Welcome WELCOME to the Jewish Family & Children s Service Wellness Program. We congratulate you on your decision to take ownership of your health through participation in this voluntary program. This year you will be empowered to become deeply engaged in living a healthy and productive lifestyle. You will also have the chance to earn a cost reduction on your medical premiums by participating in the biometric screening (if participating in JFCS medical plan) and a reward in 2017. Your wellness provider, Kersh Health, is bound by HIPAA, State and Federal law to maintain the confidentiality of your personal information. Privacy and confidentiality is our priority. We strongly encourage you to read through this Program Guide completely. You will find step-by-step instructions and details on the Wellness Program and Incentive Plan. Your wellness plan is designed to help you achieve your best health. If you are unable to meet a biometric testing result standard under this wellness program due to a disability, contact Kersh Health at (800) 467-3005. We will work with you on a one-on-one basis to determine an alternative for you. Wellness program rewards will be announced at a later date. Best wishes your upcoming wellness journey! Enrollment All employees enrolled in the JFCS medical plan may participate in the -17 Wellness Program. Once enrolled, the goal is to keep track of your activities on the Incentives page of your Kersh account and stay on top of your wellness. Enrolling in the wellness program requires you to log into your Kersh Health account. Please register your account by going to KershHealth.com, click on Member Login then Register as a New User. After registering, you ll be redirected to your Kersh account portal. To receive the -17 premium discount, complete these two required steps: 1. The online Qscore assessment 2. Biometric testing Step One: Take your Qscore: 1. Complete your Qscore by logging into your Kersh account at www.kershhealth.com. 2. Click on the Qscore tab and complete the 15 minute health assessment. 3. The Qscore is available: March 1 May 31,. Step Two: Complete biometric testing: 1. Complete biometric testing on-site in May. Deadline to complete biometric testing and submit results is May 31,. 1. Click on the Biometric Scheduler tab to signup for an appointment. Fasting is required. This means nothing to eat or drink for 8-12 hours prior to testing, with the exception of water and prescribed medications. New Hires June 1, and later: refer to page 6 for details about how to enroll. ** Biometric screening and Qscore components are optional, participants can still actively participate in the Wellness program to earn towards 2017 rewards. In order to receive the medical premium discount and KAM device, Biometric screening and Qscore are required. App to download! The Kersh Health App: You can take your Qscore, complete your Education and monitor your incentive! This App is free and available in the ios and Android Store. Pg. 3

The Qscore & Biometrics THE QSCORE The Qscore is a powerful tool aimed at identifying the health risks that exist in your life. After completing a short but thorough assessment of your health and lifestyle, Kersh combines this information with your biometric data (if available) to determine your Qscore. Each participant will receive a Qscore between 1 100. This number represents where you are in line to see a doctor. An individual with a lower Qscore will be towards the front of the line and may need medical attention before an individual with a higher Qscore. The higher the Qscore the healthier you are! Your health is not ruled by chance it is the outcome of your environment, your genetics, and your choices. Based on your assessment, you will receive recommendations on ways to achieve a higher Qscore. Just by making a few healthier choices, you can have a significant impact on your health. Your optimum Qscore helps you see what healthier choices can do for your score. Biometric Section of the Qscore Assessment: Please make sure to selfreport your biometric results. Kersh will update this portion with your actual results approximately two weeks after you attend onsite testing in May. BIOMETRICS During the enrollment process, baseline biometrics are performed either on-site or through LabCorp. Completion of your biometric testing is a requirement in order to receive a medical premium discount. All participants will receive their baseline biometrics and the wellness premium discount. Testing takes about 10 minutes and all results are confidential. Remember to fast at least 8 hours before testing. Continue to drink water and take all prescribed medications. The test consists of: Blood Pressure Total Cholesterol HDL LDL Triglycerides Glucose (Blood Sugar) A second round of biometrics will take place in May 2017. The results will be compared against your results to determine whether you met the goals or improvement criteria. You are required to meet 4 out of 6 of the goals and enroll into the 2017-18 Program to earn a 2017-18 premium discount. New hires after December 31,, please refer to page 10. Pg. 4

