Introduction This document has been developed to address quality management systems for product supply organizations in the petroleum and natural gas industry. When coupled with applicable customer-specific requirements, it identifies the fundamental quality management system requirements for those claiming conformity to the requirements of this document. The requirements of this document are consistent with those of other quality management system documents (for example, API Q2). The requirements are structured in a way to minimize the likelihood of nonconformity. While this document may include some elements of other management systems, it does not include all requirements specific to those systems, such as those particular to environmental management, occupational health and safety management, financial management or risk management. This document may be used either in conjunction with or independent of other industry-specified documents. This document can be used by internal and external parties, including certification bodies, to assess the organization s ability to meet customer, statutory and regulatory requirements applicable to the product and the organization s own requirements. This document promotes the integration of a process approach into the application of specific clauses when developing, implementing and improving the effectiveness of a quality management system. This provides for continuous control over the stated requirements, as well as facilitating the overlap of processes. For a product supply organization to function effectively, it has to determine and manage numerous linked activities. An activity that transforms inputs into outputs can be considered a process. Process activities include determination of need throughout the product supply organization, provision of resources and product realization, identification of the proper sequence or order in a series of activities, monitoring and measuring the effectiveness of the activities performed, applying changes or corrections to those activities as needed. Goal of the document The goal of this document is to provide the minimum requirements for the development of a quality management system that provides for continual improvement, emphasizes defect prevention and strives to minimize variation and waste from product supply organizations. It is designed to promote reliability in the manufacturing sector of the petroleum and natural gas industry. It is not the intent of this document to imply uniformity in the structure of quality management systems or uniformity of documentation. Relationship of Spec Q1 to Spec Q2 This document establishes the API quality system requirements necessary for organizations to consistently and reliably manufacture products in accordance with API or other specifications for use in the petroleum and natural gas industry. This specification is also applicable to, and sets requirements for, all organizations desiring to acquire and maintain a license to use the API Monogram and applies at all times within the organization when a location has been licensed by the API Monogram program. This includes those activities that otherwise may be considered a service (such as heat treatment, threading or testing) if these activities or the results of these activities are identified as an API Monogrammable product under any applicable API product specification in the API Monogram program. API Spec Q2 (Specification for Quality Management System Requirements for Service Supply Organizations for the Petroleum and Natural Gas Industries) establishes the API quality system requirements necessary for service supply organizations to consistently and reliably provide services that meet customer, legal and other applicable requirements. This specification applies to service-related activities in oil and gas well construction, intervention, production, and abandonment, as well as equipment repair/maintenance. This specification does not apply to the API Monogram program or any product that is identified by license as eligible for marking with the API Monogram. ix

API Specification Q1 Specification for Quality Programs for the Petroleum, Petrochemical and Natural Gas Industry 1 Scope This document establishes the minimum quality management system requirements for product and service supply organizations for the petroleum, and natural gas industries. It is intended to apply to the manufacture of products and the provision of related services associated with equipment used in the petroleum and natural gas industry. This document specifies requirements of a quality management system for an organization to demonstrate its ability to consistently provide reliable products and related services that meet customer and legal, statutory, regulatory, and other applicable requirements. If an organization performs any requirements of this document, no claims to exclusion of those requirements are permitted. Where any requirement of this document cannot be applied due to the nature of an organization, the requirement can be considered for exclusion. Where exclusions are made, the basis for claiming exclusions are to be identified and such exclusions not affect the organization's ability, or responsibility, to provide products and related services that meet customer and applicable regulatory requirements. 3 Terms, definitions and abbreviations 3.1 Terms and definitions For the purposes of this document, the terms and definitions given in ISO 9000 and the following shall apply. When identical terms are defined in ISO 9000 and this document, the following definitions shall apply. 3.1.1 acceptance criteria specified limits of acceptability applied to process or product characteristics 3.1.2 acceptance inspection demonstration through monitoring or measurement that the product complies with specified requirements 3.1.3 calibration comparison and adjustment to a standard of known accuracy 3.1.X.X calibration (from Q2) comparison and adjustment to a standard of known accuracy 3.1.X.X collection (from Q2) process of obtaining, assembling and/or organizing applicable documentation with the intent of meeting the requirements of 4.2.3 3.1.X.X compliance (from Q2) act or process of satisfying the legal, statutory, regulatory, and other applicable requirements of a regulation or regulatory body 3.1.4 control feature organization s documented method to perform an activity under controlled conditions to achieve conformity to specified requirements 3.1.X.X critical that defined by the organization, customer or applicable product specification and deemed as indispensible or essential, impacting the purchased product or subsequent product realization or final product, and requiring specific action 3.1.X.X critical success factor (from Q2) element of service that is essential for the customer to achieve its goals or stated objectives 3.1.5 delivery point in time and physical location at which the agreed transfer of ownership takes place 3.1.6 design acceptance criteria defined limits placed on characteristics of materials, products, or services established by the organization, customer, and/or applicable specifications to achieve conformity to the product design 2

