Attention Directors: PLEASE READ EVERYTHING!! ****ALL THE RECOGNITION information is due by October 5, 2012.**** All directors and consultants must pay their registration fee by September 10, 2012! This information must be sent to Rita Klais by September 20th, If space permits from Sept. 10th 30th, 2012 the price will increase to $127 There will be a Director s meeting on Friday, October 19th, 2012 with Lisa, Gloria and Linda 12:00 4:30 p.m. Cost for the Sales Director meeting is $10 at the door if you are not attending the retreat. No lunch will be served. Please make sure to send your information on the proper forms. Also, please print your consultants names CLEARLY so that we have correct spellings. Please make checks payable to MADA or Propay Lisa Madson by transferring funds to nsdlisapp@gmail.com with an explanation of what event your payment is for. (For example: 5 people for Smith Unit, Madson Retreat ) Have your consultants propay you or write checks to you (the director) and you send MADA ONE check or Propay ONE amount for you and your Consultants. Consultants should book accordingly. Pick up times 2:00 pm, 5:00 pm, and 8:30 pm. We will have ONE shuttle from Milwaukee to the hotel on Oct. 19th. Pickup is 4:30 p.m. It is on a first come, first served basis. (14 total on the shuttle) WE need names of people riding on the shuttles to reserve a spot. Shuttles will be filled a first come, first served. Please Jeanne the names and flight information. 7 Hotel Room Reservation Instructions: PLEASE READ!!!! Once you select your roommates for the retreat, please fill out the attached form. Each reservation will need to be made on or before September 15, Credit cards will be charged for 1 night s stay the day you make the reservation. If you are unable to attend the retreat, you are still responsible for your part of the hotel room expense. Each person must check in at the hotel and show ID and credit card. ALL RESERVATIONS MUST GO THROUGH THE HOTEL Reservations must be faxed to the hotel You may check in anytime after 4:00 p.m. Pre-check in time starts at 10:00 a.m. You may begin using the water park as early as 10:00 a.m. by picking up your wristbands at the front desk. Check-out time is at 11:00 a.m. You may continue to use the water park until 9:00 p.m. that same day. Remember that all reservations will be made through the Kalahari. They will charge your credit card the day you make the reservation for 1 night.
