SPONSOR(S): WMVP (AM) and it s licensee, Sports Radio Chicago, LLC (Hereinafter collectively referred to as Sponsor ) and Nestle USA, Inc. and the Chicago Bulls, (collectively, the Prize Sponsor ). II. ELIGIBILITY: Entrants must be 21 years of age or older and must be legal residents of Illinois, Indiana, Wisconsin or Michigan who reside within 75 miles of the United Center at time of entry. Any individuals (including but not limited to employees, consultants, independent contractors, and interns) who have, within the past six months, performed services for Nestle and Sport Radio Chicago L.L.C., Chicago Professional Sports Limited Partnership, BSV Limited Partnership, CBLS Corp., Chicago Bulls Limited Partnership, Sponsor any organizations responsible for sponsoring, fulfilling, administering, advertising or promoting the contest or supplying the prize, and/or their respective parent, subsidiary, affiliated and successor companies, and immediate family and household members of such individuals, are not eligible to enter or play. "Immediate family members" shall mean parents, step-parents, children, step-children, siblings, step-siblings, or spouses. "Household members" shall mean people who share the same residence at least three months a year. III. HOW AND WHEN TO ENTER: NO PURCHASE NECESSARY. One (1) winner will be randomly selected from all eligible entries on or about Tuesday, November 4, 2013 by 11:59 PM CT. All text message entries must be received by the end of the Sweepstakes Period. One entry, per cell phone number, or per household allowed per day. Invalid entries will not be acknowledged, returned, or eligible for further consideration. The use of automated entry devices of any kind is prohibited. Prize Sponsor and Administrator are not responsible for lost, late, or indecipherable entries. All entrants must have a text messaging two-way capable handset and digital service in order to enter the Sweepstakes using the text messaging method. Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, modify or suspend the contest. If canceled, Sponsor reserves the right at its discretion to determine the winner using all non-suspect eligible entries received up to the time of cancellation. On-air method of entry During the Sweepstakes Period, participant may tune into the Carmen & Jurko Show weekdays (Monday-Friday) from 10A-2P CT and Waddle & Silvy Show weekdays (Monday-Friday) from 2P-6p for their chance to win Chicago Bulls tickets. At times during the aforementioned shows, a Bulls Highlight will be broadcasted; participants must place a call to (312) (the Hotline ). The winner will be the number of caller that directly corresponds to the jersey or uniform number of the Chicago Bulls player that was featured in the highlight broadcasted on air. Winner must provide his or her name, age, address and phone number. There will be a total of 10 winners the during the Sweepstakes Period. Methods of entry are limited to call-in, and/or texting. Opportunities to enter via these methods will be announced during the same period. Only calls to the specified phone number during the specified time can win. Sponsor is not responsible for inability of caller to complete call within specified time, whether due to busy lines, disconnections, telephone malfunctions or other circumstances. There is no limit on the number of calls that a caller may place to the Hotline in an attempt to win a prize. However, callers should not call the Hotline after the highlights are broadcasted for promotional purposes, such as, the advertising of this Sweepstakes, or a demonstration of the highlight effects. Callers cannot win a prize at these times. The Released Parties (as defined in VI. Releases below) are not responsible for failed, partial or garbled computer transmissions, or for technical failures of any kind, including but not limited to electronic malfunction or damage of any network, hardware or software. If for any reason the sweepstakes is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or other causes that corrupt or affect the administration, security, fairness, integrity or proper conduct of the sweepstakes, Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the sweepstakes. No responsibility is assumed for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries; or any problems or technical malfunctions of any telephone network or lines, computer online systems, servers, or providers, computer equipment, software, failure of any or electronic entry to be received on account of technical problems or traffic congestion on the Internet or at any

