7 Original Page 7 APPLICATION OF PRICE LIST This price list contains the regulations and rates applicable to interexchange common carrier communications service provided by Global Crossing Telecommunications, Inc. (hereinafter "the Company") between and among the points listed herein for service within the state of Florida.

8 Original Page 8 PRICE LIST FORMAT A. Sheet Numbering - Sheet numbers appear in the upper right corner of the page. Sheets are number sequentially. However, new sheets are occasionally added to the price list. When a new sheet is added between sheets already in effect, a decimal is added. For example, a new sheet added between sheets 14 and 15 would be B. Sheet Revision Numbers - Revision numbers also appear in the upper right corner of each page. These numbers are used to determine the most current sheet version. For example, the 4th revised sheet 14 cancels the 3rd revised sheet 14Consult the Check Sheet for the sheet currently in effect. C. Paragraph Numbering Sequence - There are nine levels of paragraph coding. Each level of coding is subservient to its next higher level: A A.l A.1.(a) A.1.(a).I A.1.(a).I.(i) A.1.(a).I.(i).(1) D. Check Sheets - The check sheet lists the sheets contained in the price list, with a cross reference to the current revision number. 9 Original Page 9 SECTION 1 - TECHNICAL TERMS AND ABBREVIATIONS Access Line - A dedicated arrangement which connects a Customer location to a Company NSC (see below for definition). Administrative Change - The modification of an existing circuit, dedicated access line or port, at the request of the Customer, that involves changes in authorization codes, speed numbers, route guide, consolidation of billing, verification of testing performed by parties other that, or any other administrative change not covered by a Billing Record Change. (See below for definition) Application for Service - A standard Company order form which includes all pertinent billing, technical and other descriptive information which will enable the Company to provide the communication service as required. Authorization Code - A multi-digit code, one or more of which are available to Metered Use Service Customers to enable them to identify individual users or groups of users, and thereby allocate the costs of their long distance service; except where Equal Access Dialing is used by the Customer and costs are allocated on the basis of the telephone number from which the Customer uses the service. Authorized User - A person, firm, corporation or other entity authorized by the Customer to receive or send communications. Such permission includes, without limitation, 1) the use of the Customer's Authorization Code where the Authorization Code has been made known to the user by the Customer, its employees, or agents and 2) the use of Customer's telephone number when Equal Access Dialing is used. Billing Record Change - A change in Customer billing address or a change from one billing package to another using the same access method. Cancellation of Order - A Customer initiated request to discontinue processing a service order, either in part or in its entirety, prior to its completion. Cancellation charges will be assessed for each dedicated access line canceled from an order prior to its completion by the Company under the following circumstances: If the local Telco has already confirmed in writing to the Company that the dedicated access line will be installed; or If the Company has already submitted facilities orders to an interconnecting telephone company. (This differs from a Disconnection, see below for definition).

10 Original Page 10 SECTION 1 - TECHNICAL TERMS AND ABBREVIATIONS, (Cont d) Circuit Termination - The point at which the Company's circuit originates, terminates, or drops for the insertion or removal of a Customer's signal. Company - Global Crossing Telecommunications, Inc. unless otherwise indicated by the context. Customer - The person, firm, corporation or other entity which orders service, including the ordering or use of Equal Access Dialing, either for its own use, as an Other Common Carrier, or as a non-profit manager of a sharing group, and which is thereby responsible for the payment of charges and for compliance with the Company's price list regulations. For billing purposes, a Customer is considered to be an account. If a Customer orders an authorization code or orders or uses Equal Access Dialing, in more than one Company originating city, or requests the assignment of more than one Authorization Code in a particular city, each such account is a separate Customer for billing purposes. Disconnection - The disconnection of a dedicated access line or port connection being used for existing service. (This differs from a Cancellation; see above for definition.) Equal Access Dialing - The use of service through Telco Equal Access facilities allowing the Customer to access the Company's NSC through either a 1+ or dialing sequence. Customers using Equal Access Dialing may be billed to the telephone number from which the Equal Access Dialing originates. Installation - The connection of a dedicated access line or port for new or additional service. Joint User - A person, firm, or corporation designated by the Customer as a user of communication facilities furnished to the Customer by the Company, and to whom a portion of the charges for such facilities are billed under a joint user arrangement. National Holidays - Company -recognized National Holidays are New Year's Day, Fourth of July, Labor Day, Thanksgiving Day and Christmas Day. Network Switching Center (NSC) - Any location where the Company has a network switch installed.

