Checklist of Authorization for handling Bio Medical Waste Received application for BMW authorisation will be dispose of by the PCC, DD & DNH within a period of ninety days from the date of receipt of duly filled application along with following necessary documents under the Bio-Medical Waste Management Rules, 2016. 1. Form II 2. Copy of Valid membership of CBWTF for treatment of Bio Medical Waste. 3. Copy of Previously issued Bio medical Waste Authorization. 4. Bio medical waste categorization as per the schedule I of the Bio Medical Waste Management Rules, 2016 along with per day generation as prescribed in Form II.