3 Web-site, or any combination thereof, including any injury or damage to entrant s or any other person s computer related to or resulting from participation in or downloading any materials from this sweepstakes. In the event of a dispute over an electronic entry, prize will be awarded to the owner of the account, not the name on the entry form. Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, modify or suspend the contest. In the event than an error occurs and more than the designated number of prizes are claimed, the designated number of prizes will be awarded in a second round drawing from among eligible claimants. PRIZE(S) On air Prizes: Ten (10) winners: Winners will receive a pair of tickets to the December 7, 2013 home Chicago Bulls vs. Detroit Pistons game at the United Center. Approximate retail value ($300). On-Air entrants must be available to attend the December 7, 2013 Chicago Bulls vs. Detroit Pistons game at the United Center. No other costs associated with attending the game, including but not limited to transportation, parking and food/beverages, are included in prize. All federal, state and local taxes are the responsibility of the winner. Text to win Prize: One (1) Grand Prize winner: One winner will receive a trip for two (2) to Los Angeles to see the Chicago Bulls vs. L.A. Clippers on November 24, 2013,. Winner and one (1) guest will receive round-trip, economy class airfare, roundtrip, from Chicago to Los Angeles, hotel accommodations at the Four Seasons Beverly Wilshire, 9500 Wilshire Blvd., Beverly Hills, CA and two (2) tickets to the Chicago Bulls game in Los Angeles. All travel arrangements will be made at Prize Sponsor s sole discretion. WINNER AND GUEST MUST BE AVAILABLE TO TRAVEL TO ATTEND NOV. 24 CHICAGO BULLS GAME, no other tickets, time or arrangements are available. Winner s guest must be age 21 or older at time of winning prize, or the child of winner. ARV of the prize: $3,750 Text entrants must be available to travel to Los Angeles, CA for the Nov. 24 Chicago Bulls vs. Los Angeles Clippers game, travel likely to occur between November 22 and November 25, Winner and guest must travel on the same itinerary. All expenses not specifically provided for herein are the winner s sole responsibility. Some restrictions may apply. Except with respect to the airfare and hotel accommodations for grand prize winner or tickets for on-air winners set forth above, travel, parking, meals, drinks, gratuities, souvenirs, incidental personal expenses and all other expenses not specified herein are the responsibility solely of winner.

5 If the actual value of the prize is less than the stated ARV, the difference will not be awarded. Federal, state and local taxes, if any, are the sole responsibility of the winner, who will receive an IRS Form 1099 reflecting the final actual value of any prize valued at $600 or more. Winners are not entitled to exchange or transfer prizes or to obtain cash or other substitutes, but Sponsor in its sole discretion may substitute prizes of equal or greater value. The Prize Sponsor reserves the right, in its discretion, to substitute for any Prize to be awarded in the Sweepstakes, another prize of equal or greater value or a check in the amount of the ARV for the Prize being substituted. Prize is subject to availability. Prize Sponsor is not responsible for cancellation or preemption of show/contest/event/meeting or for other circumstances that make it impractical or impossible to fulfill this element of the prize. If it becomes impractical or impossible to fulfill this element, Sponsor s only obligation will be to provide the remainder, if any, of the prize. VI. RELEASES By participating, entrants agree to be bound by these official rules and the decisions of the Sponsor/Judges and the Administrator, which shall be final in all respects. By participating in the contest and/or accepting any prize that they may win, entrants agree to release, Nestle USA, Sports Radio Chicago, LLC, Chicago Professional Sports Limited Partnership, BSV Limited Partnership, CBLS Corp., Chicago Bulls Limited Partnership, and their respective parent, subsidiary, affiliated and successor companies, advertising and promotion agencies and prize suppliers, and each of their respective officers, directors, agents, representatives and employees, as well as each of their respective successors, representatives and assigns (collectively, the Released Parties ) from any and all actions, claims, injury, loss or damage arising in any manner, directly or indirectly, from participation in the contest and/or acceptance or use of the prize. Entrants authorize the Released Parties to use their name, voice, likeness, biographical data, city and state of residence and entry materials in programming or promotional material, in any media now known or hereafter created or conceived, worldwide in perpetuity, or on a winner s list, if applicable, without further compensation unless prohibited by law. Sponsor is not obligated to use any of the above mentioned information or materials, but may do so and may edit such information or materials, at Sponsor s sole discretion, without further obligation or compensation. By submitting entry materials, entrants certify that such materials are original and created by entrant, that entrants have the necessary rights, permission and authority to submit such materials, and, if applicable, that entrants maintain a valid copyright in the materials. Entrants acknowledge that such materials may be included and/or discussed in a broadcast, that such discussion may include statements or opinions by the broadcast talent or others about the entrants, the entrants appearances and/or the materials, and that those statements and opinions may be considered surprising, embarrassing, defamatory or otherwise offensive or injurious to them. Entrants release

6 the Released Parties from any actions, claims, injury, loss or damage allegedly caused, directly or indirectly, by any such statements or opinions made during a broadcast. To claim prize winner must: (a) provide proof of winner s age, residency, and legal status; (b) sign and return eligibility, publicity, liability, rules compliance and any other releases requested by Sponsor sign and return releases requested by Sponsor; all within the time period(s) specified by Sponsor. VII. MISCELLANEOUS; WINNERS LIST Void where prohibited. All federal, state and local laws and regulations apply. Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process and to cancel, terminate, modify and/or suspend the contest in the event of tampering or other circumstances, including but not limited to acts of God or civil disturbance that render it impractical or impossible to complete or fulfill the contest as originally planned.