11 Original Page Description of Service SECTION 2 - RULES AND REGULATIONS The Company is an interexchange common carrier furnishing communications services between specified locations under the terms of this price list The Company, when acting at the Customer's request, and as his/her authorized agent, will make reasonable efforts to arrange for service requirements which may include terminal equipment and circuit conditioning or Equal Access Dialing. 2.2 Limitations Service is offered subject to the availability of facilities and the provisions of this price list The Company reserves the right to immediately discontinue furnishing service, upon written notice, when necessitated by conditions beyond its control or when the Customer is using the service in violation of the provisions of this price list, or in violation of law. 2.3 Terms and Conditions Service is provided and billed on the basis of a minimum period of at least one month, beginning on the date that billing becomes effective, and continues to be provided until canceled, by the Customer, in writing, on not less than thirty (30) days notice. In the event a Customer orders a Company option that does not require a monthly subscription fee, the Company reserves the right to treat a period of non-use of not less than 90 days as a cancellation and to discontinue furnishing service, without notice, after such ninety (90) day period Service is offered on a monthly basis For the purpose of computing charges in this price list, a month is considered to have thirty (30) days The name(s) of the Customer(s), including members of a sharing group, desiring to use the service must be stipulated in the application for service.

12 Original Page Liability SECTION 2 - RULES AND REGULATIONS, (Cont d) The liability of the Company, for whatever reason, shall not exceed an amount equivalent to the proportionate charge otherwise payable for the period of time during which any mistake, omission, interruption, delay, error or defect, or any other event giving rise to a claim, occurs. In no event shall the Company be liable for special, consequential or incidental damages The Company shall be indemnified and held harmless by the Customer against: a) claims for libel, slander, or infringement of copyright arising out of the material, data, information, or other content transmitted over the Company's channels; and b) patent infringement claims arising from combining or connecting the Company's furnished channels with apparatus and systems of the Customer; and c) all other claims arising out of any act or omission of the Customer in connection with any service provided by the Company The Company is not liable for any act or omission of any other company or companies furnishing a portion of the service.

13 Original Page Liability (Cont d) SECTION 2 - RULES AND REGULATIONS, (Cont d) The Customer indemnifies and holds the Company harmless from any and all loss, claims, demands, suits or other action, or any liability whatsoever, whether suffered, made, instituted or asserted by the Customer or by any other party or persons, for any personal injury to or death of any person or persons owned by the Customer or others, caused or claimed to have been caused directly or indirectly by the installation, operation, failure to operate, maintenance, removal, presence, condition, location or use of such equipment, where such installation operation, failure to operate, maintenance, condition, location or use is not the direct result of the Company's negligence The Company is not liable for any defacement of, or damage to, the premises of a Customer resulting from the furnishing of channel facilities or the attachment of instruments, apparatus, and associated wiring furnished by the Company on such Customer's premises or by the installation or removal thereof when such defacement or damage is not the result of the Company's negligence. No agents or employees of other participating carriers shall be deemed to be agents or employees of the Company The Customer is responsible for taking all necessary legal steps for interconnecting his Customer-provided terminal equipment or communications systems with Company facilities. Customer shall secure all licenses, permits, rights-of-way, and other arrangements necessary for such interconnection The Customer shall ensure that his equipment and/or system is properly interfaced with 2.5 Cancellation of Service by a Customer If a Customer cancels an order for service before the service begins, before completion of the minimum period, or before completion of some other period mutually agreed upon by the Customer and the Company, portions of expenditures or liabilities incurred expressly on behalf of the Customer by the Company and not fully reimbursed by installation and monthly charges. If, based on an order by a Customer, any construction has either begun or been completed, but no service provided, the nonrecoverable cost of such construction shall be charged to the Customer.

