Day 3 Schedule Evidence-based Medication Therapy Management to Optimize Patient Outcomes Date: Friday, 05/03/2019 Check-in: 7:30AM-8:00AM Educational Session: 8:00AM 1:30PM This case-based seminar will focus on the safe, effective and affordable use of evidencebased pharmacotherapy comparing new agents against existing agents and the highlevel knowledge required to help patients and providers optimize therapeutic outcomes for individual patients. Participants will be given a set of typical patients from a primary care practice setting. At the completion of this activity, the participant will be able to: Evaluate multiple primary care patient cases exhibiting commonly seen medical problems. Demonstrate motivational interviewing techniques for a diverse range of patients. Interpret patient-specific case data, issues, and medication therapies to identify medication-related problems. Suggest goals of therapy for each medical condition and medication-related problem identified. Formulate plans to resolve medication-related problems. Analyze pharmacokinetic and pharmacodynamic changes when reviewing an elderly person s medication regimen. Recommend appropriate, evidence-based, cost effective therapies to replace inappropriate therapies to improve patient outcomes. Effectively communicate recommendations to other health care providers.
