Persn-centered planning is a way f helping undervalued peple, such as peple with intellectual disabilities, get what they need and want in their lives. (Hlburn, Grdn, & Vietze, 2007) Gals f Persn-Centered Planning 1. Cmmunity presence: lives, wrks, and plays in typical places 2. Cmmunity participatin: is amng ne s netwrk f friends and relatins 3. Chice: is able t make decisins abut everyday matters 4. Respect: has a valued rle in cmmunity life 5. Cmpetence: is able t d the things that cntribute t their success (Hlburn, Grdn, & Vietze, 2007) PATH Principles 1. Peple with disabilities shuld live like peple withut disabilities 2. The pathfinder is at the center f planning 3. See the whle persn 4. Start where yu are; use what yu have 5. PATH is a team prcess 6. Team members need t make a persnal cmmitment 7. 3 3 The 8 Steps f PATH The first thing the graphic recrder des is t draw the abve frmat n a big piece f pster paper abut 1 x 3 meters (3 x 10 feet). 4 4 Step 1: Tuching the Dream Lcate yur Nrth Star What key wrds and images express yur dreams? What wrds and images capture yur Nrth Star directin? What values will help yu evaluate yur chices as yu travel n yur Path? What are yu passinate abut? What gives yur life meaning? What are yur strengths and talents? What nurishes yur sul? What ideals d yu mst want t realize n yur Path? Imagine there are n barriers t achieving yur dreams! But I being pr, have nly my dreams. I have spread my dreams under yur feet; Tread sftly, because yu tread n my dreams. ~ William Butler Yeats Ntes t the facilitatr: Invite the pathfinder t rient the path t their dream. If dreams seem unrealistic, try nt t blck them. Even if it isn t evident hw the dream can be nurtured int a practical utcme, listen deeply fr what is behind the wrds, and give the dream time and space t grw. Ask the pathfinder t share the key feeling wrds assciated with their Nrth Star. Summarize the Nrth Star and ask the pathfinder t cnfirm the summary is accurate. Tips fr the graphic recrder: As peple speak, capture their key wrds and images n the chart. Use the pathfinder s wn wrds (d nt paraphrase). Recrd pictures and images, whenever pssible; use wrds t clarify. Use bld clrs like black, blue, green, and red t recrd what peple say. Add the feeling wrds/images t the recrd f the Nrth Star.

5 5 Step 2: Sensing the Gal Generate a visin f a psitive pssible future Chse a date by when yu want t have imprtant results (abut 1 year). Imagine the date has arrived: What have yu dne t get there? See, hear, taste, smell, and tuch yur accmplishments. Translate yur accmplishments int gal(s) that represent yur Nrth Star. Create SMART gals: S = Specific M = Measurable A = Achievable R = Relevant T = Time-framed (Andersn & Heyne, 2012) Yu see things and yu say, Why? But I dream things that never were and I say, Why nt? ~ Gerge Bernard Shaw Ntes t the facilitatr: Invite the pathfinder t travel frward in time t vividly imagine what they want t create. Keep asking fr details t add clarity t the pathfinder s visin f their future. Ask the pathfinder t share the key feeling wrds assciated with the future they are creating. Summarize the pathfinder s sense f success. Get cnfirmatin frm the pathfinder that yur summary is accurate. Check if anything is missing frm the drawings and, if s, add it. Tip fr the graphic recrder: Add the pathfinder s feeling wrds/images assciated with sensing their gal.

6 6 Step 3: Grunding in the Nw Prvide a snapsht f the present. Where are yu nw? Describe the current situatin as accurately and hnestly as pssible. Be bjective and factual. One must have chas within neself in rder t give birth t a dancing star. ~ Nietzsche Ntes t the facilitatr: Ask the pathfinder t share the key feeling wrds assciated with the Nw. As a final step, ask the pathfinder t quietly review the recrd f Nw. Summarizes the Nw and check fr accuracy. Talk abut the differences between hw the pathfinder feels tday and ne year frm nw. Watch the clck. The first tw t three questins take abut half the available time. Tips fr the graphic recrder: Cntinue t add wrds and images t recrd the pathfinder s respnses. Add the pathfinder s feeling wrds/images related t the Nw.

7 7 Step 4: Inviting Peple t Enrll Identify/invite peple wh yu want invlved in yur PATH prcess: Family Friends Significant ther Staff and service prviders (teacher, scial wrker, aide, recreatin therapist, etc. ) Wh shares and will strengthen yur cmmitment t yur Nrth Star? What cntributin can each persn make t the future yu wish t create? Highlight the peple yu feel cmfrtable appraching. Muntain climbers always help each ther. ~ Tenzing, Munt Everest climber Ntes t the facilitatr: Encurage the pathfinder t identify specific peple by name wh they believe share their cmmitment. Ask the pathfinder t identify the peple they feel cmfrtable appraching. If the pathfinder des nt have access t a persn they have identified, ask ther team members if anyne can help make cntact with the persn. Tip fr the graphic recrder: Cntinue t capture the pathfinder s wrds, feelings, and images n the recrd.

8 8 Step 5: Recgnizing Ways t Build Strength What d yu need t get strng and stay strng as yu mve alng yur Path? What knwledge d yu have and what knwledge d yu need t reach yur Nrth Star? What skills d yu have and what skills d yu need t develp? What relatinships d yu need t maintain r develp? What cmmunity resurces can help yu achieve yur dreams? Hw can yu stay healthy and well as yu wrk tward creating what yu want? Independence? That s middle class blasphemy. 9 9 Step 6: Charting Actin fr the Next Few Mnths Take actin (d it) and learn frm it (reflect): Review Step 1 (Nrth Star) and Step 2 (gal images) and cnsider all f their aspects. What are the mst imprtant steps t take in the next 3 t 6 mnths? Identify specific actin(s) tward achieving the gal(s). Check if actins are cnsistent with the dream (Nrth Star). Identify actins that seem mst pssible t d first. Security is mstly a superstitin. It des nt exist in nature, nr t the children f men as a whle experience it. Aviding danger is n safer in the lng run that utright expsure. Life is either a daring adventure r nthing. ~ Helen Keller Ntes t the facilitatr: Ask the pathfinder t check that the actins they have chsen are cnsistent with their Nrth Star. Summarize the pathfinder s ideas and check fr accuracy.

10 10 Step 7: Planning the Next Mnth s Wrk Specify wh will d what, by when: What needs t be dne t accmplish the gal(s)? Wh will d it? By what day f the mnth?