14 Original Page Use of Service SECTION 2 - RULES AND REGULATIONS, (Cont d) The services offered herein may be used for one or more of the following: a) for the transmission of communications to or by the Customer; b) for the transmission of communications to or by an authorized user or joint user Service may be arranged for joint use or authorized use. The joint user or authorized user shall be authorized to use such service in the same manner as the Customer, but subject to the following: a) One joint user or authorized user must be designated as the Customer. The designated Customer does not necessarily have to have communications requirements of his own. The Customer must specifically name all joint users or authorized users in the Application for Service. Orders which involve the start, rearrangement, or discontinuance of joint use or authorized use service will be accepted by the Company only from that designated Customer and will be subject to all regulations in this price list. b) All charges for the service will be computed as if the service were to be billed to one Customer and any discount arrangements will be based on the actual number of circuits installed at the beginning of the billing period. The Customer shall be responsible for allocating charges to each joint user or authorized user.

15 Original Page Use of Service (Cont 'd) SECTION 2 - RULES AND REGULATIONS, (Cont d) Service may be arranged for use by Other Common Carriers for the purposes of resale subject to the following: a) Other Common Carriers will be responsible for charges, costs, etc. incurred by the Company with respect to Services as referenced in Section 4. b) Other Common Carriers are responsible for all interaction and interface with their own subscribers or Customers Service shall not be used: a) for any unlawful purpose; b) for any purpose for which payment or other compensation is received by the Customer, except when the Customer is a duly authorized and regulated common carrier. This provision does not prohibit an arrangement between the Customer, authorized user or joint user to share the cost of the service so long as this arrangement generates no profit for anyone participating in a joint use or authorized use arrangement A Customer who resells or rebills any services contained in this price list is required by law to have a Certificate of Public Convenience and Necessity as an interexchange carrier issued by the Florida Public Service Commission. 2.7 Payment Arrangements The Customer is responsible for payment of all charges for services furnished to the Customer or any authorized user through Customer's Authorization Code or Equal Access Dialing telephone number. Charges for installation, physical or administrative changes, or for cancellation of orders are payable upon completion. Recurring charges are billed in advance. Estimated usage charges are determined by the Company based on the Customer's projected use of service or at the Company's option, on the highest previous monthly usage charges incurred by the Customer.

16 Original Page Payment Arrangements, (Cont'd) SECTION 2 - RULES AND REGULATIONS, (Cont d) Bills are payable upon receipt. Interest at the rate of 1½ percent per month (unless prescribed by law, in which event at the highest rate allowed by law) will accrue upon any unpaid amount commencing 20 days after rendition of billing Applicants or Customers whose financial condition is not acceptable to the Company, or is not a matter of general knowledge, may be required at any time to make a deposit not to exceed an amount equal to two times the average bill during the preceding six (6) month period for services at the Customer's premises, or if such information is not available, up to an amount equaling the installation charges, if applicable, and/or up to two months' estimated charges for the service to be provided. In the case of deposit, interest at a rate established by the Florida Public Service Commission will be paid for the period during which the deposit is held by the Company. Credit will be deemed established and any deposit refunded (or credited to the Customer's final bill) with interest if the applicant has been a Customer of any telephone utility within the last 24 months during the last 12 months of service did not have more than two (2) occasions in which a bill was paid after becoming delinquent, paid with a NSF check, been disconnected for nonpayment or used service in a fraudulent or unauthorized manner. [The Company may require a new or existing Customer to increase the amount of a deposit previously taken in accordance with the Florida Public Service Commission Rules as set forth in Section ]. The Company may require, upon reasonable written notice of not less than 15 days, a new or additional deposit in order to secure payment of current bills. However, when the Company has good reason to believe payment by a nonresidential Customer is in jeopardy and toll usage provided by or billed by the LEC is significantly above normal for that Customer's, the Company may request a new or additional deposit within 48 hours. If the deposit requested is not paid within 48 hours, the Company may discontinue service. Should the Company determine that credit information given by a Customer is false or material incorrect, the Company may immediately require the Customer to pay a new or additional deposit equal to the deposit which would have been charged at the time of the request for service had the correct credit information been given by the Customer The charges set forth in this price list for circuit terminations contemplate installation made in normal locations and under normal working conditions. Any installation to be made under other circumstances are subject to additional charges, which will be price listed as appropriate If notice of a dispute as to charges is not received, in writing, by the Company within three months after an invoice is rendered, such invoice shall be deemed to be correct and binding upon the Customer.

