1.0 Executive Summary A pre-renovation asbestos survey and a lead-based paint inspection/risk assessment have been completed of the vacant duplex located at 362 Maria Avenue, Ramsey County, Minnesota (Site.) Based on our visual observations and our evaluation of analytical data, we conclude the following: The following materials meet the definition of asbestos-containing materials (ACMs) per the results of our survey: functional space material description asbestos content approximate quantity exterior tan cement board siding 15% 4,000 s.f. The Pre-Renovation and Demolition Rule (Minn. R. 7035.0805.) requires solid wastes, hazardous wastes, oil, tanks, appliances, and any components or items with loose or peeling lead-based paints, mercury, asbestos (depending on the type and condition), Chlorofluorocarbons and/or poly-chlorinated biphenyls (PCBs) to be removed prior to renovation or demolition.

2.0 Project Scope The purpose of this survey included the following: to identify and sample suspect asbestos-containing materials (ACMs) and to provide information regarding the identity, location, and approximate quantities of analyticallyconfirmed ACMs in preparation for renovation of the duplex; and to conduct a lead-based paint inspection/ risk assessment. A lead-based paint inspection is a surface-by-surface investigation to determine the presence of lead-based paint. A risk assessment involves evaluating a dwelling for lead-based paint hazards, and if present, providing lead hazard reduction options. 3.0 Site Description The subject site is located at 362 Maria Avenue, St. Paul, Ramsey County, Minnesota (Site). The parcel identification (PID) for the Site is 32-29-22-13-0081. At the time of this survey, the structure at the Site included a vacant 1884- wood-frame-constructed two-story duplex with a basement foundation. A Site location map and Site sketches of the floor plan are shown on Figures 1 thru 3, respectively (Figures, Appendix A). Building materials within the duplex appeared of similar age, of similar construction, and of similar materials. Building materials within the units consisted of popcorn, ceiling tile and plaster ceilings; plaster walls; wood and vinyl window components; wood door components; wood baseboards; vinyl and wood flooring. 4.0 Asbestos Survey Asbestos Bulk Sampling Results The Site was surveyed for asbestos-containing materials (ACM) on August 26, 2013 by Kristine Stahl, Certified Asbestos Inspector #AI8690 ( MDH Inspector Certifications and Laboratory Certificate, Appendix B), in accordance with the Minnesota Department of Health Asbestos Abatement Rules (MR Chapter 4620), and the Pre-Renovation and Demolition Rule (Minn. R. 7035.0805.) Bulk sampling was performed in general accordance with the sampling protocols established in the Code of Federal Regulations (CFRJ) 40, Section 763, Subpart E. Sampling within the structure included the collection of ceiling, wall, flooring materials, and the exterior siding. 4

The bulk samples collected for laboratory asbestos analysis were submitted to Environmental Hazards Services, an accredited NVLAP laboratory for analysis using polarized light microscopy (EPA 600/R-93/116 Method.) Laboratory analysis results indicate the presence of asbestos (an asbestos content of more than one percent (1%) in the following materials surveyed for this project: functional space material description asbestos content approximate quantity exterior tan cement board siding 15% 4,000 s.f. Additional sampling information is summarized on Table 1 (Inventory of Suspect Asbestos- Containing Materials Surveyed, Appendix C), the approximate sampling locations are shown on Figures 2, 3, and 4 (Figures, Appendix A), and the laboratory analytical report attached to this report. Discussion Asbestos abatement should be performed by properly trained abatement contractors using appropriate controls to prevent fiber emissions during the removal process. Workers performing removal should be properly protected to prevent exposure, including the use of respiratory protection with HEPA filtration. Asbestos containing waste materials should be properly contained and transported for offsite disposal at a properly permitted facility. The contractor conducting abatement work is responsible for complying with local, state, and federal standards for worker protection and NESHAP regulations regarding asbestos fiber emissions. The MPCA and the Minnesota Department of Health (MDH) must be notified of asbestosrelated work. A Notification of Asbestos-Related Work must be submitted to the MPCA and Minnesota Department of Health prior to asbestos abatement work. The MPCA must be notified of demolition work. The Minnesota Pollution Control Agency (MPCA) must be notified with a Notification of Intent to Perform a Demolition ten working days prior to the start date of a demolition project. All materials regulated under the Pre- Renovation and Demolition Rule, including RACM, building components with peeling leadbased paints and all other items prohibited from being deposited in a landfill, must be removed from the structure two days prior to the start date that was submitted for the notification. Limitations Statement Asbestos was widely used until the late 1970 s in thousands of building materials, making it difficult to locate all areas of ACM usage. This assessment reflects the data collected from the specific locations tested to identify ACM in those locations and may not be all encompassing. There is always potential for asbestos containing materials to be missed due to problems with accessibility, and the broad variety of uses. The conclusions and recommendations within this 5

