Employee Burned by Electrical Arc-flash Background Information Before beginning this, please review this and the next page which contain information that will introduce the concepts of and Systems of Safety. Creating a safe and healthy workplace requires a never ending search for hazards that sometimes are not obvious to us. These hazards exist in every workplace and can be found by using various methods. are just as the name suggests: learning from incidents to prevent the same or similar incidents from happening again. Systems Are Not Created Equal: Not equal in protection and not equal in prevention. Using our Systems Focus to uncover system flaws or root causes is only one part of controlling hazards. We also need to look at the systems involved to decide on the best way to deal with the problem. The most effective way to control a hazard is close to its source. The least effective is usually at the level of the person being exposed. The system of safety in which the flaw is identified is not necessarily the system in which you would attempt to correct the flaw. 2

Employee Burned by Electrical Arc-flash Title: Employee Burned by Electrical Arc-flash Volume 07, Issue 87 Date Issued: December 6, 2007 Statement The worker involved in the incident was placed in a hazardous position due to failures in the Design and Engineering and Training and Procedures Systems of Safety. Though during the investigation it was determined that the worker was not wearing the appropriate PPE required to perform the work, the lack of a mechanism to ensure that he was qualified and the failure to ensure that all contractors were familiar with Standing Instructions contributed to the worker s unawareness for the required PPE (Training and Procedures). The presence of a less than rigorous work permit program (Training and Procedures) also had a serious impact on the worker s actions. Finally, the absence of a uniform refinery wide lockout/tagout program located under Design and Engineering Systems of Safety was a major factor in the incident. 4

Employee Burned by Electrical Arc flash Discussion A contract worker was installing new relays inside of a control panel for an air filter heater. The contractor did not have a work permit to perform the work and the control panel was energized at the time of the incident. The contractor was not qualified to perform work on energized equipment and there was no program in place to ensure his qualifications. While mounting one of the relays in the control panel, wires that were hanging from the new relay shorted on a 480v fuse holder. The worker suffered second degree burns to his left hand from the resulting arc-flash. When the incident occurred, the worker was not trained in the Standing Instructions that required him to wear 1000v rated gloves and a flash-rated face shield in addition to the flame retardant clothing (FRC) the worker was already wearing. 5

Ensure all contractors working on and around energized electrical equipment are trained and qualified to OSHA standards to perform energized work. 2. Ensure all contractors working on or around energized electrical equipment review Standing Instructions on electrical switch houses. 3. Replace the 100 amp breaker feeding the air heater control box. 4. Ensure work permits are used for all work in the refinery. 5. Design a lockout/tagout program for all facility locations in the refinery. 7
