Application form AWS2 For a non-standard (32mm or bigger) connection to an existing water main This form allows you to apply for a non standard (i.e. 32mm or bigger) water service pipe connection. It is typically used to apply for a water connection to a commercial unit, industrial unit, a building split into a number of flats for which a standard 25mm connection is unsuitable. (If a 25mm connection is required please complete AWS1). The form serves a connection notice under Sections 45 and 55 of the Water Industry Act. Section 5: Infrastructure charges and credits Infrastructure charges are normally payable for each new water connection. Please complete this table to enable us to work out your infrastructure charge correctly. Table not required for individual metered apartments as standard infrastructure charge will apply. This needs to indicate your preferred point of connection. In addition, you may need to submit one or more of the following documents, depending on your circumstances. Please indicate which of the following you are sending to us: Letter of authority: If you are not the owner or occupier of the property that needs a new water connection, please supply an email, fax or letter from the owner, confirming that you are authorised to act on their behalf. Plumbing layout: If you are not using a WIAPS approved plumber you will need to provide a plumbing layout and a list of the water fittings that you plan to install. This must include the number, type and manufacture of all the fittings, such as taps, sanitary ware, pipework and accessories. Water re-use systems/sprinklers: If you are installing any type of rainwater harvesting, water re-use system or sprinkler system, please enclose full details of the equipment being used, including details of the manufacturer and installer of the system. Risk assessment for water pipes form: If there has ever or could be any possibility of soil contamination on the land being re-developed, you must enclose a Risk Assessment for Water Pipes form, available at unitedutilities.com F10 notice: If your development is intended for trade, business or community use, and the construction work is expected to last more than 30 days, you will need to provide a copy of your Health and Safety Executive F10 notice along with this form.
