Contractor/Subcontractor Certification "I certify under penalty of law that I understand the terms and conditions of the general Natio Pollutant Discharge Elimination System (NPDES) permit (ILR10) that authorizes the storm wa discharges associated with industrial activity from the construction site identified as part of certification." B. Attach a copy of the ILEPA Notice of Intent (NOI) The NOI requires compliance with Illinois law on: Endangered Species Historic Preservation Page 2 of 6

Inspect and Maintain Controls Inspections Every 7 days and within 24 hours of a 0.5" or greater rainfall event Inspection report - sign in accordance with the requirements of ILR10 Part VI.G Inspection certification statement "I certify under penalty of law that this document and all attachments were prepared under my direc supervision in accordance with a system designed to assure that qualified personnel properly gathe evaluated the information submitted. Based on my inquiry of the person or persons who manage the those persons directly responsible for gathering the information, the information submitted is, to the knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties f false information, including the possibility of fine and imprisonment for knowing violations." For violations of this plan - submit an Incidence of Noncompliance to the ILEPA & City of Rock Island Maintenance/repairs Record repairs/maintenance needed on inspection report. Repairs/maintenance must be performed no later than 7 days after the inspection C. Maintain Records of Construction Activities Dates when major activities occur Dates when construction activities cease temporarily Dates when construction activities cease permanently Dates when area is stabilized temporarily Dates when area is stabilized permanently D. Update/Change the Plan Update to reflect site features and operations Update when controls are not effective and are changed Update when site changes are required by permitting authority, or City of Rock Island E. Report Release of Reportable Quantities If a reportable quantity is released: Notify the Nation Response Center at 800-424-8802 Notify the appropriate local agencies Within 14 days submit to ILEPA and City of Rock Island a written description of the release providing date and circumstances and steps taken to prevent reoccurrence Modify Pollution Prevention Plan to include description, date, and steps to prevent reoccurrence F. Provide for Plan Location and Access Keep copy at construction site at all times Retain records for 3 years after completion of final stabilization Plans made available by request from ILEPA, City of Rock Island, and others 6. Final Stabilization/Termination Phase A. Implement Controls All soil disturbing activities completed & a uniform perennial vegetative cover with a density of 70% of the co unpaved areas has been established or an equivalent permanent stabilization has been constructed Notice of Termination- Submit per ILEPA requirements (copy to City of Rock Island) Page 3 of 6
