The cleanup included the removal, disposal and replacement of water-damaged suspended ceiling panels, over-sprayed asbestos-containing fireproofing from insulated ductwork and fiberglass drain pipe insulation coated with asbestos-containing black mastic along seams adequate to provide access. As per the Operations and Maintenance (O&M) plan prepared for the property, cleaning was performed adequate to complete these repairs. Because of the amount of asbestos-containing materials (ACM) affected by the work, Zapolski Real Estate, LLC (Zapolski) contracted Demolition and Asbestos Removal, Inc. (DARI) located in Raleigh, North Carolina to perform the work. OLME was also contracted to inspect the work and obtain ambient air sampling to determine the air quality resulting from the work activities. This report summarizes our work observations and the ambient air monitoring result obtained. DARI s crew was onsite July 27, 2016 performing the desired scope of work. OLME representative, Oral McGirt, was also onsite during the work and met with DARI project supervisor, Alejandro H. Arreal, and Zapolski property manager, Dan Ivey to coordinate the work and to establish cleaning procedures to follow. DARI s abatement crew consisted of one licensed supervisor and three workers. Each maintained an asbestos accreditation demonstrating that they were licensed by North Carolina s Health Hazard Control Unit (HHCU) to perform asbestos-related abatement activities. Prior to performing any activity that would further disturb or expose the identified asbestos-containing fireproofing exposed by the damages, DARI s crew was required to establish a regulated work area prior to performing the work. Workers installed 6-mil polyethylene sheeting (poly) barriers over doorways and openings to restrict access during the work. 6-mil polyethylene sheeting was placed on the floor to catch any fallen debris. Wet methods and HEPA-equipped vacuum(s) were also required, as per the O&M plan, to control airborne particle during the work. Ambient air sampling was performed to determine the level of airborne particulate generated during the work and to gauge the thoroughness of the cleaning. Sampling was performed inside and outside the regulated work area to gauge the level of airborne particulate generated by the work. Sampling and analyzes of the air sampling obtained were performed by Mr. McGirt, who maintains the appropriate State of North Carolina accreditation for the type of services performed. Details of Mr. McGirt s accreditation information are attached table is entitled, OLME Project Personnel Summary Table. Project Setup and Preparations: Prior to disturbing the identified ACM, DARI s crew 6-mil polyethylene (poly) drop cloths was installed on the floor beneath the water-damaged section of ceiling in order to catch any falling debris during the work. Pre-cleaning of the corridor was also performed to remove visible ACM prior to the start of the work. See the attached Photographs for views of the work preparations and abatement methods. Mr. Arreal, reported that the crew planned to thoroughly saturate the affected materials with water using a Hudson-type sprayer(s). Handtools and a HEPA-equipped vacuum were used throughout the work. He reported that loose and over-sprayed fireproofing was removed from ductwork surfaces adequate to perform the desired repairs. Asbestos-contaminated debris generated during the work was placed inside 6-mil poly waste disposal bag(s) and again be coated with water to minimize fiber release. Once adequately filled, waste was then gooseneck sealed and then placed in a second waste bag and sealed.

The ambient air sampling were obtained and analyzed using Phase Contrast Microscopy sampling and analysis procedures. Analysis of the ambient air sampling performed outside the work area and after cleaning were determined to be acceptable for public access, less than 0.01 fiber per cubic centimeter of air (f/cc) for each sample obtained, as per the North Carolina State regulations. AIR MONITORING METHODOLOGY
AIR MONITORING METHODOLOGY The final ambient air clearance sampling for this project were obtained and analyzed to determine the fiber concentration in each air samples. The samples were analyzed by Phase Contrast Microscopy using the National Institute for Occupational Safety and Health (NIOSH) 7400 Method. This method is essentially similar to the Occupational Safety and Health Administration (OSHA) Reference Method and specifies the procedure for analyzing air samples for asbestos. The 7400 Method counts all fibers that meets the counting criteria and does not distinguish between asbestos and non-asbestos fibers. To distinguish between asbestos and non-asbestos fibers would require analysis by Transmission Electron Microscopy. The concentration of airborne fibers in an air sample is controlled by several parameters, including total number of fibers counted on the filter, total number of fields counted, filter area, monitoring time and flow rate. The total number of fibers and fields counted in a sample are governed by the counting rules of the 7400 Method. The filter area is a constant and the monitoring time is a known factor controlled by the sampler. Several high flow electric pumps were utilized to obtain area monitoring. Documentation of the calibration of the pumps was performed before and after monitoring using a secondary standard (a rotometer), which in turn is calibrated every six months using a primary standard. Area samples were generally collected by pulling air at a rate of 10.0 liters per minute (lpm) through a 25-millimeter diameter mixed cellulose ester filter (pore size 0.8 micrometer). Some filter cassettes were analyzed as box blanks. One cassette from every box of fifty was analyzed without having been opened to determine the suitability of that particular box of cassettes for air monitoring. Monitoring was performed for a period of sufficient duration for the limit of qualification to be less than 0.01 f/cc for area sampling. Samples with fiber concentrations below the quantification limit were reported as below (<) the quantification limit.
