Page 1 VACATION I. POLICY Monongahela Valley Hospital, Inc. provides vacations with pay for all eligible employees. II. RESPONSIBILITIES A. Supervision - Supervision is responsible for approving in advance any time taken off as paid vacation, using as criteria for permission department work schedules and the possible expenditure of budgeted part-time hours. B. Employee - Employees are responsible for scheduling vacations such a way that work obligations, departmental operations, or patient care do not suffer. C. Human Resources - The Human Resources Department is responsible for interpretation and administration of this policy. III. REGULATIONS A. General Regulations that apply to both full-time and part-time employees: 1. Each employee who has attained at least twelve (12) months of continuous service and has performed work in the current calendar year shall receive a vacation. 2. An employee forfeits a right to vacation benefits if the employee quits, retires or is discharged prior to January 1 of the vacation year. Page 2 3. Vacations provided by this section are not to be accumulated and must be taken within the calendar year. 4. Employees with longer service will be given preference for their selection of vacation. 5. Vacation will, so far as possible, be granted at times most desired by employees, but the final right to allow vacation periods, and the right to change vacation periods is exclusively reserved to the Department Head. Any change in vacation schedules may be realized by mutual consent. In the event the Department Head changes a schedule causing the employee to suffer financial loss, the hospital will reimburse the employee for provable loss. 6. Advance vacation pay is available to any employee who requests the advance two (2) weeks prior to the payday in which the advance is to be paid. Advances are only available on the payday preceding the vacation period. 7. The rate of pay for the vacation period shall be the employees regular rate of pay. 8. If an illness or accident preventing work occurs prior to and extends into an employee's scheduled vacation, the affected employee may elect to have the vacation period continue with appropriate pay, or elect to have the vacation postponed and another period assigned. However, if payment is elected by the employee for the designated vacation period, no other payment will be made under any benefit for the paid vacation period. 9. If an illness or accident preventing work begins after an employee commences the vacation, the original vacation as schedule shall remain in effect, and no other payment will be made for the paid vacation period. If the illness or injury causes the employee to be unable to return to work, then the provisions of the sick and accident policy will become effective. 10. No employee may receive vacation pay in lieu of taking time off. B. Regular Full-Time 1. An eligible regular full-time employee who has obtained twelve (12)

Page 3 months of continuous service shall receive vacation in each calendar year according to the following schedule: YEARS OF VACATION VACATION CONTINUOUS SERVICE TIME OFF PAY 1 year but less than 5 years 2 weeks 80 hours 5 years but less than 10 years 3 weeks 120 hours 10 years but less than 20 years 4 weeks 160 hours 20 years or more 5 weeks 200 hours 2. An employee is eligible for two (2) weeks vacation after the first year, one of which can be taken after six (6) months of continuous service. 3. Vacation periods will consist of seven (7) calendar days. However, single vacation days may be scheduled according to the following: VACATION TIME ALLOWABLE ALLOTMENT SINGLE VACATION DAYS Portion of vacation time allotment which may be taken as single vacation days 2 weeks 5 days 3 weeks 5 days 4 weeks 10 days 5 weeks 10 days 4. Single vacation days must be requested and approved prior to November 1 st. 5. Single vacation days must be requested at least six (6) weeks in advance of the schedule posting. 6. Scheduling and approval of single vacation days will depend on adequate staff coverage for the Department. 7. The approval and scheduling of single vacation days will occur only after full vacation weeks have been scheduled. 8. A person who has scheduled a single vacation day will not be "bumped" by a person wishing to schedule a personal convenience day. 9. A person who has scheduled a weeks vacation will not be "bumped" by a person desiring a single vacation day, regardless of length of service.

Employees with one (1) or more years of Hospital service who give proper notice of intent to resign will be entitled to be paid for unused entitled vacation (terminal vacation pay) in the last pay they receive. Employees may not take unscheduled vacation as any part of or in lieu of their proper notice (see policy 2.11 - Terminations). C. Regular Part-Time 1. An eligible regular part-time employee who has obtained twelve (12) months of continuous service shall receive vacation according to the following schedule: HOURS WORKED IN VACATION VACATION PREVIOUS CALENDAR YEAR TIME OFF PAY 416 to 831 2 Days 16 Hours 832 to 1,247 4 Days 32 Hours 1,248 or More 6 Days 48 Hours 2. Terminal vacation will be granted by the Hospital to the extent of vacation eligible provided a written notice of resignation is given at least two (2) weeks in advance. D. Temporary and casual employees are not eligible for paid vacation. IV. PROCEDURE A. Regular Full-Time 1. The Human Resources Department will notify all regular full-time employees of their vacation eligibility for the following year. This notice (vacation slip) will be distributed no later than November 1 of each year. 2. The employees will notify the Hospital on the vacation slip of the desired periods of vacations. This notice will be returned to the respective department head no later than December 1 of each year. 3. The approved vacation schedule will be posted not later than the first Monday of January of the year in which the vacation is to be taken. B. Regular Part-Time 1. The Human Resources Department will notify all regular part-time

Page 5 employees of their vacation eligibility for the current year. This notice (vacation slip) will be distributed not later than February 1 of each year. 2. The employee will notify the Hospital on the vacation slip of the desired periods of vacation. This notice will be returned to the respective department head no later than February 15 of each year. 3. The approved vacation schedule will be posted not later than the first Monday in March of the year in which the vacation is to be taken. 4. Employees may take vacation in January and February, in accordance with Section IV, provided approval is received by the department head and the Human Resources Department.
