GENERAL EXCHANGE TARIFF Section 6 25th Revised Sheet 2 Replacing 24th Revised Sheet 2 CALL MANAGEMENT SERVICES 6.1 GENERAL REGULATIONS 6.1.1 Call Management Services are available to residence and business customers and are subject to availability of facilities, technology, and compatibility with customer access line, associated service(s) and premises equipment. 6.1.2 Call Management Services are not available to Plexar, Payphone Exchange Access Service or DigiLine subscribers unless otherwise specified in Paragraphs 6.1.3 and 6.2 following. Additional service restrictions and hunting limitations may also apply to specific services as specified in Paragraph 6.2 following. 6.1.3 Auto Redial and Speed Calling are price deregulated for business and residence customers, and individual customer pricing is available. In addition, all Call Management Services for business and residential customers are price deregulated in the following exchanges: Lindsborg (Residence Service only) All Call Management Services will be available for purchase individually, subject to the service descriptions contained in this tariff. 6.1.4 Auto Redial, Call Blocker, Call Return, Call Trace, Priority Call, Selective Call Forwarding, Caller ID-Calling Number Delivery and Calling Name Delivery are functional when both the call originating customer and the call terminating customer are served from central offices capable of sending and receiving the calling name and number and are linked by appropriate facilities. 6.1.5 When multiple services are activated on the same line, certain services may take precedence over others. (MT) (MT) 6.1.7 Additional regulations specific to each Call Management Services service are included in the individual service descriptions as set forth in paragraph 6.2 following. 6.2 SERVICE DESCRIPTIONS 6.2.1 Auto Redial Auto Redial is price deregulated, and individual customer pricing is available. Enables the customer to redial automatically the last outgoing telephone number. If that telephone number is busy, the Company s equipment will keep trying to call the number for a maximum of thirty (30) minutes beginning with the customer's activation of Auto Redial, in an attempttoestablishthecall. Thecustomerwillbesignaledwithadistinctiveringwhenthecall can be completed. 6.2.2 Call Blocker Enables the customer to block calls from preselected telephone numbers (quantity varies by technology) and/or the last incoming call (without knowing the number). To block specified telephone numbers, the customer builds a screening list. To block an unknown number after receiving a call, the customer enters a code to add the number to their screening list. If facilities are unavailable to provide incoming call screening via the customer's list, standard call completion will occur. Customers whose telephone numbers are blocked are directed to a Company recorded announcement.

GENERAL EXCHANGE TARIFF Section 20 3rd Revised Sheet 32 Replacing 2nd Revised Sheet 32 RULES AND REGULATIONS APPLYING TO ALL CUSTOMERS' CONTRACTS 20.20 PRICE DEREGULATION All services in the exchanges listed below are price deregulated, except for Lifeline Service. (RT) Linsborg (Residence Service only) (RT) Residence and business packages or bundles are price deregulated statewide. Price deregulated packages or bundles of services are defined as the offering of a local telecommunications service with one or more of the following subscribed together, as one service option offered at one price: one or more call management services, intralata long distance service, interlata long distance service, internet access, video services or wireless services. Price deregulated packages or bundles shall not include only a single residence access line or up to four business local exchange access lines(1) at one location with only intralata long distance service or interlata long distance service, or both. (1) Business lines include, Flat Rate 1-Party (1FL), Message Rate 1-Party (1ML), Multi-line (1BH - Kansas City and s), Reserve Line (12L), Announcement Facilities (788), Group Alerting and Dispatching Systems (E3T), Public Response Calling Service (1MN, 1M2)

PRIVATE LINE SERVICE TARIFF Section 1 4th Revised Sheet 24 Replacing 3rd Revised Sheet 24 REGULATIONS 1.11 PROMOTIONAL PROGRAMS The Company may, during certain promotional periods, offer a customer special rate incentives and/or waive in full or in part the nonrecurring charge for the purpose of stimulating tariff offerings. The Company will notify the Kansas Corporation Commission by letter specifying services offered, terms of the promotion, location, and dates of each promotional period, 20 days in advance. 1.12 PRICE DEREGULATION Private Line rate elements are price deregulated in the following exchanges: Lindsborg (Residence Service only)

INTEGRATED SERVICES TARIFF Section 1 11th Revised Sheet 1 Replacing 10th Revised Sheet 1 INTEGRATED SERVICES DIGITAL CIRCUIT SWITCHED VOICE (CSV) / CIRCUIT SWITCHED DATA (CSD) 1.0 The following rate elements provide for digital CSV/CSD transport through central office switching equipment. These rate elements are for Local Exchange usage. Each rate element shall constitute a local exchange access arrangement and shall be counted in combination with those of the Local Exchange Tariff and the General Exchange Tariff. These CSV/CSD local service elements are offered only in connection with services provided under this tariff and not offered on a stand-alone basis. These rate elements do not apply to the SuperTrunk Option. ACCESS SERVICE TARIFF Section 2 4th Revised Sheet 8 Replacing 3rd Revised Sheet 8 GENERAL REGULATIONS 2.7 FILING OF CUSTOMER CONTRACTS WITH THE KCC Pursuant to the Commission's Order 16 in Docket No. 02-GIMT-555-GIT, contracts for intrastate access services provided on an Individual Customer Pricing (ICB) basis will be filed with the KCC within 30 days of the effective date of the contract and will include customer identification number, service description and the applicable rates and a verified statement that the price of the service offered under the agreement is above the long-run incremental cost of the service. The contract will be filed with all customer specific information redacted. A confidential non-redacted cover sheet for the ICP contract will be filed with the KCC pursuant to K.S.A. 66-1220a. The following information will be made available, at the request of the KCC, for the life of the contract term, and handled pursuant to applicable proprietary agreements between the Company and the KCC: A. Customer name and location(s) B. The incremental cost study C. The contribution level used D. The payment option selected E. Monthly revenue 2.8 UNE TO ACCESS CONVERSION LANGUAGE The UNE to Access Conversion Language in FCC Tariff #73, Section 2.1.1 only applies to UNE to Interstate Access conversions. 2.9 PRICE DEREGULATION Non-Switched Access Service rate elements are price deregulated in the following exchanges: Lindsborg (Residence Service only)

All other services are billed as provided by Missouri tariffs. (4) These customers are located in Kansas and served by Picher, Oklahoma. Kansas rates and charges are billed, as set forth in the Kansas Local and General Exchange Tariffs. (5) See paragraph 1.8 for special service arrangement serving -Winfield Joint Airport. (6) See paragraph 1.7.2 for Exception Rates applicable to Paola and Abilene. These Exception Rates do not apply to the isolated area of the Abilene exchange called Enterprise. (7) This exchange area includes an isolated area called Enterprise as shown on the exchange and isolated base rate area maps for Abilene. Customers in this isolated area are billed local exchange rates as set forth in paragraph 1.7.1. (8) MetroPlus available - See paragraph 1.15. (9) Customers in this exchange may call customers on a local basis in United Telephone Company's exchange of Lancaster, Kansas, who subscribe to the MetroPlus optional calling plan.
