1 REQUEST FOR QUOTATION (RFQ) RFQ/2016/05/03 Date: May 29, 2016 Dear Sir / Madam: We kindly request you to submit your quotation for the construction services in order to improve butcher shop condition, details of which are in part-3 of this RFQ. The prices quoted for the work shall be all inclusive costs such as customs, transportation to the location, insurance, packing, labeling, installation, testing inclusive of all taxes direct or indirect, or any other costs, or fees that may be necessary to pay. b. Services proposed shall be reviewed based on completeness and compliance of the quotation with the specifications provided in part-3. c. The quotation that complies with all of the specifications and requirements and offers the lowest price shall be selected. Any offer that does not meet the requirements shall be rejected. d. Any discrepancy between the unit price and the total price (obtained by multiplying the unit price and quantity) shall be re-computed by ADWSO. The unit price shall prevail and the total price shall be corrected. If the vendor does not accept the final price based on ADWSO s re-computation and correction of errors, its quotation will be rejected. e. ADWSO is not bound to accept any quotation, nor award a contract/purchase Order, nor be responsible for any costs associated with your preparation and submission of a quotation, regardless of the conduct or outcome of the selection process. f. ADWSO reserves the right to accept or reject any Proposal, to render any or all Proposals as non-responsive, and to annul the solicitation process and reject all Proposals at any time prior to award of contract, without thereby incurring any liability to the affected Proposer, or any obligation to inform the affected Proposer(s) of the grounds for ADWSO s action. ADWSO shall neither be obliged to award the contract to the lowest price offer. g. Prior to the expiration of proposal validity, the contract may be awarded to the Proposer whose proposal is determined to be in the best interests of the Agency, based upon the evaluation method indicated in the Data Sheet. 3/6 RFQ/2016/05/03

The window should made of square or rectangular tubes steel (30 mm x 60 mm x 18 gage) There are 100 shops one window with approximate size of 9 square meters. The costs include supply and installation of door lock and the glass, galvanizing, painting and installation of the window in target provinces and districts. Please provide your price per square meter. There are 100 shops in the target area and each shop floor size would be 3x3 meter and the wall size 2.4x11 meter to be paved by tile and ceramic. The cost includes the tile and ceramic cost, skill and unskilled labor, mortar and/or any other costs incurred for paving the tile and ceramic in the target provinces and districts. 100% plastic emulsion. There are 100 shops in the target provinces and each shop would be different in size. The approximate size of the shop would be around 36 square meters and most of the shops not painted yet may need maintenance to be prepared for painting. Please provide your price per shop meter. 6 Note: All above supplies and services are delivered to 100 butchers shop in the 5 target provinces and its associated districts which is listed in Part-1 of this RFQ. The supplier should consider all associated costs like installation and transportation to destination with supply of items listed.
