General: DATE: 31 May 2016 HILL AIR FORCE BASE CEN/CEIE PLAN REVIEW CHECKLIST FOR NEW DEVELOPMENT AND REDEVELOPMENT CONSTRUCTION Submittal of the documents identified herein, completion of the site plan review by CEN and CEIE, and use of this checklist are required by the Utah Pollutant Discharge Elimination System (UPDES) permits issued by the Utah Division of Water Quality (DWQ), for construction projects disturbing an area one acre or greater in size. These permits include the Storm water General Permit for Construction Activities (obtained directly by the contractor from DWQ) and the General Permit for Discharges from Small Municipal Separate Storm Sewer Systems (MS4); a permit issued to the base. The attached checklist is required to be submitted by the contractor with the Storm water Pollution Prevention Plan (SWPPP) during the project review process. The contractor is responsible for compliance with all Department of Defense (DoD), Air Force Instruction (AFI), Unified Facilities Criteria (UFC), and State of Utah DWQ requirements. In addition, the contractor is responsible for compliance with the DoD Implementation of Storm water Requirements under Section 438 of the Energy Independence and Security Act (EISA) and obtaining all necessary permits and approvals. This form is intended for use by the design engineer in determining the necessary submittals for a plan review. It is also intended to be used by CEN and CEIE for review and approval of these documents. This checklist is not intended to be allinclusive; consult with CEN and CEIE for site-specific issues. Proper completion of this checklist will expedite the approval processes. Instructions: The attached form is a list of typical storm water review criteria for all construction projects. The A-E s responsible engineer is required to check each item on the list; if an item is not applicable to this project, the item will be noted as N/A. Documents submitted to CEN and CEIE shall be organized according to the following general format: A. Title and general Sheets (project name, vicinity map, sheet index, notes, etc.) B. Boundary and topographical survey C. Grading/Drainage Plan D. Master Utility Plan E. Plan and Profile Sheet(s) (for final submittal) F. Detail Sheets G. Operation/Maintenance Plan for Post-Construction Stormwater Controls Upon submittal for plan review, the A-E Project engineer must sign this form indicating the checklist and all required documents have been submitted by the responsible engineer. A copy of this checklist must accompany the plans with all subsequent reviews. Plans will not be accepted for review without this signed checklist. In providing approvals, CEN and CEIE assumes that applicants have not made any errors and have complied with all applicable regulations. If after an approval, an error is discovered or it is discovered that some aspect of approved drawings does not comply with applicable regulations, the applicant shall, at his own expense, revise the drawings and modify any infrastructure as necessary to correct the problem.
