VENDOR will provide labor and materials as detailed in the scope of work below. The proposal lists the following exceptions and assumptions that the following work will be performed by others: A ground bar and connection to ground will be provided and installed at the communications backboard. Plywood backboards as needed. All through floor sleeves, through wall sleeves, conduit, outlet boxes and device rings. VENDOR can provide this work at an additional charge and if so, it should be listed separately. o This includes any sleeves needed but not specifically shown on the drawings. Fire-stopping is not included in our proposal. This pricing does not include patch cables. VENDOR is not responsible for any failed tests on existing fiber due to abnormalities along the run.

Scope of Work Work area outlet locations: o 1 - Supply, install, terminate and test communications cabling to support the outlet locations as This includes: specified Re-termination, testing and housing for existing fiber between MDF and IDF (closet H148) in High School. In MDF (B106) - Re-terminate & relocate existing 4 Strand MM OM1 Fiber Cable (currently in wall mounted panel with ST terminations) into a new 4RU Rack Mounted Fiber Panel (Century FTS-700) with new 12 port LC adapter panel using the appropriate LC connectors. Re-terminate existing 12 strand MM OM1 & 12 Strand SM OS1 to Primary School, 12 strand MM OM1 & 12 Strand SM OS1 to Middle School, 12 strand MM OM1 to (IDF H128) and 12 strand MM OM1 (IDF E109) (currently in rack mounted 3M Volition panel) into the same 4RU panel previously mentioned with new 12 port LC adapter panels using the appropriate LC Connectors. New 4RU Panel to be installed where the current Volition panel is currently located. In IDF (H148) Re-terminate & relocate existing 4 strand MM OM1 Fiber Cable (currently in wall mounted panel with ST terminations) into a new 1RU Rack Mounted Fiber Panel (Century FTS- 175) with new 12 port LC adapter panel using the appropriate LC connectors. Re-terminate 12 strand MM OM1 to (MDF B106) (currently in rack mounted 3M Volition panel) into the same 1RU panel previously mentioned with new 12 port LC adapter panels using the appropriate LC Connectors. o 2-6-strand MM Re-termination, testing and housing for existing fiber between MDF and IDF (closet E109) in High School. In IDF (E109) Re-terminate & relocate existing 4 strand MM OM1 Fiber Cable (currently in wall mounted panel with ST terminations) into a new 1RU Rack Mounted Fiber Panel with new 12 port LC adapter panel using the appropriate LC connectors. Re-terminate 12 strand MM OM1 to (MDF B106) (currently in rack mounted 3M Volition panel) into the same 1RU panel previously mentioned with new 12 port LC adapter panels using the appropriate LC Connectors. o 3-6-strand MM Re-termination, testing and housing for existing fiber between High School and Middle School. In IDF A (Middle School) - Re-terminate existing 12 strand MM OM1 & 12 Strand SM OS1 fiber cable from the High School MDF (currently in rack mounted 3M Volition panel) into a new 1RU Rack Mounted Fiber Panel (Century FTS-175) with new 12 port LC adapter panels using the appropriate LC connectors. Re-terminate & relocate existing 6 Strand MM OM1 to (IDF B in same Bld.) and the existing 6 Strand MM OM1 to Field Allen into the same 1RU panel previously mentioned with new 12 port LC adapter panel using the appropriate LC Connectors.

This includes: o 4-6-strand SM This includes: 6-strand MM Re-termination, testing and housing for existing fiber between High School and Primary School, closet C. In Closet B of Primary School - Re-terminate existing 12 strand MM OM1 & 12 Strand SM OS1 fiber cable from the High School MDF (currently in rack mounted 3M Volition panel) into a new 2RU Rack Mounted Fiber Panel (Century FTS-350) with new 12 port LC adapter panels using the appropriate LC connectors. Re-terminate the existing 6 strand MM OM1 fiber cable from both closets A & C within the building into the same 2RU panel previously mentioned with a new 12 port LC adapter panel using the appropriate LC Connectors. The new 2RU Panel will be installed where the 3m Volition one currently is. In Closet A - Re-terminate existing 6 strand MM OM1 fiber cable from Closet B into a new 1RU Rack Mounted Fiber Panel (Century FTS-175) with new 12 port LC adapter panel using the appropriate LC connectors. In Closet C - Re-terminate existing 6 strand MM OM1 fiber cable from Closet B into a new 1RU Rack Mounted Fiber Panel (Century FTS-175) with new 12 port LC adapter panel using the appropriate LC connectors. In both closets A & C the new 1RU panels should replace the existing 1RU panels in place. o 5-6-strand MM Testing for existing fiber between Middle school (Hub A) and Field Allen. This Includes: Details mentioned in line item 3 above and line item 6 below. o 6-6-strand MM w/ ST ends. Testing for existing fiber between Middle school (Hub A) and Central Office. In Field Allen IDF Re-terminate the 1 st 4 strands of the 6 strand MM OM1 Fiber Cable from the Middle School (currently in wall mounted panel with ST terminations) into a new 1RU Rack Mounted Fiber Panel (Century FTS-175 S/TM) with new 12 port LC adapter panel using the appropriate LC connectors. Re-terminate the 1 st 4 strands of the 6 strand MM OM1 Fiber Cable to the Central Office (currently in the same wall mounted panel with ST terminations) into the new 1RU Rack Mounted Panel previously mentioned into the same 12 port LC adapter panel using the appropriate LC connectors. Splice the Slate & White strands of both of the cables together in a splice tray inside the same 1RU Panel. In Central Office Install new wall mounted splice panel (Century FIU-24-S) right where the fiber enters the basement on the plywood. Run a new 6 Strand MM OM1 indoor rated roughly 80 to 100 from that splice panel to the back left side of the basement into a new Rack Mounted 1RU Fiber Panel (Century FTS-175). Splice the 6 strands in the new splice panel and terminate the new 6 Strand MM OM1 into the same 1RU panel previously mentioned with new 12 port LC adapter panels using the appropriate LC Connectors.

