Vendors are asked to replace the before mentioned pages with the revised pages that follow. Bidders must acknowledge receipt of this Addendum by completing and returning to the Procurement Office, by no later than the time and date of the bid/proposal opening. Failure to do so may subject the bidder/proposer to disqualification. The Department will decide if orders are to be shipped via box or pallet based on Warehouse/Distribution needs. Orders for quantities fitting 5 or more pallets will be shipped on pallets via Freight Company unless otherwise requested by the Department.
