IF A REPLACEMENT WELL, SMALL WATER TEST ATTACHED IF A NEW WELL, LARGE CHEMICAL TEST REQUIRED, THEN SMALL WATER TEST PRIOR TO TEMPORARY OR CERTIFICATE OF OCCUPANCY. COMPLETED WELL REGISTRATION FORM COMPLETED WHEN WELL IS FINISHED.
