VERIZON WIRELESS PAGE 1 PART 1 - GENERAL 1.01 PROJECT DESCRIPTION VzW is planning on reclaiming significant space in the existing Vitesse Semiconductor manufacturing plant and to use it as a datacenter. The existing manufacturing facility has considerable mechanical, electrical and manufacturing processing systems and components which will need to be removed in order to accommodate future data/communication equipment and to house future mechanical and electrical equipment. The following summarizes project work scope: A. All work under this contract shall be coordinated with owner representative, MR. David Gulbransen with Sysak Hennessy Group. B. Removal of exiting mechanical equipment from facility. This equipment includes air handling units and the associated pumps and air ducts. Mechanical equipment also includes removal of boilers and various processing equipment components. Review mechanical plans for more details. C. Removal of miscellaneous electrical equipment. This would include all of the power distribution equipment associated with mechanical equipment mentioned above. Review electrical demolition plans for scope detail. D. Contractor shall provide VzW with list of ALL equipment removed and stored. The list shall be broken down per category and shall provide detailed description of major items removed, such as panelboard or transformer size, manufacturer name, etc. E. The contractor will be responsible for removal and returning of equipment to owner. F. The contractor shall also be responsible for providing storage plan to owner representative prior to commencing with demolition of any equipment. The plan should indicate how the contractor is planning on separating and storing removed equipment. The plan shall also show where the contractor is planning on storing equipment. G. Contractor shall store equipment per manufactures recommendation. H. VzW will determine at later date how they want to salvage removed equipment.

SUPERVISORY AUTHORITY: The Contractor is to avail themselves of all responsibilities under applicable regulations related to their supervisory authority over Subcontractors and personnel performing work related to hazardous materials. B. EMERGENCY CONTACTS: Designated qualified representatives of the Contractor and Subcontractors are to be available on a 24-hour emergency basis for the duration of the Work. Provide contact information to the Owner s Representative for inclusion in the Project emergency contact list.

VERIZON WIRELESS PAGE 4 C. GENERAL HAZARDOUS MATERIALS SUBMITTALS: Contractor is to review the scope of work and submittal requirements outlined elsewhere in the sections of Related Work. All contractors performing work impacting asbestos, lead, PCBs, etc. are to submit pertinent information required by associated technical specifications. Examples of work and impact may include abatement, demolition, saw cutting, roto-hammering, welding, sanding, drilling, scraping or other remodeling and metals-related impact, impact of asbestos-containing joint compound or other material with <1% asbestos, PCB ballast removal or light tube removal and disposal. 2.04 ASBESTOS A. Asbestos abatement is not included in this Project. B. The Contractor shall refer to the Phase 1 Environmental Site Assessment. The Contractor shall ensure that copies of this information are made available to and retained on the Project Site by all Subcontractors. 2.05 LEAD A. Lead-containing Items may be included in this Project. Requirements related to lead-containing items are outlined below. B. The Owner has conducted a survey of the areas of Project to be impacted by the Work for the presence of lead-containing painted coatings and lead-containing components. Findings and related analytical data are included in the Phase 1 Environmental Site Assessment. Consider all painted coatings similar to those testing positive for lead to be lead-containing. C. The Contractor shall comply with all applicable regulations, laws and ordinances concerning the impact, removal, handling, storage, disposal, monitoring and protection against exposure or environmental pollution related to lead. Impacts to lead that may be required by the Work include, but are not limited to: manual demolition, mechanical demolition, cutting or scraping. Confirm required impacts with drawing sheets. D. Work impacting lead-containing painted coatings and lead-containing items within this Project is the responsibility of the Contractor, and all affected Subcontractors, and shall be performed in accordance with all applicable local, state and federal regulations. 1. Contractor is responsible for the disposal of all dust, debris, disposable protective equipment, cleaning rags, wash water, and any other materials contaminated with residues from activities impacting lead-containing painted coatings or lead components, such as surface preparation, sanding, scraping, etc. 2.06 POLYCHLORINATED BIPHENYLS (PCBs) A. Light Ballasts