17 Original Page Change in Service Agreement SECTION 2 - RULES AND REGULATIONS, (Cont d) When a change in service arrangement involves the continued use by the Customer of circuits furnished by the Company, installation charges do not apply to the circuits continued in use. The minimum service period for the circuits contained in use is determined from the date of the initial installation thereof. 2.9 Restoration of Service The use and restoration of service in emergencies shall be in accordance with Part 64, Subpart D of the Federal Communications Commission's Rules and Regulations, which specifies the priority system for such activities Inspection The Company may, upon reasonable notice, make such tests and inspections as may be necessary to determine that the requirements of this price list are being complied with in the installation, operation or maintenance of the Customer or Company equipment. The Company may interrupt the service at any time, without penalty to the Company, because of departure from any of these requirements Cancellation for Cause by the Company (See Section 2.5 for Cancellation by Customer) Upon nonpayment of any sum owing to the Company, or upon a violation of any of the provisions governing the furnishing of service under this price list, the Company may, upon written notification to the Customer, without incurring any liability, immediately discontinue the furnishing of such service Testing and Adjusting Upon reasonable notice, the circuits provided by the Company shall be made available to the Company for such tests and adjustments as may be necessary to maintain them in satisfactory condition; no interruption allowance will be granted for the time during which such tests and adjustments are made Local Charges In certain instances, the Customer may be subject to local telephone charges or message unit charges in using the Company's service. The Company is not responsible for any such local charges imposed directly on the Customer by the local telephone company for gaining access to the Company's intercity network.

18 Original Page Taxes and Surcharges SECTION 2 - RULES AND REGULATIONS, (Cont d) Customer will be billed for and is liable for payment of all applicable federal, state, and local taxes, surcharges or other assessments including such amounts as the Company may be authorized to pass through to the Customer Special Rules for the Handicapped Directory Assistance There will be no charge for up to fifty (50) calls per billing cycle from lines or trunks serving individuals with disabilities. The Company will charge the prevailing price list rates for every call in excess of fifty (50) within a billing cycle Hearing and Speech Impaired Persons Intrastate toll message rates for TDD users shall be evening rates for daytime calls and night rates for evening and night calls Telecommunications Relay Service For intrastate toll calls received from the relay service, the Company will, when billing relay calls, discount relay service calls by fifty (50) percent off of the otherwise applicable rate for a voice nonrelay call. Where either the calling or called party indicates that either party is both hearing and visually impaired, the call will be discounted sixty (60) percent off of the otherwise applicable rate for a voice nonrelay call. The above discounts apply only to time sensitive elements of a charge for the call and shall not apply to per call charges such as credit card surcharge.

19 Original Page 19 SECTION 3 - DESCRIPTION OF SERVICE 3.1 General Description of the Company's Services: The Company's services are offered to subscribers on a monthly basis All service shall remain in effect for a minimum of thirty (30) days The Company offers its services subject to the availability of the necessary facilities and/or equipment. The Company reserves the right to refuse to provide service to or from any location where the necessary facilities and/or equipment are not available.