3.1.7 design validation process of proving a design by testing to demonstrate conformity of the product to design requirements NOTE Design validation can include one or more of the following (this is not an all inclusive list): a) prototype tests b) functional and/or operational tests of production products c) tests specified by industry standards and/or regulatory requirements d) field performance tests and reviews 3.1.8 design verification process of examining the result of design or development output to determine conformity with specified requirements NOTE Design verification activities can include one or more of the following (this is not an all inclusive list): a) confirming the accuracy of design results through the performance of alternative calculations b) review of design output documents independent of activities of design and development c) comparing new designs to similar proven designs 3.1.9 field nonconformity product nonconformity that is detected after delivery or use has started 3.1.X.X first article: representative sample of a product, component or output from a process used to verify that prescribed production methods have satisfied the requirements as specified by the organization. 4 Quality Management System Requirements 4.1 Quality Management System 4.1.1 General The organization shall establish, document, implement and maintain at all times a quality management system for all products provided for use in the petroleum and natural gas industry. The organization shall measure the effectiveness and improve upon the quality management system in accordance with the requirements of this document. The quality management system shall be in a form suitable for the organization s method of operations. 4.1.2 Quality Policy Comment [b3]: Intended to meet API Spec Q1, Section 7.1; subject to further scrutiny. The organization s policy for its commitment to quality shall be identified, documented, and approved by top management. The organization s top management shall review the quality policy (see X.X.x) to ensure that it is appropriate to the organization, is the basis for the development of quality objectives (see X.X.x), and is communicated, understood, implemented and maintained at all relevant functions and levels within the organization. The policy shall include a statement by the organization for its commitment to comply with requirements and continually improve the effectiveness of the quality management system 4.1.3 Quality Objectives Management, with approval from top management, shall ensure that quality objectives, including those needed to meet product and customer requirements, are established at relevant functions and levels within the organization. The quality objectives shall be measurable and consistent with the quality policy. 4.1.5.2 External The organization shall determine, document and implement the process for communicating with external organizations, including customers, to ensure requirements are fully understood and risk is managed (see X.X) throughout execution of contract and product realization including activities such as Comment [b6]: Intended to meet API Spec Q1, Section 7.2.3 a) execution of inquiries, contracts or order handling (see X.X) and amendments (see X.X) b) provision of product information, including product nonconformities (see X.X) c) service quality plans and subsequent changes (see X.X), d) addressing feedback and complaints. [Need to add Management of Change? ] 4.2 Management responsibility 4.2.1 Organization Structure Top management shall ensure the availability of resources essential to establish, implement, maintain and improve the quality management system. NOTE Resources can include human resources and specialized skills, organizational infrastructure, technology and financial resources. Management shall provide evidence of its commitment to the development and implementation of the quality management system and continually improving its effectiveness by a) communicating to the organization the importance of meeting customer as well as legal and other applicable requirements b) ensuring that quality objectives are established including key performance indicators for use in data analysis, and c) conducting management reviews (see X.X). 4.2.2 Responsibility and Authority Roles, responsibilities and authorities shall be defined, documented, assigned within, and communicated throughout the organization. 4.2.3 Management Representative Top management shall appoint a member of the organization s management who, irrespective of other responsibilities, shall have responsibility and authority that includes a) ensuring that processes needed for the quality management system are established, implemented and maintained, b) reporting to top management on the performance of the quality management system and any need for improvement, c) initiating action(s) to minimize the likelihood of the occurrence of nonconformities, and d) ensuring the promotion of awareness of customer requirements throughout the organization. 7

