1 GUIDELINES FOR COMPLETING THE OVERSIZE/OVERWEIGHT PERMIT APPLICATION Contents Applying For An Oversize/Overweight Permit Instructions For Completing The Oversize/Overweight Permit Application Submitting and Saving the On-line Oversize/Overweight Permit Application Applying For An Oversize/Overweight Permit You can apply for an Oversize/Overweight permit by: 1. Completing the Application Form On-line, or 2. Printing a Blank Form to apply in person, by fax or by mail to a Permit Issuing Office. See Office Locations section of the Guide to Oversize/Overweight Vehicles and Loads in Ontario for a list of locations. When paying by credit card, you must complete the Credit Card Authorization Letter and send by fax or mail to the same Ministry of Transportation (MTO) Permit Office, where you submitted your application. When submitting the application on-line, indicate the Permit Application Tracking Number (located on page three of the on-line application form) when faxing or mailing supporting documentation (e.g. the Credit Card Authorization Letter) to: Ministry of Transportation O/O Permit Office, 3 rd Floor, 301 St. Paul St., St. Catharines, ON, Canada, L2R 7R4, Fax: (905) Technical Information To apply on-line, your computer must have the following: Adobe Acrobat Reader Version 6, and Access to with a valid account. Adobe Acrobat Reader Version 6 can be downloaded free from the Adobe Web site. Additional documentation can be scanned and provided electronically, formatted as: bmp, tif, jpg, pdf, doc, ppt, xls. The on-line Oversize/Overweight Permit Application Form has been tested with Netscape Navigator 6 and MS Internet Explorer 5.5.

2 Instructions For Completing The Application Form These instructions will assist you in completing each section of the Oversize/Overweight Permit Application. When completing the on-line application form, complete the sections in order, starting at section 1. Mandatory information requirements are identified with an asterisk (*). Section 1. Application For: Check appropriate box for permit type required. For a renewal, replacement or amendment, provide previous permit number and a copy of permit. If application is for renewal only, complete sections 1, 2a, and 9. Provide additional information if changes to permit are required. Indicate the total number of identical permits required. Check applicable box to indicate how you would like to receive your permit(s). Check applicable box to indicate method of payment or prepaid MTO account number (if applicable). All personal cheques must be certified unless payment is sent by mail. When paying by credit card, you must complete the Credit Card Authorization Letter and send by fax or mail to the Ministry of Transportation (MTO) Permit Office (see address above). To protect financial information, please do not send the Credit Card Authorization Letter electronically. Section 2(a). Applicant Information: Enter applicant's Commercial Vehicle Operator's Registration (CVOR) number. NOTE: Carriers without a CVOR must contact: Ministry of Transportation, Client Services 301 St. Paul Street, 3 rd Floor, St. Catharines, ON L2R 7R4 Check applicable box if permit is required in bilingual (French) format. Enter applicant's complete head office address and telephone number, company fax number, contact name, address (if applicable) and other fax number (to send single trip permits only). Section 2(b). Agent Information: NOTE: This section is to be completed by the Permit Agency ONLY, if application is made on behalf of the carrier. Enter agency s company name and complete address and telephone number, fax number, contact name and address (if applicable). Section 3. Proposed Movement Information: Enter the START date of the proposed move and the anticipated END date of the move. Start date must be within 30 days from the application submission date. Enter all Ontario border-crossing locations, if applicable. Enter the ORIGIN of the move, (complete address is required). Enter the DESTINATION of the move, (complete address is required). Enter the desired travel route in full, including all municipal and county roads. NOTE: MTO permits are only valid on King s highways. The carrier is responsible for acquiring all municipal permits before commencing a move. The indicated travel route will be verified to ensure that all King s highways listed are correct and that construction activities do not impact the travel route including, width, height or weight restrictions. If requesting travel on the 407 Electronic Toll Route (ETR), you must indicate by checking the applicable box and provide the Transponder serial number, tractor and trailer licence plate numbers.

