Woofstock Survivor Charity Walk Participants raise money for the Survivor Program which funds the emergency and life saving care for thousands of abused, neglected, injured, and seriously ill animals each year. This program also covers the costs associated with rescuing over 200 animals each year who are at the greatest risk for euthanasia in other Indiana shelters. In 2017, our supporters raised over $30,000 - and with your support, we'll achieve our goal of $50,000 this year! Amberly, 8, is alive today because of the Survivor Program. 1

6 Food vendor Agreement Form Thank you for your interest in participating as a vendor at Woofstock! We expect it to be a fun-fill, action packed day for the whole family (including our furry friends)! The following highlights the food vendor "rules and regulations" to ensure a safe, fun, and successful day. You will receive an exhibitor booth assignment by September 28, To be fair to other vendors, we ask that you conduct your business from the designated area. Booths must be set up and ready to open by 11:00 a.m. and stay open until 6:00 p.m. You are welcome to sell any type of food and/or drinks, with the exception of alcoholic beverages Woofstock is a rain or shine event and vendor donations will not be refunded. Please set up your booth with the safety of other in mind including the children and dogs who may approach your booth. Prior to the opening of business and during specified event times, you agree at your cost and expense, to comply promptly with all laws, ordinances, rules and regulations, as required by Federal, State, and City governing agencies affecting the operation of your food sale area, including but not limited to the generality of the foregoing. If you require electricity, you must provide your own generator for power. You agree to pay a $200 vendor space fee to the Humane Society for Hamilton County, due no later than September 15, I have read all of the above rules and regulations and fully understand and agree with the guidelines. I accept full financial responsibility for any damages done on my part, my associates and my booth area. I will be professional, respectful, and conduct good business to the customers and associate vendors according to the regulations. I indemnify and hold harmless the Humane Society for Hamilton County, its agents, officers and employees, organizers, sponsors, and the City of Fishers, as it pertains to any injury or damages sustained by myself, associates, property or business.