report are based on current regulations and the professional experience of the certified professionals involved in this project. 5.0 Lead-Based Paint Inspection/ Risk Assessment (PIRA) This project included a lead-based paint inspection and risk assessment. Additional information on statues, rules, and protocols used for this inspection are described in the Standard Operating Procedures for Lead-Based Paint Inspections, Appendix D attached to this report. The on-site lead-based paint inspection/ risk assessment was conducted on August 27, 2013 and involved testing surfaces using an X-Ray Fluorescence Spectroscopy (XRF) to determine lead concentrations. Interior sampling included the ceilings, the walls, the window components, the door components, the cabinets, the stair components, two tubs, the baseboards, and the flooring. Exterior sampling included the siding, the soffit, the fascia, the foundation, and the porches. The XRF testing locations and results are summarized on Table 2 (, Appendix E.) The XRF results indicated lead-based paints are present on the exterior vinyl window casings, soffit, and fascia. However, the positive readings are likely due to lead-based paints underlying the vinyl building components. The XRF records depth index readings on a scale of one to ten with ten signifying readings at the greatest depths, and readings collected in light substrates, such as vinyl, are collected at greater depths. All XRF depth index readings of the exterior vinyl building components resulted in ten. Based on the XRF depth reading of ten and the light substrate of the vinyl, the positive readings are likely due to lead-based paints underlying the vinyl building components. Depth index readings are shown in the Depth Index column on the table attached to this report. To further screen for lead risk hazards, six dust wipe samples were collected from each unit at the Site and submitted to Accurate Analytical Testing, an accredited laboratory. The results of the following dust wipe samples indicated lead levels above HUD and the Minnesota Department of Health (MDH) standards for lead in dust: UNIT AREA TESTED main-level unit kitchen floor, living room floor, bedroom #2 floor, bedroom #2 window sill, 2 nd -level unit kitchen floor, bedroom #3 floor, bedroom #1 floor, 8

UNIT AREA TESTED living room window sill, and bedroom #3 window sill. A summary of the dust wipe and collection with the analytical results for each unit are summarized on Tables 3 and 4 (Summary of Laboratory Analytical Results, Appendix F) and copies of the laboratory reports are attached to this report. Soil sampling was not required for this project, since the ground surface at the Site was covered with vegetation and mulch with no bare soil noted around the foundation of the home or in the yard at the time of this inspection. Recommended lead hazard control measures and a reevaluation schedule are provided on Tables 5 and 6, respectively (Lead Hazard Reduction Options, Appendix G and Maintenance/Monitoring Schedule, Appendix H.) In general, all painted surfaces should be monitored. A negative result does not necessarily indicate that no lead is present in that surface, but rather indicates that any lead present in that surface is below the 1.0 mg/cm 2 threshold in the area tested. Therefore, all painted surfaces should be maintained in intact condition. Based on the findings of the lead-based paint survey, lead safe work practices should be followed if building components identified with lead-based paints above are disturbed. Painted surface to be disturbed means a paint surface that is to be scraped, sanded, cut, penetrated or otherwise affected by rehabilitation work in a manner that could potentially create a lead-based paint hazard by generating dust, fumes, or paint chips (24 CFR 35.110 Definitions.) Lead Safe Certified contractors should use extreme care and lead safe work practices should be followed during the rehabilitation work. All available records and reports concerning lead-based paint and/or lead-based paint hazards are required to be disclosed to purchasers and tenants at the time of sale or lease or upon lease renewal. Disclosure Information is attached to this report (Disclosure Information, Appendix I). This lead inspection/risk assessment report should be kept by the inspector and all future owners for the life of the structure. 6.0 Additional Pre-Renovation Items of Concern The Pre-Renovation and Demolition Rule (Minn. R. 7035.0805.) requires solid wastes, hazardous wastes, oil, tanks, appliances, and any components or items with loose or peeling lead-based paints, mercury, asbestos (depending on the type and condition), Chlorofluorocarbons and/or poly-chlorinated biphenyls (PCBs) to be recycled or properly disposed of prior to renovation or demolition. 9

Appendix C: Inventory of Suspect Asbestos-Containing Materials Table 1 Definitions Asbestos-containing material or ACM. Asbestos-containing material or ACM is defined by the asbestos NESHAP as any material containing more than one percent (1%) asbestos as determined using Polarized Light Microscopy (PLM.) Asbestos-containing building material (ACBM). Surfacing ACM, thermal system insulation ACM, or miscellaneous ACM that is found in or on interior structural members or other parts of a building. Friable asbestos material. Friable asbestos material means any material containing more than one percent asbestos by microscopic visual estimation by area, that hand pressure can crumble, pulverize, or reduce to powder when dry. Friable asbestos material includes previously nonfriable asbestos material which becomes damaged to the extent that when dry all or a portion of the material may be crumbled, pulverized, or reduced to powder by hand pressure. Homogeneous area. Homogeneous area means an area of surfacing materials, thermal system insulation materials, flooring, or other miscellaneous materials which upon examination for properties such as age, color, and texture appear to be composed of the same material. Functional Space. A room, group of rooms, or homogeneous area (including crawl spaces or the space between a dropped ceiling and the floor or roof deck above), such as classroom(s), a cafeteria, gymnasium, hallway(s), designated by a person accredited to prepare management plans, design abatement projects, or conduct response actions. Regulated Asbestos Containing Material (RACM) Miscellaneous material. Interior building materials on structural components, structural members or fixtures, such as floor and ceilings tiles and does not include surfacing material or thermal system insulation. Nonfriable material. Material when dry may not be crumbled, pulverized, or reduced to powder by hand pressure. Surfacing Material. Material in a building that is sprayed-on, troweled-on, or otherwise applied to surfaces, such as acoustical plaster on ceilings or fireproofing materials on structural members, or other materials on surfaces for acoustical, fireproofing, or other purposes. Thermal system insulation. Material in a building applied to pipes, fittings, boilers, breeching, tanks, ducts, or other interior structural components to prevent heat loss or gain, or water condensation, or for other purposes. 19
