Request for Quotations/Proposal Supply, Delivery and Set-Up of Four Units of Digital Mobile X-Ray Machines under URC- USAID s Regional Health Integration to Enhance Services in East Central (RHITES-EC) Activity Project. University Research Co., LLC (URC), a US-based company working in Uganda since 2005, is implementing the USAID s Regional Health Integration to Enhance Services in East Central Uganda Activity (USAID s RHITES EC). The purpose of this project is to support the implementation of a comprehensive, integrated investment in regional health, HIV/AIDS and nutrition assistance. This fiveyear activity targets 11 districts in East-Central Uganda with an estimated total population of 4.1 million through a district-based integrated package of quality health, HIV/AIDS, nutrition and child development services. USAID s RHITES-EC provides technical support to enable implementation of comprehensive integrated health services and systems strengthening within 11 districts. The project s overall goal is to support the EC districts to increase access to and availability of high quality health care services. One of the obstacles to achieving improved TB Case Notification Rate (CNR) is limited clinical diagnosis of TB due to limited access to supportive screening and diagnostic services like X-ray services. URC is hereby requesting for quotations (RFQ) for Supply, Delivery and Set-Up of Four Units of Digital Mobile X-Ray Machine as per details and specifications indicated here-in. Important To Note: The four units are to be delivered at Iganga Hospital, Kamuli Hospital, Bugiri Hospital and Busia HC IV respectively. The vendor will incur the cost of delivery. These are digital mobile X-Ray units but the vendor must fully set up/assemble the unit at each of the facilities to be ready for use and deployment. The selected vendor will provide comprehensive training to technical staff and support services at the health facilities till familiarity with the system is attained. The vendor shall incur all costs related to offering this training. EVALUATION CRITERIA: Bids shall be evaluated based on the below evaluation criteria. Technical compliance Selection Method shall be used. 1. To establish vendor eligibility, you must submit a current (2018) trading license or an equivalent document to verify your eligibility. 2. Vendor must have experience in provision of supplies of similar nature and value (Attach proof of LPOs, Award letters or Contract Documents) 3. Vendor MUST indicate their delivery period. 4. USAID s RHITES-EC shall evaluate the technical aspects of the bid submitted to confirm that all requirements specified in the statement of requirements below have been met without any deviation or reservation.