This Includes: o 7-6-strand MM w/ ST ends. Install a new 4 post cabinet w/ casters in High School IDF (closet E109) Removed from the Scope of work at this time This includes: o Identifying 22 existing 3M fiber connections currently in use and re-terminating. Removed from the scope of work at this time. o Label and coil on existing overhead ladder tray the remaining 3M fiber connections. - Removed from the scope of work at this time. o No mechanical (Unicam style) terminations allowed anywhere throughout the project (Splice on connectors preferred, pigtails allowed but only if ALL fiber panels they are in are Splice & Term Panels with Splice trays) If using splice on connectors, Term Only panels will be used. o Please supply addition but separate pricing for each of the following: 1- Replacing the existing wall mounted fiber panel in the Blue Portable building with a new Wall mounted Fiber Panel (Century FIU-12-TM) as well as re-terminating the existing 6 strand MM OM1 fiber cable with a new 12 port LC adapter panels using the appropriate LC Connectors. 2- Running new 12 Strand MM OM3 Fiber Cable (Indoor rated) from the MDF in the High School to each of the 2 IDF closets (E109 & H148) including terminating both ends of each cables into new 12 port LC adapter panels using the appropriate LC Connectors. These can be installed into the newly replaced rack mounted fiber panels so no additional ones need to be added. 3- Running new 12 Strand MM OM3 Fiber Cable (Indoor rated) from Closet B in the Primary School to each of the 2 IDF closets C & A including terminating both ends of each cable into new 12 port LC adapter panels using the appropriate LC Connectors. These can be installed into the newly replaced rack mounted fiber panels so no additional ones need to be added