20 Original Page Metered Use Service General Description SECTION 3 - DESCRIPTION OF SERVICE, (Cont'd) Metered Use Service offers the use of intercity communication facilities shared among multiple users. The individual Customer's basic monthly charges for its use of these intercity communication facilities are based upon the total time the Customer utilizes such facilities from each originating location and can vary, depending upon the option chosen, with the distance of each call. Unless otherwise specified, calls will be measured from the end of the three second measurement point of the the Company's switch immediately following the time that the called party answers to the end of the three second measurement point of the Company's switch immediately following the time that either the calling or called party disconnects from the call, as indicated by answer supervision. No charge is made for calls not completed. It is never the Company's policy to charge a Customer for incomplete calls. A Customer may dispute any charge by contacting the Company's Customer Service Department and proper credits will be issued if, upon investigation, the disputed charges are found to be incorrect. For billing purposes, the duration of each call will be rounded as specified in the description of each service option in the following sections. The rates per minute charged for service are set forth in Section 4. A monthly listing of each individual call and its duration is available as a standard. In order to utilize the Company's facility intrastate, a subscriber must be a subscriber under the Company's corresponding interstate service. Appropriate rates and charges are publically posted on the Company's website and apply for all options and services other than intrastate calling. Rates for intrastate calling are governed by the rates set forth in Section 4. Customers may experience busy signals in less than one percent of all calls Service Options The Company metered use intrastate calling is a one-way dial up or dedicated access multipoint service whereby the Customer originates and terminates calls via Company provided local business telephone lines, dedicated access lines, or via Allnet Option A Travel Service. The rates for this service are contained in Section 4.

21 Original Page Metered Use Service, (Cont'd) Allnet Option 1 SECTION 3 - DESCRIPTION OF SERVICE, (Cont'd) Allnet Option 1 is an outbound, dial-up multipoint service whereby the Customer originates and terminates calls via Company provided local business telephone lines or Allnet Option A Travel service. Subscribers to the Company's service may originate calls by dialing the access number of the Company's NSC switch or through Equal Access Dialing. The rates for this service are contained in Section Allnet Maxcess I Allnet Maxcess I is an outbound, multipoint service designed for very high volume users. Maxcess I Customers may originate calls via voice grade special access or high capacity (T1) access arrangements provided by the Customer between the Customer's premise and the Company's switch. The Company reserves the right to limit service availability to Customers with a minimum requirement for 17 voice grade equivalent channels. The Customer's per minute usage rates are based upon the airline distance between the originating and terminating locations of each call and are contained in Section 4. Calls are measured in six (6) second increments. Any fraction of an increment is rounded up to the next whole increment. A monthly recurring charge is assessed per originating Company switch assessed. Recurring and nonrecurring charges for the dedicated access facilities associated multiplex, and Customer premises equipment will apply as set forth in the appropriate local exchange company's tariff and the Company's posted interstate rates, term and conditions, and/or the Customer's Service Agreement or Contract.

22 Original Page Metered Use Service, (Cont'd) Allnet Maxcess II SECTION 3 - DESCRIPTION OF SERVICE, (Cont'd) Allnet Maxcess II is an outbound, multipoint service designed for high volume users. Maxcess II Customers may originate calls via voice grade special access or high capacity (T-1) access provided by the Customer between the Customer's premises and the Company's switch. The applicable per minute rates are contained within Section 4 and are based on the airline distance between the originating and terminating locations of each call. Calls are measured in six (6) second increments with an eighteen second minimum for each call. Any fraction of an increment is rounded up to the next whole increment. All applicable recurring and nonrecurring charges for Maxcess II will be billed by the local telephone company in accordance with the charges set forth in its applicable tariff Allnet Maxcess III Allnet Maxcess III is an outbound, multipoint service whereby the Customer originates calls via dedicated facilities or by presubscribing long distance service to the Company. Calls can be completed from the Customer's premises to any location within the state of Florida. The rates for this service are contained in Section 4. Maxcess III calls are measured in six (6) second increment with a minimum usage of one minute per call. Any fraction of an increment is rounded up to the next whole increment.