VERIZON WIRELESS PAGE 5 1. Ballasts inspected were found to be labeled No PCBs. Ballasts labeled as containing PCBs or not marked may exist within fixtures to be impacted by the Work. The Contractor is responsible for handling, removal, storage, and proper recycling of light ballast according to all applicable regulations if unlabeled ballast is found at the Project Site. Prevent damage to any unlabeled ballasts and immediately report any leaking ballasts to the Owner s Representative. 2. Contractor shall furnish all labor, materials, equipment, services shown, or reasonably implied for the removal and handling of fluorescent light ballasts to be demolished as part of the Work. 2.07 MERCURY (Hg) A. Fluorescent Light Tubes 1. The Contractor is responsible for proper handling, removal, storage, proper packaging, and transport for proper recycling. 2. Following removal from fixtures, carefully place lamps in fully covered containers and transport to designated recycling facility. PART 3 - SAFETY REQUIREMENTS 3.01 SUMMARY A. The purpose of this section is to establish a summary of the minimum requirements for safety on the construction jobsite, the Contractor s role regarding creating and maintaining a safe jobsite and the working requirements of others. B. The Contractor is responsible for compliance with all applicable laws and regulations regarding safety whether noted in this section or not. Furthermore, the Contractor is responsible for creating and maintaining a safe working environment on the construction site whether the requirements of this section address a particular situation or not. 3.02 RESPONSIBILITY A. The Contractor shall be solely and completely responsible for conditions of the work site, including safety of all persons and property, during performance of the Work. The Contractor shall maintain the work site and perform the Work in a manner which meets or exceeds statutory and regulatory requirements for the provision of a safe place to work and which minimizes safety risks to personnel of the Contractor, Subcontractors, Owner, general public or other parties. This obligation shall apply continuously and not be limited to normal working hours.

The Contractor shall ensure that all Contractor and subcontractor personnel are provided sufficient training, and shall take such actions as are necessary to maintain a safe environment on the jobsite. Such training and actions shall include but not be limited to ensuring that such employees are familiar with governing construction safety requirements and the requirements for compliance with applicable regulations. The Contractor shall monitor the jobsite to ensure that employees do not create unsafe conditions for others, and shall notify employees of the requirement to alert the Contractor to any potential health or safety hazard affecting themselves or others, and to comply with the provisions of the Site Specific Safety Plan. 2. The Contractor shall establish and communicate clear expectations to its employees and its subcontractors (and their employees) of their obligation to notify any party concerning an observed unsafe or unhealthy act, or any pending hazard. B. The Contractor shall designate an on-site competent person to be the Safety and Health Officer who is qualified and authorized to supervise and enforce compliance with the Contractor s Site Specific Safety Plan during the performance of the Work. The Contractor is responsible to ensure that all necessary monitoring equipment, protective clothing, and other supplies and equipment are available to implement the Plan. 3.03 SUBMITTALS A. Company Safety Program: The Contractor shall submit a copy of its Company Safety Program to the Owner. The Company Safety Program shall contain, at a minimum, the following elements: 1. Organizational Structure: Include names of individuals who will perform safety duties, titles, work assignments, authority and reporting relationships. 2. Training Program. Who, how and when training is provided; method of employee training concerning safety rules and procedures; training in use of protective equipment. 3. Protective Equipment: List of personal protective equipment to be provided to employees. 4. Accident Prevention and Loss Control Plan. Work site inspection and hazard correction procedures; disciplinary procedures for safety infractions; accident response, investigation and reporting procedures. 5. Regular Safety Meetings. On-site weekly, or other frequency as appropriate, safety meetings mandatory for all employees. B. Site Specific Safety Plan: The Contractor and each of their Subcontractors shall review the Contract Documents, and the Contractor shall develop and submit a copy of a Site Specific Safety Plan to the Owner. The Site Specific Safety Plan shall be tailored to the unique issues of the Project and the specific types of hazards likely to be encountered, be in compliance with all regulatory requirements, and contain, at a minimum, the following elements. 1. Application of Company Safety Program. The Site Specific Safety Plan shall address how the following elements from the Company Safety Program will be specifically applied and modified in addressing unique issues related to the Project: organizational structure, training program, protective equipment, accident prevention and loss control plan.

