Waiting Indication requires specialized CPE not provided by the Company. It is the responsibility of the Customer to provide the necessary CPE.

28 PSC MD Tariff No. 2 Original Page 28 (X) Multiple Directory Number Distinctive Ringing: This feature allows an end user to determine the source of an incoming call from a distinctive ring. The end user may have up to two additional numbers assigned to a single line (i.e. Distinctive Ringing First Number and Distinctive Ringing Second Number). The designated primary number will receive a normal ringing pattern; other numbers will receive distinctive ringing patterns. The pattern is based on the telephone number that the calling party dials. (Y) Preferred Call Forwarding: Permits the end-user to automatically forward to another number calls received from up to six end-user pre-selected telephone numbers programmed into the features screening list. The end-user controls when the feature is active, the forward-to-number and can add or remove calling numbers from the feature s screening list. (Z) Repeat Dialing: Permits the end-user to have calls automatically redialed when the first attempt reaches a busy number. The line is checked every 45 seconds for up to 30 minutes and alerts the Customer with a distinctive ringing pattern when the busy number and the Customer s line are free. The Customer can continue to make and receive calls while the feature is activated. The following types of calls cannot be reached using Repeat Dialing: Calls to 800 Service numbers Calls to 900 Service numbers Calls preceded by an interexchange carrier access code International Direct Distance Dialed calls Calls to Directory Assistance Calls to 911

29 PSC MD Tariff No. 2 Original Page 29 (AA) Speed Calling: Permits the Customer to place calls to other telephone numbers by dialing a one or two digit code rather than the complete telephone number. The feature is available as either an eight (8) code list or a thirty (30) code list. Code lists may include local and/or toll telephone numbers. The Customer has the ability to add or remove telephone numbers and codes to/from the speed calling list without assistance from the Company. (AB) Three Way Calling: Permits the end-user to add a third party to an established connection. When the third party answers, a two-way conversation can be held before adding the original party for a three-way conference. The end-user initiating the conference controls the call and may disconnect the third party to reestablish the original connection or establish a connection to a different third party. The feature may be used on both outgoing and incoming.

30 PSC MD Tariff No. 2 Original Page Listing Services For each Customer of Company-provided Exchange Service(s), the Company shall arrange for the listing of the Customer s main billing telephone number in the directory(ies) published by the dominant Local Exchange Carrier in the area at no additional charge. At a Customer s option, the Company will arrange for additional listings for an additional charge Non-Published Service This optional service provides for suppression of printed and recorded directory listings. A Customer s name and number do not appear in printed directories or Directory Assistance Bureau records Non-Listed Service 3.9 Directory Assistance This optional service provides for suppression of printed directory listings only. Parties may still obtain the Customer s number by calling the Directory Assistance Bureau. Provides for identification of telephone directory numbers, via an operator or automated platform. Customers are provided with a maximum of 2 listings per each call to Directory Assistance.

31 PSC MD Tariff No. 2 Original Page Miscellaneous Services Main Number Retention Main Number Retention is an optional feature by which a Customer, who was formally a customer of another certified local exchange carrier at the same premises location, may retain its main telephone numbers and main fax numbers for use with the Companyprovided Exchange Services. Main Number Retention service is only available in areas where the Company maintains some form of number retention arrangement with the Customer s former local exchange carrier Pay Per Call Blocking/Unblocking This service provides the option of blocking, or subsequent unblocking, all 900 and 976 calls on a per line basis. The Company will provide for per-line blocking where the Company s switching facilities permit Vanity Number Service This service provides for the reservation of special or unique telephone number and fax number for use with the Company-provided exchange services Presubscription Services This service provides for the Presubscription of local exchange lines provided by the Company to the intralata and interlata long distance carrier(s) selected by the Customer.
