1 ADDENDUM # 2 Invitation to Bid (ITB): ITB-15-CA ITB Title: Archived Record Storage and Management Services - Southeast Region C- 15 Palm Beach County and C-19 Indian River, Martin, Okeechobee and St Lucie Counties Date: November 13, 2014 This addendum makes changes to the ITB (new in red text) and includes revisions to the following pages: 4, 15, 42, 71, 76, 77, 86, and adds pages 76a and 77a. 1.1 Introduction to the Procurement The Department of Children and Families, Southeast Region is in need of one or more vendors who will manage archive record storage and record management services to include but not limited to: storage, retrieval, delivery, transportation, indexing, re-filing, special search, destruction of records and an automated on-line database. The selected vendor(s) will provide this service under a contract(s) designed to keep record management services within one or more service areas: Circuit 15 (Palm Beach County), and/or Circuit 19 (Indian River, Martin, Okeechobee and St. Lucie counties). Any person interested in submitting a bid must comply with any and all of the terms and conditions described in this Invitation to Bid (ITB) Bid Price Sheet and Project Budget The prospective vendor(s) shall submit one signed original copy of each bid price sheet under separate sealed cover (see Appendix IV Financial Cost Proposal) with their bid proposal which must identify if the bid is for Circuit 15 and/or Circuit 19 and include the Proposed Project Summary Budget and Line Item Detail and Narrative which identifies all costs used in calculating the bid price. This should be submitted in the format specified in Appendix IV. B-8 Contract Limits B-8.3 The Provider guarantees and agrees not to bill the Department for permanent withdrawal of records/files in the event of contract expiration, nonrenewal and/or contract termination. The Provider further agrees to deliver to the Department, all records created under this contract along with a total storage accumulation printout and electronic file within 30 calendar days in the event of contract expiration, nonrenewal and/or contract termination.

2 APPENDIX III VENDORS STATEMENTS AND CERTIFICATIONS a. Acceptance of Contract Terms and Conditions I,, as the authorized representative of, hereby agree that if awarded any contract as a result of the Department of Children and Families Invitation to Bid, it will comply with the requirements, terms, and conditions stated in the Invitation to Bid and in the State of Florida, Department of Children and Families Standard Contract. In recognition thereof, the vendor s representative has read, understood, and agrees that the vendor will comply with the terms and conditions of the ITB. The vendor further agrees that any intent by the vendor to deviate from the terms and conditions set forth therein may result, at the Department s exclusive determination, in rejection of the reply. These should be customers who have purchased archived record management and storage services from your firm within the last two years and must include dates of service.