3 Enter the range of kilometres to be travelled for the proposed move. (NOTE: This applies to overweight single trip permits on King's highways only). Enter Bill of Lading number, if applicable. Check the applicable box to indicate if gross weight is within Highway Traffic Act (HTA) Legal Limits. For information on acceptable limits, please refer to the Guide to Oversize/Overweight Vehicles and Loads in Ontario, section HTA Weight & Dimension Limitations. Section 4. Is This Request To: Check applicable box to identify how load will be transported. Section 5. Exemption From Regulation 614: If an exemption from the Highway Traffic Act (HTA), Regulation 614, Security of Loads, is required, check the applicable box and provide a letter and diagram explaining your reasons for the exemption. NOTE: The exemption only applies to HTA Regulation 614 and not to Section 111(2) which states: "No person shall operate or permit to be operated upon a highway any motor vehicle or trailer unless the load that the vehicle or trailer is carrying is firmly bound, sufficiently covered, or otherwise secured or loaded, so that no portion of the load may become dislodged or fall from the motor vehicle or trailer". Section 6. Equipment Information: Check the applicable boxes for the type of equipment being used. Enter the number of axle(s) for the equipment being used, next to each applicable box. Using metric measurements only, indicate: o Vehicle (tractor) length, o Trailer width, o Trailer length, o Manufacturer s rated capacity of trailer, and o Registered gross vehicle weight. Vehicle Configuration Table: (Only required if requesting weight privileges in excess of HTA) Enter Ministry Issued Configuration Number, or Provide a diagram indicating number of axles, tires per axle, tire size, tire ratings, axle spacing and weight required per axle, or Complete the Vehicle Configuration Table. Use metric measurements where applicable. Axle Number Indicate the sequential axle number of the vehicle configuration beginning at the steer axle. Number of Tires/Axles Indicate the number of tires per axle (range from 2 to 16 tires per axles). Tire Rating (in kilograms) Indicate the manufacturer's rated capacity of tires on each identified axle (range from 2000 kg kg). Tire Size (Width in mm) Indicate the tire size on each identified axle (range from 200 mm to 600 mm). Requested Weight Per Axle (in kilograms) Indicate weight required per axle, range from 1000 kg to kg. Axle Spacing (in metres) Indicate all axle spacing of the configuration (range from 0.91 m to 20 m). NOTE: Provide a separate diagram for vehicles in excess of 9 axles, indicating number of axles, tires per axle, tire size, tire ratings, axle spacing and weight required per axle.

4 When completing the Vehicle Configuration Table on-line: The number of columns to be completed in the Vehicle Configuration Table is based on the total number of axles provided in the Equipment Section. The following information will be displayed in the Vehicle Configuration Table: number of tires; steer axle (defaulted to 2 axles and all remaining axles defaulted to 4). tire rating is defaulted to 3000 kg. tire size is defaulted to 279 mm. NOTE: Defaulted information should be reviewed and changed based on your vehicle configuration. Section 7. What Is Being Moved: Enter details about the load/commodity that is being transported. If a self-propelled vehicle is being used, provide the make, model and serial number. Check applicable box if load is destined for disassembly and/or scrap. Enter the metric measurements for the load/commodity, including, length, width, height and weight. If applicable, enter the overhang measurements from the centre of the rearmost axle. NOTE: All mobiles (homes, construction or office trailers), conveyors, stackers and screening plants must indicate overhang measurements. Section 8. Overall Dimensions Of Vehicle Or Vehicle Combination Including Load: Provide the final overall dimensions of the configuration (once the load/commodity has been loaded onto the tractor or tractor/trailer combination). Metric measurements must be provided. Section 9. Declaration: Applicant must provide name, company position/title and the date of application. Authorized applicant must provide signature*, indicating the applicant has read, understood and accepted the conditions of the declaration and has authorization to make such declaration on behalf of the carrier. *Providing a signature on the on-line form: Select the [Submit On-line] button to display the [I Accept] button that will be displayed near the Signature of Authorized Applicant. The [I Accept] button, replaces a standard authorized signature. Selecting the [I Accept] button indicates the applicant has read, understood and accepted the conditions of the declaration and has authorization to make such declaration on behalf of the carrier. Submitting and Saving the On-line Oversize/Overweight Permit Application Section 10. Support Documentation Required: (On-line O/O Permit Application Form Only) When completing the on-line application form, provide all additional documentation identified on page three, in the Mandatory Supporting Documentation section. Other supporting documents to be provided will be listed in the Support Documents section. Additional information can also be provided in the section titled use the following text area to provide additional information. Additional documentation can be provided electronically by submitting scanned files. (See Technical Section for acceptable format) To attach additional documentation see Section 11, Submitting Your On-line Application Form.

5 NOTE: If additional documentation is required, please ensure the Application Tracking Number (see Section 11, Submitting Your On-line Application Form) is written on the documentation prior to faxing or mailing. Submitting Your On-Line Application Form: After entering your address in the space provided, select the [Submit Now] button. Your unique Permit Application Tracking Number will appear below the [Submit Now] button. The Permit Application Tracking Number should be referenced on all additional documentation provided or when contacting the Ministry of Transportation Permit Office for information regarding your permit application. After selecting the [Submit Now] button, your window will be displayed. The Ministry of Transportation s O/O Permit Office address will appear in the To field and the address provided on page three of the application form, will appear in the Cc field. The completed O/O Permit Application will appear as an attachment to your . Additional supporting documentation can be included as file attachments to your . After selecting the [SEND] button, the completed application and attached supporting documentation will be sent to the Ministry of Transportation O/O Permit Office. A copy of the e- mail will be sent to your address for your records. Section 12. Printing And Saving Your On-Line Permit Application Form: A copy of the on-line permit application form can be printed by selecting the printer icon located on the tool bar at the top of your screen, or by selecting the [Print] button on page 2 of the form. Once the [Print] button is selected, any errors or missing information will be identified for correction. After completing the application, you can save the document to your computer from your e- mail attachment, either before sending your application to MTO or from the copy you receive. To save the completed application form, select the application attachment from the screen and by clicking the right mouse button (when using a mouse configured for right hand use). Select Save As from the list displayed. Select an appropriate file save location then select Save. Saving the completed application will allow you to re-use the form when requesting additional permits. Information can be edited by opening the saved application, making changes and resubmitting the application on-line. You can save your new application by following the above instructions.
