3 COMMUNITY FUNDRAISER - Q & A 1. Do I need approval in order to hold a fundraising event to benefit The Ability Center (ACT)?. All events and activities using The Ability Center name must be consistent with its mission, values, and brand strategy. The suggested way to promote the event is to list the event name followed by benefiting The Ability Center. 2. How do I obtain approval for my event? Simply fill out the Community Fundraiser Application and send it to ACT within 60 days of your planned event. 3. Can someone help me with the application? Absolutely! We are happy to help you with your application and answer any questions you may have. Contact Kimberly Danes, Director of Development at or kdanes@abilitycenter.org to discuss your event. 4. Will The Ability Center staff help me plan my event? We are glad to support your event by providing you with a promotional kit including logos, flyers, and brochures. If you do not have internet access, we are happy to take your request by phone and mail materials. ACT is not liable for any injuries sustained at the event. We are not responsible for securing sponsorships for third-party events, or permits, licenses or insurance that is needed for your event. Alcohol permits may not be obtained in the name of ACT. We may not be a party to any contract or service and do reserve the right to review any paperwork needed for the event. accounts may be opened in the name of, or on behalf of ACT. 5. Will The Ability Center staff work my event? If requested, an information table and/or an ACT representative can be present to discuss our programs and be on hand for a check presentation. We cannot provide financial assistance for the event. On-site credit card transactions for tickets or raffle items can be processed by ACT staff on the day of the event if requested. 6. Will The Ability Center staff help market my event? The Ability Center will post detailed event information on our website and post on social media channels up to six times before the event. Specific timelines can be discussed in further detail with our Marketing Department. This is dependent on how far in advance we are notified of the event. The Ability Center is also available for creation of an event flyer if needed. The ACT or ADAI logo must not be altered in any way.

4 COMMUNITY FUNDRAISER - Q & A CONT. 7. I m under 18 years of age. May I still hold a fundraising event? ; however, your parent or guardian must be the event sponsor. 8. Parts of the application do not apply to my event. What do I do? This is understandable, as the application is designed to support numerous types of events. Fill out only the sections that relate to your event, and if you have questions, please contact Kimberly Danes, Director of Development at any time for clarification. 9. Instead of holding an event, may friends or relatives make a donation to The Ability Center in lieu of gifts or favors for an anniversary, wedding, or birthday?, thank you! Charitable giving is a wonderful way to commemorate special occasions. These donations may be made directly by calling The Ability Center at , donating online at abilitycenter.org or by mailing a check (payable to The Ability Center) addressed to The Ability Center, 5605 Monroe St. Sylvania, OH Please clearly indicate on your check that The Ability Center is to receive your gift, and ask friends and relatives to indicate that their donation is in honor of your special occasion. application is required. After keeping accurate records of the event including revenue, receipts, and expenses, your event contribution will generally be your net proceeds. We are thankful to receive your check, made out to The Ability Center within 30 days of the event. 11. Can my event be denied? Please understand we cannot approve the following events that: - Involve a professional fundraiser/telemarketer or agreement to raise funds on a commission, bonus, or percentage basis - Consist of a door-to-door solicitation of any kind - Fail to comply with any county, state and/or federal law - Require our endorsement or marketing of a product or service, or our participation in the direct sale of a product or service - Promote a political party candidate, or appear to endorse a political party - Reflect discrimination on the basis of race, ethnicity, religion, nationality, or sexual preference, or are promoted by organizations associated with such discrimination