NOTE The responsibility of a management representative can include liaison with external parties on matters relating to the quality management system. 4.3 Organization Capability 4.3.1 Provision of Resources The organization shall determine and provide the resources needed to implement, maintain, and improve the effectiveness of the elements of the quality management system. Comment [b7]: Intended to meet API Spec Q1, Section 7.1b 4.3.2 Human Resources 4.3.2.1 General The organization shall maintain a documented procedure for defining personnel competency and identifying training requirements. The procedure shall identify the required training or other actions to achieve the necessary competency of all personnel. The procedure shall include provisions for determining the effectiveness of the actions taken toward the achievement of required competency. 4.3.2.2 Personnel Competence Personnel performing activities shall be competent based on the appropriate education, training, skills and experience needed to meet product and customer requirements. Evidence of the determination of competence of personnel shall be recorded and maintained (see X.X). 4.3.2.3 Training and Awareness The organization shall a) provide for quality management system training and for job training of the organization s personnel and contractors who affect execution of service or provision of service-related product b) ensure that customer-specified training and/or customer-provided training, if required, is included in the training program c) ensure that the frequency and content of training complies with legal and other applicable requirements d) ensure that its personnel are aware of the relevance and importance of their activities and how they contribute to the achievement of the quality objectives e) identify and provide training on legal and other applicable requirements, and f) maintain appropriate records of education, training, skills and experience (see X.X). 4.3.3 Work environment The organization shall determine, provide, manage, and maintain the work environment needed to achieve conformity applicable to the manufacture of the product. Work environment includes, as applicable a) buildings, workspace and associated utilities b) process equipment and its maintenance (both hardware and software) (see X.X) c) supporting services (such as transport or communication or information systems), and d) conditions under which work is performed including physical, environmental and other factors 8

4.4 Documentation requirements 4.4.1 General Formatted Table The quality management system documentation shall include a) statements of quality policy and quality objectives b) a quality manual that addresses each requirement of this document and includes 1) the scope of the quality management system, including justification for any exclusions to specific quality management system elements (see XXX), 2) identification of legal, statutory, regulatory, and other applicable requirements to which the organization claims compliance, 3) a description of the interaction between the processes of the quality management system, and 4) identification of processes that require validation 5) reference to documented procedures that control the quality management system processes. c) documented procedures established for the quality management system, and d) documents, including records, to ensure the effective planning, operation and control of its processes, and compliance with specified requirements. NOTE 1 Records are a special type of document controlled according to the requirements of X.X. NOTE 2 A single document can address the requirements for one or more procedures. Documentation can be in any format or type of medium. 4.4.2 Control of Documents The organization shall maintain a documented procedure for the identification, distribution and control of documents required by the quality management system and this document, including required documents of an origin external to the organization. The procedure shall identify the controls needed to ensure that the documents required by the quality management system, including revisions, translations and updates, are reviewed and approved for adequacy prior to issue and use. Responsibilities for approval and re-approval of these documents shall be specified. The procedure shall identify controls to ensure that documents remain legible and readily identifiable, changes and revision status are identified, and appropriate documents are available where the activity is being performed. 9

Documents of external origin shall be controlled to ensure that the relevant versions are used and maintained. Obsolete documents shall be removed from all points of issue or use, or otherwise identified to ensure against unintended use if they are retained for any purpose. A master list or equivalent shall be used to minimally identify procedures, work instructions and forms required by the quality management system, and their current revision status. 4.4.3 Use of External Documents in Product Realization When API or other industry product specification requirements, including addenda, errata and updates, are used in the design or manufacture of the product, the organization shall maintain a documented procedure for the translation of these requirements into the product realization process and any other affected processes. 4.5 Control of records The organization shall maintain a documented procedure to identify the controls needed for the initiation, identification, collection, storage, protection, retrieval, retention time, individuals responsible for collection and disposition of records. Records, including those from outsourced activities, shall be established and controlled to provide evidence of conformity to requirements and of the effective operation of the quality management system. Records shall remain legible, identifiable, and readily retrievable. Records shall be retained for a minimum of five years or as required by customer, legal, statutory, regulatory, and other applicable requirements, whichever is longer. Comment [b9]: Intended to meet API Spec Q1, Section 7.1.1 Comment [b10]: Intended to meet API Spec Q1, Section 7.1d Comment [b11]: Consider all uses of the word outsource and determine the best location in the document and any required definitions to coincide with purchasing activities (10/25/11) 5 Product Realization 5.1 Contract Review 5.1.1 General The organization shall maintain a documented procedure for the review of requirements related to the provision of products. 5.1.2 Determination of Requirements The organization shall determine a) requirements specified by the customer, b) legal and other applicable requirements, and c) requirements not stated by the customer but considered necessary by the organization for the provision of the product. Where the customer provides no documented statement of the requirements, the customer requirements shall be affirmed by the organization and records maintained (see 4.5). 5.1.3 Review of Requirements The organization shall review the requirements related to provision of products. This review shall be conducted prior to the organization s commitment to deliver product to the customer and shall ensure that a) requirements are identified and documented 10

