Position Summary The Supportive Housing ACT Clinical Case Manager supports the ACT multidisciplinary team with clinical skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients with severe and persistent mental illnesses and substance abuse in supportive housing under the clinical supervision of the program manager. Functions as a clinical member of the multidisciplinary team and provides treatment, rehabilitation, and support services or may be designated by the program manager to function as the lead mental health professional. The ACT Clinical Case Manager will through the Housing 1 st Model assist clients in with overall improved well-being, stability and quality of life in conjunction with safe and stable housing. Essential Duties & Responsibilities Provide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client; educate and support clients, and advocate for the client s rights and preferences.

Establishing initial housing, rehousing, and housing stability. Provide ongoing assessment of client s mental illness symptoms and clients response to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior which put clients at risk (e.g. suicidality). Provide symptom education to enable clients to identify their mental illness symptoms. Teach behavioral symptom management techniques to alleviate and manage symptoms not reduced with medication and to promote personal growth and development by assisting clients to adapt and cope with internal and external stresses. Coordinate with outside inpatient services to detoxify clients and establish linkage to outpatient treatment, self-help programs (e.g. Alcoholics Anonymous, Narcotics Anonymous), outpatient services, and residential facilities. Plan and provide work-related supportive services, such as assistance with grooming and personal hygiene, securing of appropriate clothing, wake-up calls, and transportation. Provide on-going assessment, problem-solving, side-by-side services, skill training, supervision (e.g. promotes, assignments, monitoring, encouragement), and environmental adaptations to assist clients with activities of daily living. Assist and support clients to carry out personal hygiene and grooming tasks. Provide nutrition education and assistance with meal planning, grocery shopping, and food preparation. Assist and support clients to perform house hold activities, including house cleaning and laundry. Help clients to access reliable transportation. Teach money-management skills. Assist and support clients to have and effectively use a personal primary care physician, dentist, and other medical specialists as required. Plan, structure and prompt social and leisure-time activities on evenings, weekends, and holidays. Receive and assist client referrals through Coordinated Entry; responsible for documenting eligibility and maintaining full capacity of program. Provide regular and timely opportunities for face-to-face meetings to assure participant s progress toward self-sufficiency. Acts as a liaison and consult with community agencies and families to maintain coordination in the treatment process. Link participants with other agencies and organizations that assist with educational and career planning. Develop contacts with public and private sectors for education and training opportunities.

Examine and evaluate goals, objectives and operations of the Supportive Housing program (as appropriate) with the Program Manager. Work creatively and professionally with other team members and Program Manager to determine best integration of activities and resources for clients in the agency and community. Work with the Program Manager to develop and maintain necessary contracts for service. Engage in appropriate teamwork functions in order to provide a professional working environment and effective service delivery. Manage case data by documenting client contacts, goals and outcomes in the Service Point data base system. Prepare and maintain accurate records and reports for the agency and grantor. Participate in regular staffing meetings with other Caritas Housing Programs staff, monthly department meetings, and monthly all-staff meeting. Monitor expenses and provide fiscal management of client assistance as needed by accounting department. OTHER RESPONSIBILITIES: Complies with all applicable training requirements. Complies with all company safety, personnel and operational policies and procedures. Complies with work schedule to ensure effective operations of Agency programs. Contributes positively as a member of a productive and cooperative team. Performs other duties as necessary to fulfill the Caritas of Austin Mission. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES Must be sensitive to and respect cultural diversity amongst clients, staff and volunteers and able to work with diverse racial, ethnic and economic groups. Flexible work schedule including evenings, nights, weekends and holidays. Ability to set appropriate limits, work under deadlines and multi-task. Ability to organize, prioritize, self-motivate, and deliver results. Excellent communication and listening skills. Possess strong work ethics. Successfully pass Law Enforcement background screening. ADDITIONAL KNOWLEDGE, SKILLS, AND ABILITIES Basic knowledge of the resources in the community available for the clients with serious and persistent mental illness and homelessness.

Have basic knowledge of severe and persistent mental illness and substance abuse. Ability to form partnerships in the community and seek out community resources. Strong oral and written communications. Strong organizational, time management, and data management skills. Proven ability to work effectively both individually and as part of a team Ability to multi-task and problem solve under pressure. Ability to provide positive customer service to challenging populations. Qualifications EDUCATION & EXPERIENCE Master s Degree in Social Work or related field preferred LMSW or LPC-I preferred One year required, two years preferred, case management experience with at least one year working with dual diagnosis clients and/or the homeless population or experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illnesses. Must have strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services. Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illness and respect for client rights and personal preferences in treatment are essential. Bilingual: English and Spanish desirable. Dependable transportation required. COMPUTER SKILLS Experience working with various software programs: word processing, spread sheet and database.
