How does Reward Your Health work? Your employees are required to complete the following core requirements: 1. Submit biometric data such as labs, weight and blood pressure. 2. Complete an online health assessment. 3. Meet biometric requirements. If your employees don t meet the biometric requirements, HAP has several convenient options to support them and allow them to earn rewards: Digital Health Assistant, a personalized, online tool Lifestyle coaching by phone (at an additional fee to the employer) At your discretion, optional rewardable wellness activities can be included. Using technology to improve health Reward Your Health combines the latest technology from WebMD Health Services with HAP s istrive for Better Health. istrive is a digital wellness portal with online tools and programs to help your employees easily manage their health, increase their overall well-being and meet their health goals. istrive for Better Health can help your employees reach their goals in: Fitness Weight management Stress management Tobacco cessation Nutrition It also offers in-depth assessments and tips about health issues such as: Asthma High cholesterol Chronic fatigue Diabetes Depression

We ll cover all the details We make it easy for you to start a wellness program for your employees. We ll assign a dedicated wellness coordinator to help administer your program. Frequently Asked Questions Q. Why are blood pressure, total cholesterol, body weight, glucose and non tobacco user chosen as the primary health measures? A: These health measures are strongly linked to chronic illnesses (heart disease, diabetes and cancer) that significantly affect quality of life but can be controlled through personal lifestyle changes such as a healthy diet and regular physical activity. Q: How do our employees earn rewards if they don t meet one or more of the biometrics? A: If they don t meet the criteria for one or more of the biometrics, we offer program options to help them work on healthy behaviors and earn the reward. Q: Are lifestyle coaches available? A: Yes. Fees may apply. Q: Can an employee s spouse participate? A: It s optional and something you decide. Q: What medical information will our company receive? A: Personal health information is strictly confidential and will not be shared with you. HIPAA, the Health Insurance Portability and Accountability Act, prohibits anyone at your place of employment from receiving employees personal health information. However, you ll get a statement of each employee s program completion status so incentives can be rewarded. Q: What if our employees are unable to complete the steps as required for medical reasons? A: Rewards for participating in a wellness program are available to all employees. If an employee believes they are unable to meet a standard for a reward under this wellness program, they can qualify for an opportunity to earn the same reward using reasonable alternatives. Contact HAP and we will work with the employee and their physician to find a wellness program with the same reward that is right for them in light of their health status.