b) requirements differing from those previously identified are resolved, and c) the organization has the capability to meet the documented requirements. Where contract requirements are changed, the organization shall ensure that relevant documents are amended and that relevant personnel are made aware of the changed requirements. Records of the results of the review, including resulting actions, shall be maintained (see 4.5). 5.2 Planning ]The organization shall identify and plan the processes and documents needed for product realization. Planning of product realization shall be consistent with the requirements of the other processes of the quality management system (see X.X). The procedure shall a) identify the techniques, tools and their application used in the identification of risk (potential or real), b) identify the methods used to assess risk, and c) identify controls for the selection, communication and implementation of the mitigation or preventive control measures to reduce risk. Risk assessment and its management shall be part of the annual management review. Records of risk management and actions taken shall be maintained (see X.X). 5.4 Design and Development 5.4.1 Design and Development Planning Comment [b15]: Ensure that there is language in this section on outsourcing of designs. A documented procedure to plan and control the design and development of the product shall be maintained. 11

The procedure shall identify e) the plan(s), including plan updates, used for the development of designs f) the design and development stages g) the resources, responsibilities, authorities and their interfaces to ensure effective communication, h) the review, verification and validation activities necessary to complete each design and development stage, and i) the requirements for the independent final review of the design (see X.X) When design and development activities are performed at different locations within the same organization, the procedure shall identify the controls required to ensure that the designs meet specified requirements. When design and development are outsourced, the organization shall ensure the supplier meets the requirements of X.X (see X.X [outsourcing]). NOTE Design and developmental review, verification and validation stages have distinct purposes. They can be conducted and recorded separately or in any combination, as suitable for the product and the organization. 5.4.2 Design and Development Inputs Design and development inputs shall be identified and reviewed for adequacy, completeness and lack of conflict. Design inputs shall include functional and technical requirements, and the following, as applicable: a) customer-specified requirements (see X.X), b) requirements provided from external sources, including API product specifications, c) environmental and operational conditions, d) methods, assumptions and formulae, e) historical performance and other information derived from previous similar designs, f) legal requirements, and g) results from risk assessments (see X.X) Design input records shall be maintained (see X.X). 5.4.3 Design and Development Outputs The outputs of design and development shall be documented to allow verification against the design and development input requirements. Outputs shall a) meet the input requirements for design and development, b) provide applicable information for purchasing, production and post-delivery service provision c) identify or reference design acceptance criteria, 12

d) include applicable calculations, and e) specify the characteristics of the product that are essential for its safe and proper use. Outputs shall be approved prior to release to the next stage in the development process and shall be controlled in accordance with X.X (records section). Comment [b16]: Need to move? 10/26/11 5.4.4 Design and Development Review At suitable stages, reviews of design and development shall be performed a) to evaluate the suitability, adequacy and effectiveness of the results of design and development stages to meet specified requirements, and b) to identify any problems and propose necessary actions. Participants in such reviews shall include representatives of functions concerned with the design and development stage(s) being reviewed. Records of the results of the reviews and any necessary actions shall be maintained (see X.X). A final design review shall be conducted and documented by competent (see X.X) individual(s) other than the person or persons who developed the design. Records of the final review shall be maintained (see X.X). 5.4.5 Design and Development Verification Design verification shall be performed in accordance with planned arrangements (see X.X) to ensure that the design and development outputs have met the design and development input requirements. Records of design verification and any necessary actions shall be maintained (see X.X). 5.4.6 Design and Development Validation Design validation shall be performed in accordance with planned arrangements (see X.X) to ensure that the resulting product is capable of meeting the specified requirements. Validation shall be completed prior to the delivery of the product, when possible. Records of the design validation and any necessary actions shall be maintained (see X.X). 5.4.7 Design Approval and Release All activities of the design and development stages shall be confirmed to ensure the suitability, adequacy and effectiveness of the design activities and to approve the completed design. Records of design approval shall be maintained for a minimum of five year or the lifetime of the design whichever is greater. 5.4.8 Design and Development Changes Changes in the approved design shall be reviewed and verified by the management of change process (see X.X). Design and development changes shall be identified and records maintained. The changes shall be reviewed, verified and validated, as appropriate, and approved before implementation. The review of design and development changes shall include evaluation of the effect of the changes on product and/or their constituent parts already delivered. Design and development changes including changes to design documents, shall require the same controls as the original design and development, and design documentation. Records of the review of changes and any necessary actions shall be maintained (see X.X). 13