VERIZON WIRELESS PAGE 7 2. Specific Hazards. The Site Specific Safety Plan shall address, as applicable, the following, and other specific hazards for the Project: a. Overhead Hazards/Flying Objects b. Hazardous Materials and Chemical Exposure (asbestos, lead, PCB, mercury, mold, etc.) c. Electrical Safety d. Scaffolding and Personnel Lifts e. Noise and Dust f. Lockout/Tagout and Control of Hazardous Energy g. Tripping Hazard 3.04 REQUIREMENTS A. Safety Training: Contractor shall provide work site orientation for all employees (including subcontractor employees) to become familiar with the Site Specific Safety Plan. B. Respiratory Equipment: Any personnel requiring use of respiratory protective equipment shall be fully trained in the use of such equipment. Contractor must have a respiratory protection program and ensure that all workers wearing respirators have medical clearance and fit testing, as appropriate, for the type of respirators used. C. Personal Protective Equipment: Contractor shall supply all identified Personal Protective Equipment in accordance with Labor and Industries standards. As a minimum requirement, all personnel working on the project shall be required to use approved hardhats, safety glasses, and appropriate work footwear. D. First Aid: The Contractor shall maintain at the work site office, or other well known place at the work site, all materials necessary for giving first aid to the injured (e.g., a first aid kit), and shall establish, publish, and make known to all employees procedures for ensuring immediate removal to a hospital or a doctor's care, persons, including personnel, who may have been injured on the site. E. Reporting Injuries and Incidents: Contractor shall immediately notify the Owner s Representative of any injury or incident to personnel on the site. Contractor shall conduct an immediate investigation with an emphasis on preventative actions and lessons learned. The Contractor shall submit a copy of the OSHA 301 form documenting the investigation of the incident to Owner within 24 hours of incident. Contractor shall report on a monthly basis the total number of hours worked on-site by Contractor and its employees and Subcontractors, and the total number of recordable incidents and lost time accidents. F. Reporting Potentially Serious Hazards: Contractor shall immediately notify the Owner s Representative of any potentially serious hazard to personnel on the site. Contractor shall conduct an immediate investigation and submit a report to the Owner s Representative within 24 hours of becoming aware of the potentially serious hazard. The report shall describe the potentially serious hazard, the results of the Contractor s investigation, and any steps the Contractor has taken to prevent an injury or incident from occurring based on the potentially serious hazard.

VERIZON WIRELESS PAGE 8 PART 4 - TEMPORARY FACILITIES AND CONTROLS 4.01 DESCRIPTION OF REQUIREMENTS A. Minimum temporary facilities and controls requirements are specified in this section. These requirements have been included for special purposes as indicated. Nothing in this section is intended to limit types and amounts of necessary temporary Work, and no omission from this section will be recognized as an indication that such temporary Work is not required for successful completion of the Project and compliance with requirements of all applicable codes. B. Included in this Section are the following items: 1. Material Storage and Handling 2. Work Area 3. Protection of existing utilities 4. Shutdowns of existing equipment and utilities 4.02 MATERIAL STORAGE AND HANDLING A. Study site and present owner with plan which indicates location of material storage. Do not proceed with demolition until owner approves plan. B. Store, and handle material in accordance with the manufacturer s recommendations, using means and methods that will prevent damage, deterioration, loss or theft. C. Store material at the site in a manner that will facilitate inspection and measurement of quantity or counting of units. D. Protect building materials subject to damage, under cover in a clean and weather tight enclosure, with ventilation adequate to prevent condensation. Maintain temperature and humidity within range required by manufacturer s instructions. 4.03 WORK AREA A. Confine operations and storage to the designated work area. Maintain the work area and building in a clean and in an orderly manner. B. Security: 1. Protect Work and stored products from theft and vandalism; and protect premises from entry by unauthorized persons. At the end of work day, close temporary enclosures and lock exterior doors and/or gate. Secure all openings at any time site is left unoccupied. 2. Owner s Representative will issue keys, as required, for the Contractor to perform the Work. Prior to Final Completion, return all issued keys.

