A Healthier You Reporting Blue KC provides clients with participant information to facilitate communication campaigns and award employees incentives for completing program components. Personal Health Information (PHI) is Blue KC s highest priority and therefore the following reporting guidelines have been created on behalf of the A Healthier You (AHY) program. Reporting is available for the following areas: Health Risk Assessment Biometric Screening (onsite event attendance or submission of Manual Submission Form) Set Number of Points Earned in the A Healthier You portal Tobacco Cessation Initiatives A Healthier You Seminars A Healthier You Online Challenges A Healthier You Data and Reporting Steps: 1 2 3 Determine A Healthier You Program Incentive Requirements This includes the population to complete the requirements (i.e. subscribers, spouses and/or retirees) and deadlines. See the Incentive Checklist on the AHY Toolkit for details. Receive monthly preliminary reporting and use data to send reminders to employees who have not yet met all requirements. Receive final completion list to award the incentive to individuals who completed the requirements. 2017 Blue Cross and Blue Shield of Kansas City is an independent licensee of the Blue Cross and Blue Shield Association.

For example, Company A has a wellness program year of 1/1 12/31. Their incentive requirements are: 1. Completion of an onsite biometric screening by May 15 OR completion Manual Submission Form (MSF) by Oct. 15. 2. Completion of the Health Risk Assessment (HRA) by Oct. 31. The requirements are for subscribers only to earn a premium differential effective 1/1 of the following year. Below is a sample timeline for how they would receive reporting from A Healthier You. WELLNESS PROGRAM YEAR JAN. 1 OCT. 15 Members interact with portal, turn in MSFs if unable to attend an onsite screening. During this time, the AHY team will offer preliminary monthly reports of those who have not yet completed requirements. The portal can also issue reminder messages to members 30 and 60 days prior to the deadline. JAN. 1 MAY 15 OCT. 15 OCT. 31 Wellness program year starts, member portals reset (including access to HRA and MSF). Onsite Biometric Screenings held. Deadline for MSF submissions. Deadline for HRA. NOV. 15 Final report is delivered to company. DEC. 1 Company uses final report to send to their payroll department to enact the premium differential starting on 1/1. DEC. 31 Wellness program year ends and participants have final opportunity to enter sweepstakes with points accumulated throughout the year.

A Healthier You Aggregate Data What type of aggregate data can the A Healthier You program provide? A Healthier You will provide client-specific aggregate reporting tied to specific program components: Health Risk Assessment (50 individual participants required) Biometric Screening Results (50 individual participants required) AHY Portal Engagement Please note, this type of reporting is distinct from the participation lists issued on a monthly basis prior to the specified deadline. Aggregate reports are designed to identify client population risk factors. Health Risk Assessment (HRA)/Biometric Screening Completion Data What type of reporting is available if Blue KC client requires employees to complete the HRA and biometric screening/manual Submission Form (MSF)? What is the schedule to receive completion lists from A Healthier You? What member-identifying information is contained on the preliminary non-completion lists? Once requirements and deadlines have been confirmed A Healthier You will provide ongoing preliminary listing of those who have not completed all requirements. Click here to view a sample report. Please note, the report will not reflect which component was not completed; it will only state that an individual has not completed all requirements. Once incentive requirements and deadlines are confirmed preliminary participation lists will be provided on a monthly schedule. As the completion deadlines approach, participation lists can be provided on a weekly schedule. The AHY Reporting team will work with the client representative to confirm the frequency of participation lists. The preliminary monthly/weekly non-completion lists will include the following information for all employees: Defined Population (Subscriber/Spouse/Retirees) Date of Birth Member ID Social Security Numbers are considered PII and are not part of AHY standard participation update reporting. If SSNs are required for internal system needs, additional approval will be required and a distribution schedule will be implemented to ensure this information is protected.

What if a member has completed the components but is reflected on the non-completion list? How can a participant confirm if they have completed the HRA and/or biometric screening? Preliminary lists should be used to support communication campaigns encouraging employees to complete all the requirements by the specified deadlines. Members are encouraged to confirm their activity completion by visiting the A Healthier You portal and viewing the Understand My Health Care Plan. Use this How to Confirm Activities flyer to promote confirmation of activities. Members can log into the AHY portal to confirm completion by viewing the Understand My Health Care Plan on the portal home page. Use this How to Confirm Activities flyer to promote confirmation of activities. How can a participant confirm if they have completed the HRA and/or biometric screening? What if the client chooses to offer only an HRA campaign or only biometric screenings (or MSF submissions)? The final report of individuals who have completed all program requirements five to 10 business days following the confirmed deadlines. This final report should be used to award incentives. The list will including the following information: Defined Population (Subscriber/Spouse/Retirees) Date of Birth Member ID The AHY Reporting team will work with the client representative to confirm the delivery of the final completion list and requirements to be included. Please note, adjustments in confirmed incentive deadline schedules may contribute to an additional delay in compiling the final data. AHY will provide preliminary participation lists that indicate non-completion of that component with the inclusion of a final completion list to award incentives. All other rules specified above will apply. Completion Lists for Other AHY Program Components A Healthier You Portal Points Data Can Blue KC clients receive participant data reflecting individual points earned in the AHY portal? Since points earned information is considered PHI by Blue KC, AHY is unable to provide point values earned by individual employees of the group. By establishing a set number of points as part of the incentive requirements (i.e. 3000 points), Blue KC will provide preliminary updates to indicate who has not reached the set number of points. The AHY Reporting team will work with the client representative to confirm the frequency of participation lists.

A Healthier You Tobacco Cessation Data What data is available to support tobacco cessation initiatives through AHY? Can the AHY portal track tobacco attestations if our group requires them of employees? Participation in the A Healthier You Tobacco Cessation Digital Coaching and/or Tobacco Cessation Seminars can be reported based on client requirements.