5.5 Contingency Planning (Business Continuity Management) 5.5.1 General The organization shall maintain a documented procedure for contingency planning. The procedure shall include incident and disruption prevention and mitigation measures. Contingency planning shall be integrated into production and supporting processes between the organization, its suppliers and the customer. The organization shall prepare contingency plans to mitigate the event of an emergency such as utility interruptions, interruptions in the Supply Chain, labor shortages, key equipment failure and field returns, taking into account the output of the resources analysis including a succession plan. (International Railway Industry Standard/Herick Lopez) 5.5.2 Planning Output Contingency planning output shall be documented and communicated to the relevant operational personnel, and updated as required to minimize the likelihood or duration of disruption of production. The contingency plan shall be based on assessed risks (see X.X) and shall include, at a minimum a) actions required in response to significant risk scenarios b) actions required to mitigate effects of disruptive incidents c) identification and assignment of responsibilities and authorities, and d) internal and external communications controls (see X.X). 5.6 Purchasing 5.6.1 Purchasing Control 5.6.1.1 Procedure The organization shall maintain a documented procedure to ensure that purchased or outsourced services or products conform to specified requirements. The procedure shall address a) the determination of the criticality of the services or products as they are applicable to conformance to product or customer specifications b) the evaluation and selection of suppliers based on their ability to supply services or products in accordance with the organization s requirements c) the type and extent of control applied to the supplier based on the criticality of the service or product d) criteria, scope, frequency and methods used when performing an assessment on a supplier, and e) maintaining a list of approved suppliers and scope of approval f) the re-evaluation of suppliers g) the type and extent of control to be applied to outsourced processes (see X.X). 5.6.1.2 Initial Supplier Evaluation - Critical For purchase of critical products, components or manufacturing processes, the criteria for the initial evaluation of 14

suppliers by the organization shall be site-specific and shall include the following: a) verification that the supplier s quality management system conforms to the quality system requirements specified for suppliers by the organization, and b) assessment of the supplier to ensure capability to meet the organization s purchasing requirements by performing an on-site evaluation of relevant manufacturing processes at the supplier s premises, or by evaluation of the results of first article inspection to ensure conformance to stated requirements. 5.6.1.3 Initial Supplier Evaluation Noncritical For purchase of noncritical products, components or manufacturing processes that impact product realization or the final product, the criteria for evaluation of suppliers by the organization shall include one or more of the following: a) assessment of the supplier to meet the organization s purchasing requirements b) verification that the supplier s quality management system conforms to the quality system requirements specified for suppliers by the organization c) assessment of the supplier upon delivery of the product or service 5.6.1.4 Supplier Re-evaluation For re-evaluation of all suppliers (critical and noncritical), the requirements of 5.6.1.3 shall apply. 5.6.1.5 Supplier Evaluation Records Records of the results of all evaluations and any necessary actions arising from the evaluations shall be maintained (see X.X). In instances when supplier assessment results in the need for actions, evidence of effective implementation of such actions shall be maintained in accordance with X.X. 5.6.2 Outsourcing Where an organization chooses to outsource any process within the scope of the quality management system, the organization shall ensure control over such processes and ensure that all applicable parts of the quality management system are satisfied. The organization shall identify the type and extent of control to be applied to these outsourced processes and shall maintain responsibility for product conformance to specified requirements. Records of outsourced activities shall be maintained (see X.X). 5.6.3 Purchasing Information The organization shall ensure the adequacy of specified purchasing information prior to their communication to the supplier. Purchasing information provided to the supplier shall be documented and adequately describe the service or product to be purchased, including where appropriate a) requirements for acceptance criteria of service and product b) requirements for approval of supplier s procedures, processes and equipment c) applicable version of specifications, drawings, process requirements, inspection instructions and other relevant technical data d) requirements for qualification of supplier s personnel, and e) quality management system requirements. 15