-17 Incentive Plan New Hires June 1 December 31, : Refer to the Pro-Rated Incentive Plan Incentive Points Program Components Your 2017 Reward Level will be based on your participation in the activities listed below. Qscore (Requirement for the -17 premium discount) 5 incentive Current Employees: Complete the Qscore Assessment at KershHealth.com between March 1 May 31,. New Hires: Complete the Qscore Assessment at KershHealth.com within 30 days of your benefit eligible date. Biometric Testing (Requirement for the -17 premium discount) 5 incentive Current Employees: Complete biometric testing between April 1 May 31, on-site or through LabCorp. New Hires: Complete biometric testing within 30 days of your benefit eligible date. Exercise: (July May 2017) 220 incentive (20 pts/month) Tier 1: 15 20 KAM per day for at least 15 days per month = 15 incentive Tier 2: 21+ KAM per day for at least 15 days per month = 20 incentive Sync your KAM/fitness device at least once every 45 days Use of Activity Log will only achieve Tier 1 Education: (June May 2017) Low Risk Individuals: Qscore of 61 and above 120 incentive (10 pts/month) 80 incentive (40 pts/challenge) Read a health article or watch a video and take the quiz at KershHealth.com, or speak with a Registered Dietitian (RD) on a monthly basis through the Q-Quest program. High Risk Individuals: Qscore of 60 and below Speak with a Registered Dietitian (RD) on a monthly basis through Q-Quest or complete the 12-week Right Weigh program. Eligible participants will be contacted and asked to participate in RW and Q-Quest based on Qscore and biometric results. Find more details about each program on page 7. Wellness Challenges: Participate and meet identified goals for each challenge as noted below. Make the Switch: August of KAM Madness: February of 2017 35 incentive Annual Physical/Preventative Screening Exams between March 1, May 31, 2017 will be accepted. Complete and submit the form at KershHealth.com under the Health Activities tab. Deadline to submit online form is May 31, 2017. Limit one form per Wellness Year. 35 incentive Annual Dental Check-Up Exams between March 1, May 31, 2017 will be accepted. Complete and submit the form at KershHealth.com under the Health Activities tab. Deadline to submit online form is May 31, 2017. Limit one form per Wellness Year. 500 incentive BONUS! Up to 200 Bonus Points Earn up to 500 incentive from the activities above. Green Light Events Participate in an organized event, such as a marathon or community walk/run. Event must occur between March 1, May 31, 2017. Complete and submit the form at KershHealth.com under the Health Activities tab. Points will cap at 200. Deadline to submit is May 31, 2017. Pg. 5

A look ahead... 2017 Health Outcomes This portion of the Wellness Program is based on your 2017 biometric testing results. The chart below describes the Goals you will need to meet to receive the premium discount starting July 2017. You must meet 4 out of 6 the Goals and/or Improvement Goals. Additionally, you must complete the 2017 Health Assessment. Education & Right Weigh Now that you ve completed your Qscore and biometrics, what can you do to earn your Education? 1. Is your Qscore 61 or higher? Stay informed with health education. Read a health article/newsletter or watch a video once a month. These articles/videos can be accessed by logging on to your health portal from KershHealth.com and clicking the Education tab. Articles are also available on the Kersh Health app! 2. Is your Qscore lower than 61? Enroll in Q-Quest and be on your way to a healthier you! Q-Quest is a free, telephonic lifestyle modification program designed to help you understand and optimize your Qscore, ultimately improving your biometrics. Kersh will outreach to eligible participants. You will have monthly calls with a Registered Dietitian that focus on lifestyle and dietary changes. Your Dietitian will set specific goals to help you: Improve your health and reduce stress Lose weight Increase KAM Change unhealthy eating habits Minimize need for certain medications Call 800.467.3005. Email qquest@kershhealth.com 3. Struggling with your weight? RightWeigh could be what you need Right Weigh is an innovative, and effective 12-week weight Management program. This program was designed by medical professionals to teach individuals how to Think, Eat and Move their way to decreased weight an optimized health. Right Weigh is available to participants with a BMI of 27 and above. Eligible participants will be contacted via email. Those who enroll and successfully complete the program will have $250 ($41.67 per paycheck) payroll deducted through 6 pay periods. Participants who enroll and do not complete the program will be charged the full program fee of $500 through 12 pay periods. Pg. 7