23 Original Page Metered Use Service, (Cont'd) Allnet Premier SECTION 3 - DESCRIPTION OF SERVICE, (Cont'd) Allnet Premier is an outbound, intrastate, multipoint service, used only in conjunction with Allnet's Premier interstate offering. It is designed for users who are not restricted as to time of usage. Allnet Premier Customers may originate calls by dialing 1 plus an area code and the desired telephone number; or by dialing 10XXX then an area code and the desired number. The rates for Allnet Premier are set forth in Section 4 and are based on the originating and terminating locations of each call. Calls are billed in six (6) second increments, with a one minute minimum for each call. Any fraction of an increment is rounded up to the next whole increment Allnet Premier Elite Allnet Premier Elite is an outbound, intrastate, multipoint service, used only in conjunction with Allnet's Premier Elite interstate offering. It is designed for medium to high volume users of equal access service. Allnet Premier Elite Customers may originate calls by dialing 1 plus an area code and the desired telephone number, or by dialing 10XXX then an area code and the desired number. The rates for Allnet Premier Elite are contained in Section 4 and are based on the originating and terminating locations of each call. Calls are billed in six (6) second increments, with a one minute minimum for each call. Any fraction of an increment is rounded up to the next whole increment.

Calls can be completed from the Customer's premise to any location within the state of Florida. Customers may originate calls via dedicated access or by dialing 1 plus an area code and the desired telephone number, or by dialing 10XXX plus one then an area code and the desired number. Charges for Allnet Vantage are assessed on a usage basis. The Customer's total monthly usage for Allnet's Vantage is charged at applicable rates set forth in Section 4 which are based on the airline distance between the originating and terminating locations of each call. There will be a thirty (30) second minimum for each call made using Allnet Vantage. The thirty (30) second minimum is applicable to all Allnet Vantage calls that occur during Business Hours or Off Hours and the duration of the call is measured in six (6) second increments. Intrastate Allnet Solution is available for use only in conjunction with interstate Allnet Solution service offerings. Customers reselling or rebilling intrastate Solution service must have a certificate of Public Convenience and Necessity as an IXC from the Florida Public Service Commission. Allnet Solution provides Customers with a single per minute nondistance sensitive usage rate for all inbound and outbound usage. Solution Customers may elect any combination of the following Solution service features: Solution I - LEC-provided dedicated access Solution II - LEC-provided switched access, high volume usage Solution III - LEC-provided switched access, low and medium volume usage Solution IV - LEC-provided switched access, high volume usage, short duration calls Residential Solution - LEC-provided switched access, high volume off-hour usage Solution provides an add-on inbound capability for Solution I, II, III, IV, and Residential Solution features Customers electing the Solution I feature may originate or receive calls via LEC-provided dedicated access lines. Usage Charges The Customer's total monthly use of Allnet Edge service is charged at the applicable rates per minute and service hours set forth in Section 4 of the price list. The Multipoint 800 Customer is billed for the calls rather than the call originator. Multipoint 800 intrastate service is only available in conjunction with Multipoint 800 interstate service. Multipoint 800 service completes calls to a Company-assigned 800 telephone number. The PIRNS entered by the caller determine the Customer-designated telephone number to which the 800 call will terminate. Each Customer may request any combination of the four digit PIRNS, excluding the PIRNs reserved for special use by the Company. Multipoint 800 service employs shared 800 telephone numbers and, by conversion to Multipoint 800 service, the Multipoint 800 Customer releases any ownership or exclusive rights of its 800 telephone number to the Company. Under the non-validated option, Multipoint 800 Customers may receive inbound Multipoint 800 calls even if the calling party enters no PIRN or an incorrect PIRN. Under the validated option, the Multipoint 800 Customer may receive Multipoint 800 inbound calls only when the correct PIRN is entered by the calling party. The service hours applicable to Multipoint 800 service are set forth in Section 4. A. Allnet Spectrum service is only available in conjunction with the Company's interstate Spectrum service offering. As a PIRN-based product that can be shared among Customers, Customers are not granted exclusive use of the 800 number used for accessing the Allnet Spectrum service and thus, may not continue to use the 800 telephone number upon cancellation of their Allnet Spectrum service. To use Allnet Spectrum service, the caller dials the 800 telephone number for the Allnet Spectrum service. The caller may then (1) enter a PIRN which routes the call to a Customer predesignated telephone number (PIRN CALL); or (2) enter a PIRN which permits the Customer to then direct dial a telephone number (DIAL TONE PIRN). Certain PIRNS are reserved for use by the Company under Allnet Spectrum service or for accessing other services. A.