5.6.4 Verification of Purchased Services or Products The organization shall maintain a documented procedure for the verification or other activities necessary for ensuring that purchased services or products meet specified purchase requirements. Where the organization or its customer intends to perform verification at the supplier s premises, the organization shall state the intended verification arrangements and method of product release in the purchasing information The organization shall ensure and provide evidence that outsourced services and products conform to specified requirements. The organization shall maintain records of verification activities (see X.X). 5.7 Production and Servicing Provision 5.7.1 Control of Production and Servicing 5.7.1.1 Production and Servicing Procedure The organization shall maintain a documented procedure that describes activities associated with the production and servicing of products. The procedure shall describe the following activities as a minimum: a) the availability of information that describes the characteristics of the product, b) ensuring design requirements are satisfied, as applicable (see X.X) c) the availability and use of suitable production, testing, monitoring and measurement equipment d) the availability of work instructions, as necessary e) ensuring any required traceability of production, testing, monitoring and measurement equipment (see X.X) f) the implementation of monitoring and measurement activities, and g) the implementation of product release, including final acceptance criteria (see X.X), delivery and post-delivery activities 5.7.1.2 Process Control Documents Process controls shall be documented in routings, travellers, checklists, process sheets or equivalent controls required by the organization and shall include requirements for verifying conformance with quality plans (see X.X), API product specifications and/or other applicable product standards/codes. The process control documents shall include or reference instructions, workmanship and acceptance criteria for processes, tests, inspections, and customer s inspection hold or witness points. 5.7.1.3 Product Realization Capability Documentation The organization shall develop and maintain documentation that includes but is not limited to product realization plans and records of review/verification, validation, monitoring, measurement, inspection and test activities, including criteria for product acceptance that demonstrates the capability of the organization to satisfy specified product and/or servicing requirements. NOTE Product realization documentation is evidence of the capability of the organization to manufacture products or families of products and does not extend to every work order or individual product manufactured. 16

5.7.1.4 Validation of Processes for Production and Servicing The organization shall validate any processes for production and servicing where the resulting output cannot be verified by subsequent monitoring or measurement and as a consequence, deficiencies become apparent only after the product is in use or the service has been delivered. Validation shall demonstrate the ability of these processes to achieve planned results. Where an organization chooses to outsource any process that requires validation, the organization requires that the supplier conform to the requirements of X.X, (see X.X [Outsourcing]) The organization shall establish documented procedures for these processes including, as applicable a) methods for review and approval of the processes b) identification of the acceptance criteria of the processes c) approval of equipment d) qualification of personnel e) use of specific methods f) requirements for records (see 4.2.4), and g) revalidation. The organization shall validate those processes identified by the applicable product specification as requiring validation. If these processes are not identified, or there is no product specification involved, the processes requiring validation shall include, as a minimum, non-destructive examination, welding and heat treating, if applicable to the product. 5.7.1.5 Servicing Procedure The organization shall maintain a documented procedure that describes activities associated with the servicing (see 3.1.X.X) of products. The procedure shall describe the following activities as a minimum: a) review and implementation of the organization s servicing requirements b) review and implementation of customer-specific servicing requirements c) the availability and use of required servicing equipment, including suitable testing, monitoring and measurement equipment d) the availability of work instructions, as necessary e) the implementation of monitoring and measurement activities, including acceptance criteria and/or performance requirements f) the release or transfer of custody to the customer [Spec Q1, 5 th & 6 th Edition]: Where servicing is a specified requirement, the supplier shall establish and maintain documented procedures for performing, verifying, and reporting that the servicing meets the specified requirements. 5.7.2 Product Quality Plans 5.7.2.1 General Comment [b17]: Waiting on Stacy on draft language 1/18/12 17
