P.U.C. OR. No. 18 Tariff, reflecting an effective date of April 12, Competitive Response 4th Revised Sheet 3 The purpose of this filing is to remove the 12-month renewal clause from the Regional Value and Regional Essentials Retention and Winback Offer. Beginning April 12, 2009, customers who renew this offer will renew with their initial term discounts on a month-to-month basis. 2 4th Revised Sheet 3 3rd Revised Sheet 3 Regional Value and Regional Essentials Retention and Winback Offer GENERAL The Regional Value and Regional Essentials Retention and Winback Offer provides discounted offers for customers who are either disconnecting service from Verizon or returning to Verizon local service for competitive reasons, or responding to a Verizon marketing offer they received. CONDITIONS Qualifying residential customers are those who call Verizon and either: (a) Indicate that they intend to disconnect their Verizon primary or additional lines, and subsequently agree to retain their service, or (b) Indicate an interest in changing their local service from another service provider to Verizon and subsequently do so as a result of this offer, or (c) Change their local service from another provider to Verizon and within 30 days of that change call Verizon and specifically request these discounts as a response to direct mailings, advertising or other Verizon marketing activities. (d) Have Verizon Online high speed internet service or FiOS Internet service but no local service from Verizon Northwest and accept this offer in response to a direct mailing, advertising, or other Verizon marketing activity. Customers participating in this offer are not eligible to participate in other Verizon Northwest Inc. discount offers or promotions on the same dial-tone line. It is limited to one discounted offer per customer line, not to exceed two lines billed on the same bill. The offers are not available to employees of Verizon. The offers are not transferable or redeemable for cash. If prior to the end of the 12-month term the customer removes the qualifying product from their account, the customer will lose the associated discount. After the initial twelve (12) month term, customers will automatically renew at the renewal rates specified herein on a monthto-month basis. Qualifying unlimited long distance calling plans must be consistent with the Plan O Service - Unlimited rates, terms, and conditions as found in the Bell Atlantic Communications, Inc D/B/A Verizon Long Distance Product Guide.
