3 Cold chain management The essential reference is the current edition of the National Vaccine Storage Guidelines: Strive for 5 The supplementary reference is the NHMRC Australian Immunisation Handbook 7 Essential steps Monitor the temperature, and formally record the minimum and maximum temperatures twice daily when the practice is open Ensure your fridge is reliable, maintains a stable temperature, and is an appropriate size Document your processes Provide appropriate training to all relevant staff, including what to do in case of a temperature breach 8 Safe vaccine storage Cold chain is the system of transporting and storing vaccines within the safe temperature range of +2 C to +8 C This starts at manufacture, and continues through until vaccine administration A breach is the existence of vaccines outside the recommended temperatures for more than 15 minutes, at temperatures beyond +12 C 9

4 Storage tips Store vaccines in their original manufacturer s packaging to protect them from temperature fluctuations and UV light Clearly label containers, and do not crowd the vaccines Keep fridge door openings to a minimum Consider fixing a map of the vaccine positions to the outside of the fridge to assist with quick access Do not store other items within the vaccine fridge 10 Why Strive for Five? Striving for five refers to the point midway between +2 C to +8 C, the temperature range at which vaccines should be maintained Vaccines are sensitive to temperature, with most destroyed by freezing, and many heat sensitive There needs to be confidence that the vaccines administered to patients are effective and potent Vaccines are also expensive, and thus should be treated with care 11 Types of refrigerators: purpose built Purpose-built fridges are preferred, but not mandatory These fridges have features to ensure vaccines are maintained at the appropriate, and stable, temperature These fridges are also expensive, but are worthwhile if your practice administers vaccines 12

5 Types of refrigerators: other Domestic fridges are designed for food and drink storage, not the strict temperature maintenance of vaccines Temperatures vary across the areas within the unit, certain areas are inappropriate for storage, and fluctuations in temperature take longer to recover from Ensure that appropriate measures are in place if you use a domestic fridge, including the use of additional packaging to maintain temperatures, no storage in the fridge door, and the use of water bottles and ice packs for temperature variation buffers Lastly, do not store food, drinks or other items in the fridge 13 Vaccine management protocols A trained person overseeing cold chain A trained support person A handover process Appropriate training for all staff involved in handling vaccines Systems for reporting cold chain breaches, equipment maintenance issues, and power failures Back up storage options All processes documented 14 Monitoring and recording temperatures Minimum / Maximum Temperature Chart Location of Refrigerator Month and Year +16 Dat e Temperature ( Celsius) Temperature ( Celsius) Date 1-4 Time Time Initials Initials Note: GPA ACCREDITATION plus has provided this document as a guide only you may add or delete items as relevant to your practice. 12-MKW Temperature must be formally recorded twice daily, every day that the practice is open, noting the minimum and maximum temperature Logically, this should happen when the practice opens, and before the practice closes A logbook or chart should be used, and each vaccine storage unit needs its own record Data loggers are useful, but can not be used in place of the manual recordings Ensure you also check state and territory legislation for requirements around temperature monitoring 15

6 Monitors Data loggers measure temperatures at defined intervals and record temperatures over a period of time. They can can identify cold spots, and create a temperature map of the fridge They are usually stand-alone units connected to a computer for data extraction, however some fridges have inbuilt data loggers Thermometers must be able to give you minimum and maximum reads, and should be reset after each temperature recording Make sure your thermometer reads in celsius, not fahrenheit Thermometers can also be used for offsite vaccine temperature monitoring 16 Maintenance Your vaccines fridge(s) should be included on your maintenance schedule Routinely check that it is not leaking, that it operates quietly, and that it closes and seals When performing routine cleaning, move the vaccines to an alternative fridge, or to a suitable cooler storage Ensure your thermometer is working properly, and change the batteries at least annually (or perhaps with daylight saving, if you have that change) Have data loggers calibrated 17 Managing power failures Practices that keep vaccines must have a back-up plan to ensure vaccines can be properly maintained in the event of a power failure This may be done using a back-up power supply, an offsite monitored fridge (with an agreement in place), or a cooler Test the back-up plan to ensure the arrangements are sufficient, for example, the cooler can accommodate all the vaccines, vaccines can be transported offsite in a controlled environment 18
