B Spectral Templates for Real - Time PCR Determination of Spectral Matrix For in vitro Diagnostic Use INTENDED USE The Focus Diagnostics Simplexa Spectral Calibration Panel is intended to estimate the spectral response of four dyes (FAM, JOE, CFR610 and Q670) across the four channels on the 3M Integrated Cycler and determine the Spectral Matrix. This product is not for diagnostic use in determination of any diseases or conditions. SUMMARY AND EXPLANATION The Simplexa Spectral Calibration Panel provides four unique all-in-one reaction mixes (Spectral Templates) containing doublestranded nucleic acid molecules, primers, and enzymes that are used to generate defined fluorescent signals for analysis by the Integrated Cycler Studio software. After the Spectral Calibration Panel is run on the 3M Integrated Cycler, Focus Diagnostics Technical Services staff will calculate and supply a new spectral matrix. This supports the continued use of Simplexa assays on the 3M Integrated Cycler. Instruments that detect multiple dye colors of closely spaced emission wavelengths need corrective methods to resolve each dye signal across all channels, so that the raw channel data can be converted into dye data. Most instruments use a combination of hardware (optical filters) and software (chemistry and calibration routines) to distinguish each dye signal. The Simplexa Spectral Calibration Panel provides a unique and pure fluorescent signal for each of the four dyes, through the all-in-one singletarget chemistry design of each Spectral Template. The Integrated Cycler Studio software measures the spectral properties of each dye and calculates values to distinguish the signal contribution of each dye from its principal and neighboring channels. PRINCIPLES OF THE PROCEDURE The Simplexa Spectral Calibration Panel (SC1100) is designed as a set of ready to use, thaw and load Spectral Template Reaction Mixes for the Universal Disc format to allow calculation of the spectral matrix for the 3M Integrated Cycler. The Simplexa Spectral Calibration Panel consists of single-use tubes containing premixed, ready to load reaction mixes (Spectral Templates) specific to each of the 4 dye channels: SC0520 FAM, SC0560 JOE, SC0610 CFR610, and SC0682 Q670 of the 3M Integrated Cycler. The Spectral Templates are not mixed with each other nor mixed with any additional patient or environmental samples in their designed use; instead they are produced in ready-to-use all-in-one design. MATERIALS PROVIDED The Focus Diagnostics Simplexa Spectral Calibration Panel contains sufficient material for two sets of reactions. Upon receipt, store all templates at -10 to -30 ºC (do not use a frost-free freezer). Templates are stable through the end of the expiration month indicated on the kit packaging when stored at -10 to -30 ºC. Integrated Cycler with Integrated Cycler Studio Software version 3.0 or higher. 2. Universal Discs for use on the Integrated Cycler. 3. Universal Disc Cover Tape. 4. Single, multi-channel and/or repeater micropipette(s), able to accurately dispense 10 µl. 5. Freezer (manual defrost) at -10 to -30 C (for kit component frozen storage). 6. Microcentrifuge. 7. Sterile, RNase/DNase-free disposable aerosol-barrier micropipettor tips. 8. Disposable, powder-free gloves. 9. Universal Disc Loading Block. SHELF LIFE AND HANDLING 1. Store templates at -10 to -30 C (do not use a frost-free freezer). 2. After removing Spectral Templates from freezer, initiate the test within 30 minutes. 3. Do not use templates beyond their expiration dates. 4. Do not vortex. 5. Do not combine templates from different lots. 6. Do not combine templates with each other or any external materials. 7. Do not expose templates to bright light for extended periods of time. WARNINGS AND PRECAUTIONS 1. For in vitro diagnostic use. 2. Follow standard precautions. 3. Wear personal protective equipment, such as (but not limited to) gloves and lab coats when handling templates. Wash hands thoroughly when finished performing the test. 4. Do not pipette by mouth. 5. Do not smoke, drink, eat, handle contact lenses or apply make-up in areas where templates are being used. 6. Dispose of unused templates according to local, state and federal regulations. 7. Contamination of templates can produce erroneous results. Use aseptic techniques. 8. Pipette and handle templates carefully to avoid mixing of samples from adjacent wells. 9. Use proper pipetting techniques and maintain the same pipetting pattern throughout the procedure to ensure optimal and reproducible values. 10. Do not substitute or mix template from different lots or from other manufacturers. 11. Only use the protocol described in this insert. Deviations from the protocol or the use of times or temperatures other than those specified may give erroneous results. 12. Assay setup may be performed at room temperature (approximate range 18 to 25 C); templates may be kept at room temperature or cold by utilizing a cooler block, and the Universal Disc may be kept on a pre-chilled Universal Disc Loading Block while loading templates. 13. Initiate the run within 30 minutes of removing the Spectral Templates from the freezer. 14. Do not re-use Universal Discs that have already been exposed to templates. 15. Dispose of used disc without detaching or removing cover tape. 16. Reaction Mix contains no more than 5% glycerol, which may cause irritation upon inhalation or skin contact. Upon inhalation or skin contact, first aid measures should be taken. 17. If kit packaging or contents appear to be broken or damaged do not use and contact Focus Diagnostics. Contact information can be found on the last page of this document. INSTRUCTIONS FOR USE A. REAL-TIME PCR INSTRUMENT SETUP Refer to Integrated Cycler Operator Manual for details on how to configure the Integrated Cycler Studio Software, to add an assay definition, set up and analyze, and export runs on the Integrated Cycler. B. REAL-TIME PCR AMPLIFICATION AREA A dedicated area for preparation of the 96-well Universal Disc for Simplexa Spectral Calibration Panel assay should be used. Use a chilled Universal Disc Loading Block while setting up the Simplexa Spectral Calibration Panel assay. Do not prepare tubes in template free reagent preparation area. 1. Thaw each of the Spectral Templates at room temperature (approximate range 18 to 25 C). Each kit component vial contains sufficient template for 8 reactions. Prior to each use, gently mix each individual component and briefly centrifuge to pull contents down to bottom of tube.

LIMITATIONS Spectral correction is instrument specific. This assay must be performed for each 3M Integrated Cycler every 6 months or as needed.
Page 4 PERFORMANCE CHARACTERISTICS CLINICAL STUDIES The Simplexa Spectral Calibration Panel does not have any direct clinical use. It will be used for Spectral calibration of the 3M Integrated Cycler. REPRODUCIBILITY Reproducibility for the Simplexa Spectral Calibration Panel was evaluated. Focus Diagnostics internal site assessed the device s inter-instrument, inter-operator and inter/intra-assay reproducibility. The assay was performed by three operators during 4 consecutive days. At least four of eight replicate wells for each Spectral Template were error free in order for the run to be valid. No operator difficulties or errors were reported or observed in setting up or running any of the assays. All evaluated Ct Value variance components met their established acceptance criteria indicating that the panel has low variance in both Ct values and Spectral Correction values as assessed with the device s inter-instrument, inter-operator and inter/intra-assay reproducibility. Results are presented in the tables below.