Activity & Wellness Challenges 1. The KAM Device The KAM activity monitor is water resistant. A KAM will be mailed directly to your home address approximately two weeks after you have completed your Qscore and biometrics. What is a KAM? The Kinetic Activity Monitor (KAM) is an accelerometer designed to measure human activity. It records the intensity, duration and frequency of your daily activity. The KAM device holds information for 45 consecutive days, but we recommend syncing your KAM more often to avoid lost data. Please note the time displayed on the KAM correlates with the time on the computer you sync to, which may change based on time zones. Monthly exercise incentive are earned by reaching your daily KAM goals. The exercise requirement is listed below. Tier Requirement Incentive Points 1 15 20 KAM /day for at least 15 days/month 15 2 21+ KAM /day for at least 15 days/month 20 Proper way to wear KAM device: The KAM should be worn on the waist, in-line with the knee. During an activity where your hips are stationary, i.e. using a stationary bike or elliptical trainer, the KAM should be worn on the shoe or sock. Kersh Health KAM App: Free and available in the ios and Android Store. Lost or broken KAM: Contact KAM customer service immediately if your KAM is broken or lost. You will be credited for one month of KAM Tier 1 incentive once you have reported the issue to Kersh. JFCS will pay the cost for one replacement KAM per calendar year. You will be charged $50 for subsequent replacements per calendar year. Fitbit Device Integration: Participants using a Fitbit have the option to use this device in lieu of the KAM. JFCS will provide a one time $30 reimbursement to participants that wish to use a Fitbit (can be a previous or newly purchased device). Please contact Human Resources if you choose to use a Fitbit instead of a KAM for the program year. Follow the instructions in your Kersh account to upload your Fitbit data and earn incentive for the Exercise portion of the Wellness Program. Please note that Fitbit data storage capacity may vary from the KAM. Kersh Health nor JFCS will offer technical support or reimbursements for lost or broken Fitbits. The Activity Log: If you are without your KAM or Fitbit, you can record activity in the Activity Log in your Kersh portal for up to 60 days. You will earn tier 1 level credit if you record at least 15 days worth of activity. Wellness Challenges Two wellness challenges will be offered during the -17 program year. An email will be sent approximately two weeks before the challenge is set to begin. This email will contain all the details surrounding the challenge and the minimum criteria that must be met in order to earn incentive. inside, out. Nutrition Challenge: This challenge is designed to help you make smarter choices about your food and beverage intake, making you healthy from the Activity Challenge: This challenge will require the use of your KAM device/fitbit. The activity challenge will be team-based with individual components as well. Pg. 8

QuitLogix, Prevention & Green Lights Are you a tobacco user? QuitLogix could be for you. QuitLogix is an effective and comprehensive program for tobacco prevention and cessation for employees. This program employs the following components: trained coaches who use a behavioral change model for smoking cessation, nicotine replacement or smoking cessation therapy and education materials. It includes personalized, telephonic-based coaching, customized support materials, text messaging and email support. Eligible participants will be contacted via email. Those enrolled in the telephonic service, who are medically eligible, will be offered 8 weeks of NRT (patches, gum, lozenges) for an additional fee. Please review your health plan for free NRT options. Participants who successfully complete the program will have $100 payroll deducted through 8 pay periods. Those who enroll and do not complete the program will have an additional $200 payroll deducted through 12 pay periods. Preventive visits are an important step in maintaining good health. Advantages include regular checks for disease, increasing your chance for early detection and improved recovery, the opportunity to get any needed vaccines, and to discuss with your doctor recommendations for lifestyle changes and additional screenings based on age and family history. Annual Physical/Preventative Screening: Exams between March 1, May 31, 2017 will be accepted. Complete and submit the form at KershHealth.com under the Health Activities tab. Deadline to submit online form is May 31, 2017. Limit one form per Wellness Year. Annual Physical Pap Smear or Mammography Prostate Specific Antigen (PSA) Colonoscopy Dental Check-Ups: Exams between March 1, May 31, 2017 will be accepted. Complete and submit the form at KershHealth.com under the Health Activities tab. Deadline to submit online form is May 31, 2017. Limit one Dental form per Wellness Year. Whether you are just starting out in a couch to 5K program or you are training for a marathon, we want to reward you for your accomplishments. Event or visit must occur between March 1, May 31, 2017. Submission Instructions: Log onto to your account at www.kershhealth.com and click on the Health Activities tab Click the Green Light Events icon and enter requested information. Remember to click Submit. Green Light Events will be reviewed on a monthly basis by Kersh Health. Additional information may be requested in order to approve. An approval or declined notice will appear once the audit is complete and incentive will be awarded accordingly. Pg. 9

New Hires This section applies to employees hired between June 1, December 31,, who wish to receive a medical premium discount. Please complete the following steps within 30 days of your benefits eligibility date to enroll. Please note that you will not be able to create a Kersh account until your benefits eligibility date. Step One: Create your Kersh account: 1. Create an account at KershHealth.com/jfcs. Your employee ID is required. 2. To find your JFCS employee ID follow these steps, if you know your employee ID skip to Step 3: Log on to ADP then Click on the Myself tab, go to Personal Information ; click on your Personal Profile and you will find your Employee ID in the top right corner listed as Position ID 3. Logon to KershHealth.com with the User Name and password you created. Step Two: Take your Qscore Assessment: 1. Once logged into your Kersh account, click Qscore. Complete your Qscore within 30 days of your benefits eligibility date to enroll. Step Three: Complete biometric testing: 1. Visit a LabCorp site near you: Obtain a LabCorp form and instructions from your HR department. -OR- Complete biometric testing with your Physician: Obtain a Physician s Letter and instructions from your HR department. 2. Complete your screening and submit your results to Kersh Health within 30 days from your benefits eligibility date to enroll. Step Four: Participate! 1. Participate in the program activities listed in the Incentive Plan to work towards a 2017 Reward.
