The Cngenital Heart Defect Calitin (CHD Calitin) is a charitable nnprfit rganizatin whse sle purpse is t imprve the quality f life fr children and adults brn with heart disease, als knwn as a cngenital heart defect r CHD. The three main pillars f ur missin are fcused n uniting and supprting the CHD cmmunity; increasing public awareness; and raising mney fr lifesaving medical research. A significant differentiatr between us and similar rganizatins is ur cmmitment t research. Innvative research and emerging medical technlgies ffer an enrmus impact n the survival and lng-term care f individuals affected by heart disease. Unfrtunately, research specific t CHD has been seriusly underfunded due t a lack f widespread natinal awareness. While research ffers the huge ptential fr imprvement and preventin f CHDs, prgrams that supprt the cmmunity and hspitals deliver an immediate benefit t the families and children. Fr this reasn, the Cngenital Heart Defect Calitin is a very unique rganizatin that understands the imprtance f cntributing t bth. What is the CHD Awareness Walk? The Cngenital Heart Defect Awareness Walk & Family Fun Day, r CHD Awareness Walk fr shrt, is much mre than a fundraiser. It is a magnificent event that allws families and friends t publicly rally in supprt f their special CHD Warrir. It hnrs the Angels wh have lst their battle t cngenital heart defects thrugh an emtinal butterfly release ceremny. It is like n ther benefit walk by prviding games, activities, fd, and music t deliver a truly unique experience. This event brings the cmmunity tgether t escalate public awareness, sustain lifesaving research, and imprve the lives f individuals affected by CHD. Tgether, we are united t make a change. Where des the fundraising g? The CHD Calitin is perated and led by vlunteers with n salaried emplyees. Fundraising allws the CHD Calitin t cntinue t prvide exceptinal prgrams t hspitals and families, schlarships, and advance medical research, which ultimately has a direct impact n the lives f everyne affected with cngenital heart disease. 2 CHD Awareness Walk When, Where Sunday, Octber 1, 2017 Darlingtn Park, 600 Darlingtn Avenue, Mahwah, NJ The event will be held rain r shine. Onsite registratin and check-in begins at 10:00 AM, s it is imprtant t arrive early and have sufficient time t park and check-in. Team phts start at 10:30 AM. The event cncludes at apprximately 2:00 PM. This is nt a race. The walking distance fr this event is apprximately 1.0 mile n a paved path. Cmpleting the walk is nt mandatry t enjying the day s festivities and t supprt the CHD Warrirs. Nt allwed in Darlingtn Park The fllwing is nt permitted in Darlingtn Park during the CHD Awareness Walk event. Individuals nt adhering t these restrictins will be asked t leave the event. N pets N smking N alchl Fd deliveries are allwed befre 10:30 AM. After that time, the rad will be clsed and deliveries will nt be pssible. Event registratin Everyne wh attends the CHD Calitin annual CHD Awareness Walk must register nline, via the mailed brchure, r at the day f the event between the times 10:00 AM t 10:45 AM. The nline registratin system will shut dwn at 6:00 PM n Saturday September 31 st. It is highly recmmended t register in advance t avid delays the day f the event and t allw sufficient time fr fundraising effrts. Everyne wh registered in advance must check-in at the registratin table at the day f the event between 10:00 AM t 10:45 AM. There is a $25 registratin fee fr individuals 17 years and lder. The registratin fee helps cver a prtin f the event csts such as the t-shirts, games, etc. Please d nt cnsider the registratin as a substitute t fundraising; yur fundraising effrts will directly influence the lives f individuals impacted by cngenital heart disease. There is n fee t register children 16 and under, thugh a cmpleted registratin is required t attend the event. There is n fee t register a CHD Warrir f any age, thugh a cmpleted registratin is required t attend the event. A CHD Awareness Walk t-shirt is prvided t everyne wh registered and selected that they wish t receive the t-shirt. 3 Dnatins Online dnatins shuld be assigned t an individual participant. The team dnatin ttal will be calculated by cmbining the ttal dnatins f all f the team members. If yu have a check r cash dnatin, yu can mail it t ur ffice alng with a nte f the team name and the participant wh shuld receive the dnatin. We will enter the amunt int the system fr yu. Alternatively, yu can enter in the check/cash infrmatin as an ffline dnatin. i. G t the event website at ii. Lg int yur prfile page iii. Click n View Dnatins Received n the left menu bar iv. Click n Add Offline Dnatin in the middle menu v. Fill ut the frm infrmatin and click Add Dnatin buttn vi. Yu will need t mail the check/cash t ur ffice. The ffline dnatin will nt be fficially applied t yur dnatin ttal until we receive and prcess the check/cash. 4 The tp individual participant wh raised the mst mney by Octber 31, 2017 will receive an Ipad Pr. This prize will be mailed t the tp participant. The CHD Calitin must pay a 5% fee fr all nline dnatins fr the DnrDrive fundraising system. If yu wuld like t help allcate mre mney twards research and cntinuing the CHD Calitin hspital prgrams, please help cver the prcessing fee by selecting the ptin displayed belw. In this example, an extra $5 cvers the prcessing csts fr a $100 dnatin. CHD Warrir Psters We will be hanging psters t hnr all f the CHD Warrirs. These psters will be n display thrughut the event fr everyne t view and read. The psters are t be created by the CHD Warrir and/r family. Yu can use any size pster paper that yu wish. Include the persn's name, CHD diagnsis, a shrt stry abut yur family's CHD jurney, date f birth, pictures, etc. If yu have any questins, please inf@chdcalitin.rg and we will send yu samples. Please bring the pster t the event. Fr safe keeping, yu shuld take yur pster hme with yu at the end f the day. 5 Change yur prfile infrmatin & picture G t the event page and lg int yur prfile. If yu registered anther user n their behalf, at tp right click n Welcme, yur name and then Change User. Select the persn yu want t change then the Change User buttn at bttm. If yu want t change yur user prfile infrmatin r picture, click n Edit Prfile buttn. 6 Change yur event persnal and team page Lgin and select the user that yu want t change (step #1 and #2 abve). Click n the EVENTS buttn t see the list f events that yu are registered. Click n the event name. Yu have a multiple ptins n this screen. T edit yur persnal fundraising page, click n the Edit Yur Page link n the tp left. Yu can nw change the picture and text fr yur persnal fundraising page. If yu are the team captain, click n the Yur Team Fundraising link at the tp. T change the team page infrmatin, click n the Edit Yur Team Page link n the right. Make changes and click Save at the bttm.
