GLOBAL HEALTH SUPPLY CHAIN QUALITY ASSURANCE Medical Device Product Questionnaire This questionnaire is used to collect information from vendors with regards to medical devices that fall in any of the categories below: Product has been reviewed and approved by US FDA, Product is WHO prequalified, and/or Product is CE Marked Products that do not meet the above criteria may be considered on a case by case basis and may require additional documentation at the discretion of USAID GHSC - QA.
