Foreword It will be difficult to find an ophthalmic surgeon who will gladly do his work entirely without viscoelastics. Within just a few years this group of substances has enlarged the field of ophthalmic surgery enormously. Many procedures have become safer and simpler and other techniques could only be developed because of the availability of visco elastics. Especially cataract surgery and implantation of intraocular lenses have benefitted. Implantation of an intraocular lens can be performed much more reliably into the capsular bag without endangering the posterior capsule. Implantation of foldable lenses would be almost impossible without visco elastics. However, other surgical maneuvers also necessitate viscoelastics, especially when the anterior chamber must be maintained, the corneal endothelium must be protected and delicate tissues must be manipulated. A corneal transplant can be sutured safely into the recipient corneal ring using viscoelastics. Even in glaucoma surgery, visco elastics gain importance as they are being used in trabeculectomy or in deep sclerectomy with additional viscocanalostomy. But which substance is best for which purpose? Just as much as a basic pharmacological knowledge is necessary for a rational use of drugs, in-depth knowledge of physicochemical properties and objective investigations are prerequisites for a logical selection of visco elastics from an ever increasing number of available substances. H. Burkhard Dick and Oliver Schwenn, both ophthalmologists at the department of ophthalmology at Mainz university, have embarked on the enterprise to systematically investigate visco elastics and to collect the necessary knowledge. They present the basic principals and properties of substances and an up to date selection of the techniques that can be performed using visco elastics. This book is going to be a standard text and the reference for any viscoelastic which is available at present or will be available in the future. The book contains a precise presentation of the basics as well as a survey of practical and clinical information. Without any doubt it will find many thankful readers. Prof. Dr. med. Preface Nowadays viscoelastics are applied in almost every cataract operation and in many other ophthalmic surgeries. It is hard to think of related fields and surgical steps whithout viscoelastic substances. Everyday we are confronted with a increasing number of new viscoelastic products and accordingly with new surgical techniques. Last but not least recent research results present a number of new facts which are not always easy to correlate to the actions in the clinical field. Qualified handling of viscoelastics however is impossible without the foundation of knowledge and without relation to practical matters clinical research will not be successful. The main purpose of this book was to give a critical overview about all aspects of the possible application of viscoelastics. It imparts updated basic knowledge as well as practical information and experiences concerning clinical matters. We tried to emphasize on combining updated basic knowledge and comparative studies with easy to understand information of practical relevance. Important fresh impetus was added to this project due to the interdisciplinary research cooperation with the Max-Planck-Institute for Polymeric research, department of rheology, in Mainz, Germany.