VERIZON WIRELESS PAGE 9 C. Waste Management: 1. Dispose of all refuse and waste material off Owner's property. Do not stockpile waste material on Owner's property. Immediately clean up any spilled material. Clean all trash and debris from work area daily. Keep work area, site, and adjacent properties free from accumulations of waste materials, rubbish and debris resulting from construction operations. Provide on-site containers for collection of waste materials, debris and rubbish. Periodically remove waste from the site, at a frequency acceptable to Owner. Do not use Owner's waste containers for construction waste. Dispose of all flammable, hazardous, and toxic waste materials daily. Storage of these materials will not be permitted on the interior of building. Coordinate dumpster location with Owner s Representative. 4.04 PROTECTION OF EXISTING UTILITIES A. The existing concealed utilities shown on the drawings are not necessarily exact with respect to location or completeness; therefore, Contractor shall take the following steps: 1. In the event utilities are damaged during construction, temporary services and/or repairs must be made immediately to maintain continuity of services. Damaged utilities, installed by the Contractor, shall be repaired at the Contractor's sole expense. 4.05 SHUTDOWNS OF EXISTING EQUIPMENT AND UTILITY SERVICES A. It is generally critical that all building systems remain operational within occupied buildings, except for brief shutdowns that might be required to integrate or connect new work. B. Equipment or utility shutdowns required to facilitate the Work shall be accomplished in accordance with the following requirements: 1. Submit a schedule of equipment and utility shutdowns. 2. Confirm all requests for equipment and utility shutdowns in writing to the Owner not less than (3) days prior to the proposed date. 3. The duration of all shutdowns shall be held to a reasonable minimum as determined by Owner. PART 5 - CONSTRUCTION WASTE MANAGEMENT 5.01 SUMMARY A. Construction, Demolition, and Land Clearing (CDL) Waste: Includes all non-hazardous solid wastes resulting from construction, remodeling, alterations, repair, demolition and land clearing and material that is recycled, reused, salvaged or disposed as garbage. B. Salvage: Recovery of materials for reuse. C. Reuse: Making use of a material without altering its form. Materials can be reused on-site or reused on other projects off-site. D. Recycling: The process of sorting, cleaning, treating, and reconstituting materials for the purpose of using the material in the manufacture of a new product. E. Source-Separated CDL Recycling: The process of separating recyclable materials in separate containers as they are generated on the job-site. The separated materials are hauled directly to a recycling facility or transfer station. F. Approved Recycling Facility: Any of the following: 1. A facility that can legally accept CDL waste materials for the purpose of processing the materials into an altered form for the manufacture of a new product. 2. Material Recovery Facility: A general term used to describe a waste-sorting facility. Mechanical, hand-separation, or a combination of both procedures, is used to recover recyclable materials. 5.03 PERFORMANCE REQUIREMENTS A. General: Divert CDL waste, by weight, from the landfill by one, or a combination of the following activities: 1. Salvage 2. Reuse 3. Source-Separated CDL Recycling B. CDL Waste materials that can be salvaged, reused or recycled include, but are not limited to, the following. Refer to drawings for equipment to be reused and stored on site: 1. Acoustical ceiling tiles 2. Drywall 3. Equipment 4. Fluorescent lights and ballasts 5. Metals 6. Paint (through hazardous waste outlets) 7. Field office waste, including office paper, aluminum cans, glass, plastic, and office cardboard.

VERIZON WIRELESS PAGE 11 5.04 SUBMITTALS A. Coordinate requirements of this section with owner s representative, Mr. David Gulbransen. Proposed methods for salvage, reuse, recycling and disposal during construction including, but not limited to, one or more of the following: 1. Requiring Subcontractors to take their CDL waste to a recycling facility. 2. Contracting with a recycling hauler to haul recyclable CDL waste to an approved recycling or material recovery facility. 3. Processing and reusing materials on-site. 4. Self-hauling to a recycling or material recovery facility. 5. Name of recycling or material recovery facility receiving the CDL wastes. 6. Handling and Transportation Procedures: Include method that will be used for separating recyclable waste including sizes of containers, container labeling, and designated location on Project Site where materials separation will be located. 7. A record of the type and quantity, by weight, of each material salvaged, reused, recycled or disposed. 8. Disposal Receipts: Copy of receipts issued by a disposal facility for CDL waste that is disposed in a landfill. 9. Recycling Receipts: Copy of receipts issued by approved recycling facility. 10. Salvaged Materials Documentation: Types and quantities of materials salvaged for sale or donated to a third party. A percentage of proceeds, as agreed upon in advance, will be credited to the Owner.