Certification: All locations will be mechanically labeled, OTDR tested, and certified to the performance standard by the vendor. Each MM fiber will be OTDR tested at both 850nm and 1300nm wavelengths. Each SM fiber will be OTDR tested at both 1310nm and 1550nm wavelengths. OTDR Test results will be provided to the owner s representative in a digital format suitable for printing. Total - Section Material and Labor: _$ (enter cost here) VENDOR will have prepared the following budgetary price with the following exceptions and assumptions: This Is a Budgetary price only. VENDOR reserves the right to amend this price once new information is provided. VENDOR assumes all existing and new underground conduit will be large enough to accommodate any new cabling required to be installed and is clear of any debris. All conduit man-holes/pull boxes will be located and accessible prior to cable install. Conduit run will not have more than two man-holes. Correct Cable length is the responsibility of the vendor. New SM Fiber run to Primary School: Work area outlet locations: Supply, install, terminate and test communications cabling to support the outlet locations as specified: 1 12F SM Armored Indoor/Outdoor TB (OFCP) cable from the High School MDF Closet to Closet B of the Primary School. All materials in this quote should match same brands and types as the rest. The Cable will be run in existing and newly installed underground conduits between the two buildings. On both ends it will be fusion spliced with LC type or onto pigtails and housed in new rackmount enclosures provide in the scope of work above. If pigtails are used they must be in a splice and term panel. If splice on connectors are used, a standard term only panel will be used. Certification: All locations will be mechanically labeled, OTDR tested, and certified to the performance standard. Each SM fiber will be OTDR tested at both 1310nm and 1550nm wavelengths. OTDR test results will be provided to the owner s representative in a digital format suitable for printing. Notes and Clarifications: VENDOR is not responsible for any failed tests on existing fiber cabling due to defects not related to installation of the new connectors. VENDOR has Registered Communications Distribution Designers (RCDD s) on staff if structured cabling design services are needed. Quote does not include conduit, core holes, fire-stopping, wire mold, cable tray, or mounting plywood unless specified otherwise in the scope of work above. Any electrical power requirements are the responsibility of the owner. Patch cords are not included in this proposal. They can be priced separately if needed. No electronic equipment (e.g. network switches; media transceivers; telephone system) is included in this quote. Uninterruptible power supplies (UPS) are not included in this proposal. These devices can be provided once the power requirements and run time information is provided. All work is to be done during normal business hours unless indicated otherwise in the scope of work above. All areas to be free of asbestos and lead based paint. This quote does not reflect Federal or State prevailing wage requirements. Terms: Amounts outlined in this Quote are exclusive of all city, state and federal excise taxes, including, without limitation, taxes on manufacture, sales, receipts, gross income occupation, use, and similar taxes. Whenever applicable, any tax or taxes shall be added to the invoice as a separate charge to be paid by the Owner. Additions to this quote will be priced separately as Change Orders. VENDOR shall conduct itself in a manner that is compliant with OSHA. Likewise, Owner must notify VENDOR, in advance of VENDOR commencing its work pursuant to this quote, of any present or potential hazardous condition located at the jobsite(s) at which VENDOR will perform its work. Owner is obligated to provide a worksite that is OSHA compliant. Quote is valid for 30 days from the date below. Owner shall hold harmless and indemnify VENDOR and its respective officers, directors, employees and agents from and against any claim, loss, damages, and liability pertaining to bodily injury, property damage, personal injury, governmental fines or assessments or any other type of claim, arising out of or occurring in connection with the services provided by VENDOR pursuant to this proposal/quote except

to the extent to have resulted from the negligence of VENDOR. Such indemnification shall include but not be limited to attorneys fees incurred in defending such claims. Owner specifically waives any statutory immunity conferred upon it as an employer under any workers compensation act (including but not limited to the Maine Workers Compensation Act, 39-A M.R.S.A. 104, and its successors, the New Hampshire Workers Compensation Act and the Massachusetts Workers Compensation Act). Sealed proposals for furnishing RSU 14 with Fiber Termination and Testing REV.1 will be received by RSU 14 s Assistant Superintendent, (hereafter referred to as Finance Director) 228 Windham Center Road, Windham, ME 04062, until 2:00 PM on May 5, 2017, at which time and place they will be publicly opened and read. Due to the nature of public competitive bidding, the Windham Raymond School District will not accept bids via Fax or E-mail. Bids shall be submitted on the attached form in sealed envelopes, plainly marked Fiber Termination and Testing REV.1 and shall be addressed to the Finance Director at the above-listed address. In determining lowest responsible bidder, in addition to price and compliance with all applicable bid specifications, the Finance Director or Finance Committee where appropriate, shall consider: The ability, capacity and skill of the bidder to perform the bid or provide any services that may be required; whether the bidder can perform the bid or provide any service that may be required promptly, or within the time specified, without delay or interface; the character, integrity, reputation, judgment, experience and efficiency of the bidder; references from non-rsu 14 placements; and the quality of performance of previous bids; the previous existing compliance by the bidder with laws and ordinances relating to the bid; the sufficiency of the financial resources and ability of the bidder to perform the bid;

the quality, availability and adaptability of the equipment to the particular use required; the ability of the bidder to provide future warranty replacement and service for the use of the subject of the bid; the number and scope of conditions attached to the bid; and Such other factors as may be indicated by the Finance Director demonstrating that the bid is not in the Windham Raymond School District s best interests and does not promote the underlying purposes of this policy. It is the custom of the Windham Raymond School District (RSU 14) to pay its bills within 20 to 30 days following delivery of and receipt of bills for all items covered by the purchase order. In submitting bids under the attached specifications, bidder should take into consideration all discounts, both trade and time allowed in accordance with the above-mentioned payment policy. All bidders shall quote net prices, therefore, exclusive of all taxes. Any bidder submitting a bid hereby certifies that no member of the School Committee, agent, or employee of the District is peculiarly interested in the bid; that the proposal is made in good faith without fraud, collusion, or connection of any kind with any other bidder for the same work; and that the bidder is competing solely on his/her behalf without connection with, or obligation to, any undisclosed person or firm. The School Committee reserves the right at its sole discretion to waive any informality or irregularity in any bid, to reject any or all bids, wholly or in part, to call for rebids, to negotiate with any bidder, or to accept any bid even if that bid is not the lowest, if the School Committee deems it to be in the best interest of RSU 14.
