1 # 66 Go West Tours Fill = Green 204 SAFE WORK PROCEDURE Workplace: Page: 1 of 6 TASK / OPERATION: Forklift Truck Petrol/Diesel Overall Risk Rating After Controls 4 Acute 3 High 2 Moderate 1 Low Personal Protective Equipment Fill = Green 204 Foot High Visibility Head Protective Clothing Hearing Eye Hand Normal Requirements: Safety footwear, high visibility shirt or vest, hard hat and sun protection if required (broad brim hat, sun screen, tinted safety glasses), Protective clothing. As Required: Hearing protection, eye protection (goggles), hand protection (gloves). Safety Notes Fill = Green 204 Check local regulation for training requirements. Generally: Trainees are permitted to operate equipment provided they are under direct supervision and maintain a logbook. Nationally uniform Certificates of Competency issued by States are automatically recognised. Certificates do not have to be changed over to work interstate. Queensland: Requires trainees to undergo an accredited course (30496QLD Course in operating load shifting equipment) supervised by a

Fitting of personal protective equipment. Manual handling. Operating instructions by an authorised person. Review manufacturer s operation manual. Site Awareness Struck by. Work area: Is to be inspected to identify hazards and appropriate prevention/control measures are to be implemented. Appropriate paths are to be determined for the movement of loads and equipment/vehicles. Barricades and/or signs should be in place for the protection of site and visitors. Note: Forklifts, by their design, limit the operator s vision. This must be taken into account when traffic movements are considered. Pedestrians must be kept at a safe distance from working forklifts. Preparation Incorrect operation on forklift. Review: Log book. Manufacturer s operational guide/owners manual. 3 Sequence Pre Start Daily Checks Incorrect operation of forklift. Understanding of start up/shut down procedure. Damage to the forklift. Damaged attachments. Oil and hydraulic fluid levels. Water. Brake operation. Operation of lights (if fitted). Hazards warning systems, for example horns and audible reversing alarms are functional. Operating of emergency controls and any other safety devices. Report any defects. Check attachments where applicable: Extended forks for damage and load rating. Person lift cage. Drum Lifter / Clamp. Forks spacing positions. Entering and seating Personal injury Slips and falls. Sprain and strain injuries from poor driving position. Mounting and dismounting: Do not hold steering wheel or levers. 4 Operation Sequence Struck by. Damage to forklift. Roll over and unexpected movements. Damage to plant. Struck by. 3H Note: Seatbelt is to be worn at all times. If a forklift overturns, the safest place for the operator is in the cabin with a seatbelt on. Ensure: Material is shifted using appropriate equipment such as drum lifters. Weight of load is compatible with the safe working limit for the forklift and attachments. Loads are placed so that they are stable. While operating: Tilt mast slightly backwards. Travel with load close to the ground. Operate smoothly. Travel slowly when turning. Don t turn on inclines. Ensure that there is enough room for the tail to swing around. Keep clear of upright mast and carriage lift chains. If a warning light comes on STOP the forklift truck IMMEDIATELY and seek assistance to rectify. Operation on roads Damage to person and property. 3H Forklift must be: Operated in accordance with statutory regulations at all times. All speed limits, obligatory and recommended, must be observed. 5 Refuelling Sequence Burns 3H Fuel tank must be refuelled by the operator by the end of the shift. Fire and fuel leaks or spillages. There are NO naked flames in the vicinity. Tank gauge before refuelling. Tank for any possible damage. Earth lead has been connected when refuelling. That there is no over spill. Lid has been secured and any vapour residue has been wiped away. On completion Struck by Damage to forklift. Roll away. Ensure that forklift is shut down in accordance with the manufacturer s instructions. Park in suitable location from danger areas. Lower forks/attachments to ground. Apply motion locks (where fitted) and breaks. Stow any used equipment correctly. (Example pallets, ropes, person lift cages etc.) General Maintenance Increased down time. Check tool boxes for missing items. Grease as per manufacturers instructions. Tool boxes for missing items. Equipment for defects. If defects are found segregate and report. Grease as per manufacturers instructions.
