Prequalified subcontractors will be required to have one of the following California contractor's licenses: A License (General Engineering) OR C-8 License (Concrete) OR B License (General Building) GENERAL DESCRIPTION OF WORK: The Project will construct a six to seven-story, approximately 120,000 gsf building. The building will be LEED certified, targeting a Gold rating. The building will contain learning and support spaces including formal learning rooms, informal learning spaces, common areas, student amenities and administrative offices. Formal learning rooms shall include a 220 seat lecture hall, two 200-seat multipurpose rooms, case study rooms, teaching labs, problem based learning rooms and seminar rooms. The building shall also contain a public lobby and entrance, administrative and Deans offices and student oriented spaces. The scope of the work shall also include relocation of and connections to campus utilities, provision of building systems, provision of life safety systems, provision of a communications vaults and conduits, roadway reconfigurations and landscape and hard scape improvements. The total Project Construction Budget is $74,000,000 to $85,000,000. Attendees must allow ample time to drive to the above location in heavy traffic, find a parking space, walk to the building, and arrive in the designated Meeting Room prior to the required time. 5. Each prospective subcontractor must answer all of the questions and provide all requested information, where applicable. Any prospective subcontractor failing to do so may be deemed to be not responsive or not prequalified with respect to this application. 6. All information submitted for prequalification evaluation will be considered official information acquired in confidence, and the University will maintain its confidentiality to the extent permitted by law. 7. It is critical that the prospective subcontractor fills out all information required accurately, completely, truthfully and to the best of their knowledge. Ambiguous or incomplete information may lead to an unfavorable rating and subsequent status as not prequalified. 8. Those subcontractors who have submitted the prequalification application will be notified in writing whether they are prequalified and will be invited to participate in submitting a lump sum bid for Bid Package 15 Cast in Place Concrete & Reinforcing Steel. If a prospective subcontractor is determined by the University not to be prequalified, said subcontractor may request a review by the University. Any such request must be received by the University within 3 calendar days after receipt by the subcontractor of the determination. The decision resulting from such review is final and is not appealable within the University of California. Any person or entity not satisfied with the outcome of the prequalification must file a writ challenging the outcome within 10 calendar days from the date of the University's written notice regarding prequalification determination. Any assertion that the outcome of the prequalification process was improper will not be a ground for a bid protest. 9. PREQUALIFICATION CRITERIA Subcontractor shall meet the following minimum requirements to prequalify to bid on the UCLA Teaching and Learning Center for Health Sciences, Bid Package 15 Cast in Place Concrete and Reinforcing Steel: 1. Declaration: Properly signed by the authorized person. 2. Subcontractor has been in business for at least seven (7) years. 3. License: Subcontractor (or if Subcontractor is a joint venture, the joint venture entity and each member of the joint venture) is a licensed contractor in California with either an A (General Engineering), a C-8 (Concrete) or a B (General Building) classification. Additionally, Subcontractor (or if Subcontractor is a joint venture, the joint venture entity and each member of the joint venture) must never have had its license revoked by the State of California Contractors State License Board (CSLB). 4. Neither the Subcontractor nor its parent company (if any) has been disqualified, debarred, removed, terminated or otherwise prevented from bidding on, or completing any public agency project at any time within the last seven (7) years. 5. Neither the Subcontractor nor its parent company (if any) has been subject to a disciplinary proceeding by a public agency as the result of a crime for which he or she was convicted, or of a disciplinary infraction such as a breach of a code of ethics, at any time within the last seven (7) years. 6. Neither the Subcontractor nor its parent company (if any) has been found in a final decision of a court to have submitted a false claim to a public agency within the last seven (7) years. 7. Prevailing Wage and Apprenticeship: Neither the Subcontractor nor its parent company (if any) has been found to have violated any provision of California apprenticeship laws or regulations or the laws pertaining to use of apprentices on public works within the last seven (7) years. 8. Subcontractor s asset to liability ratio for the 2012 fiscal year is greater than or equal to one (1). 9 Subcontractor has revenue volume of at least two (2) times the bid package value (which is $12,000,000.00) over each of the past three (3) consecutive calendar years. A. If your firm performs multiple trades, state both the annual volume for the trade you are prequalifying for and the overall annual company volume on the Prequalification Questionnaire. 10. Subcontractor s Single Project Bonding capacity is greater than anticipated bid package value (which is $12,000,000.00). 11. Neither the Subcontractor nor its parent company (if any) has had a surety company perform any or all of their work on one or more of their construction projects within the last seven (7) years. 12. Subcontractor has maintained a Workers' Compensation Experience Modification Rate (EMR) that averages below (1.15) for each of the past five (5) years. 13. UCIP: Subcontractor has reviewed and can meet UCIP requirements included in Attachment 1. 14. Construction Experience: Subcontractor must demonstrate that it has the experience to satisfy both requirements A and B below: A. Subcontractor has fully completed four (4) building projects, each meeting all of the following criteria: 1. Fully completed by Subcontractor in the United States since January 1, 2004; and 2. Work included construction of new commercial, educational, institutional or hospitality facility(ies) utilizing a cast in place concrete frame construction (excluding parking structures); and 3. Said facility(ies) was/were at least three (3) stories in height (excluding parking); and 4. Subcontractor performed cast in place concrete and reinforcing steel work costing at least $8,000,000.00. B. Subcontractor has fully completed two (2) building or parking structure projects, each meeting the following criteria: 1. Fully completed by Subcontractor in the United States since January 1, 2004; and 2. Work included construction of new building or parking facility(ies) utilizing a cast in place post tensioned concrete construction; and 3. Said facility(ies) was/were at least three (3) stories in height; and 4. Subcontractor performed cast in place concrete and reinforcing steel work costing at least $4,000,000. IV. SUBCONTRACTOR SELECTION PROCEDURES 1. The successful subcontractor will be selected on the basis of the lowest responsive lump sum bid to the University. V. CONTRACT PROCEDURES 1. The successful bidder s subcontract will be with Rudolph & Sletten, Inc., the CM/Contractor of this project (hereinafter CM/Contractor ). 2. All insurance policies required to be obtained by the subcontractor shall be subject to approval by CM/Contractor of the project for conformance to the requirements of agreement between the University and CM/Contractor. All such policies shall be issued by a company rated by Best as A- or better with a financial classification of VIII or better or an equivalent rating by Moody s or Standard & Poor s. Policies issued by companies for Workers Compensation and Employer s Liability Insurance may be issued by companies (i) that have a Best rating of B+ or better, and a financial classification of VIII or better (or an equivalent rating by Standard & Poor or Moody s); or (ii) that are acceptable to the University VI.
