Cracking is heaviest in Span 2 and near piers where spacing is approximately 6", spacing wider near abutments to 12". Extensive transverse cracking exists over piers (Photo 7). 105 / 2 Reinforced Concrete Closed Web/Box 336 ft 316 9 11 Girder Two girder lines, G1 and G5. - The soffit and webs of girders exhibit 1/64" wide cracks. Multiple longitudinal cracks spaced at 5'-6' exist in Span 3. - Vertical cracks up to 1/64" exist over the piers and throughout Spans 1 and 2, heavier in Span 1, spacing is greater than 1'. - Shear cracks up to 1/64" wide evident in Spans 1 and 2, spacing is greater than 1' (Photo 8). - Girder 4 exhibits map cracking and efflorescence in soffit at Abutment 2 between Columns 6-7, 8sq ft. - Several spalls exist at the top of Girder 2 at the east face of closure pour with no exposed rebar. - The following are larger high profile vehicle impacts in the lower west edge of the Span 1 girder: - A spall over Lane 3, 4'-8" L x 1'-4" H x 3" D, with a 4' long section of exposed and corroded rebar (Photo 9). - Two spalls over Lane 2: 1'-7" L x 6" W x 2" D, with a 16" long section of exposed rebar with slight corrosion and a 2'-2" L x 6" W x 2" D spall, with no exposed steel over Lanes 2 and 3. - A spall over Lane 1, 4'-8" L x 6" W x 2" D with exposed rebar with light surface corrosion. - Four small spalls up to 6" diameter x 1" D, with no exposed rebar. - Girder 5, Span 2 exhibits a 6" W x 6" L x 1" D spall on western face with no exposed rebar. - Span 2 exhibits several high profile vehicle impacts in Girders 1 and 5. 109 / 2 Prestressed Concrete Girder/Beam 504 ft 504 Three girder lines, G2-G4. - East edge of Girder 2 exhibits a 1" x 2" rectangular cavity which appears to be construction related. - All cell vents in Girder 2 of Span 2 are clogged with styrofoam. 205 / 2 Reinforced Concrete Column/Pile Extension 18 each 17 1 - Piers 1 and 2, Columns 1 to 3 and 6 to 9 have been retrofitted with steel jackets. - The upper north quadrant of Column 5 of Pier 1 exhibits a 8" L x 3" H x 1/2" D rock pocket. - Pier 2 columns exhibit numerous impact scrapes. 215 / 2 Reinforced Concrete Abutment 356 ft 337 18 1 Quantity includes flare. - The backwalls exhibit vertical shrinkage cracks to 1/32" wide. - The west end of the Abutment 1 backwall exhibits a 2' H x 1' W x 2" D incipient spall, with associated cracking to 1/8" wide. - The east end of the Abutment 1 backwall exhibits a 2.8' H x 1' W x 2" D spall, with exposed and corroded rebar (Photo 10). - Both east and west ends of Abutment 2 backwalls exhibit cracking and minor delamination.

Northbound: - The Abutment 1 concrete header exhibits cracks up to 1-1/2" wide, spalling and asphalt patches in the western three travel lanes (Photo 11). - Asphalt patch exhibits a small pothole between Lanes 1 and 2. - Header appears to have been replaced in Lane 4 and exit lane. Transverse and longitudinal 1/4" wide cracks exist in header/slab in Lane 4 and exit lane. - The Abutment 2 header is cracked, spalled and delaminated up to 6' L x full header width, in the northbound travel Lanes 3 and 4. Spalls are patched with asphalt but asphalt is depressed up to 2" (Photos 12-13). Southbound: - The Abutment 2 header in travel lanes appears to have been removed and replaced with concrete approach pavement, cracking indicates it bears on header below (Photo 14). 310 / 2 Elastomeric Bearing 44 each 44 Element not visible for inspection, except for leveling pads at Abutment 1, Girder 1. - No significant deficiencies. 321 / 2 Reinforced Concrete Approach Slab 2 each 0 0 0 2 - The approach slabs exhibit cracks in the southbound travel lanes up to 1/4" wide and in the northbound travel lanes up to 1/16" wide. 1/4" wide cracks are spaced at 6" in the southwest approach slab (Photos 15 and 16). 330 / 2 Metal Bridge Railing 168 ft 168 Metal tube rail mounted to top of west concrete rail. - Metal tube rail exhibits minor dents and scrapes throughout. 331 / 2 Reinforced Concrete Bridge Railing 504 ft 482 22 - The parapets exhibit map and vertical shrinkage cracks to 1/64" wide, minor impact scrapes and tire marks. - Median rail exhibits shrinkage cracking up to approximately 1/32" wide with spacing approximately 6' to 10'. 515 / 2 Steel Protective Coating 385 ft 385 - No significant deficiencies. 516 / 2 Concrete Coating 1 lump sum - No significant deficiencies. 1 640 / 2 Deck Drainage System with Drop Inlets 2 each 0 0 0 2 - The deck drains of Span 3 are completely clogged with dirt and debris (Photo 17). 646 / 2 Under Deck Lighting System 6 each 0 0 6 - All under deck lights are inoperable (lighting not on during 2012 and 2014 inspection to verify operational status). - There is an unused box with exposed wiring adjacent to the southeast under deck light, exposed wiring is secure

Relief joints have been sealed in many different areas which are failing and debonding throughout (Photo 20). 752 / 2 Metal Approach sign for Vertical Clearance 2 each 2 Minimum vertical clearance signs attached to the east and west fascia (Photos 5 and 6). - Metal clearance signs are posted for 14'-10", minimum vertical clearance measured 15'-2" and 15'-1" for westbound and eastbound respectively. 760 / 2 Metal Approach Barrier Rail 2 each 1 1 - The northwest approach rail exhibits minor impact damage for 6' approximately 100' from structure. 761 / 2 Reinforced Concrete Approach Barrier Rail 6 each 0 5 1 - Approach rails exhibit vertical cracks to 1/16" wide. - The southeast approach rail exhibits vertical cracking to 1/8" wide with an incipient spall on the inboard face to 4' L at the base (Photo 19). 770 / 2 Cement Concrete Approach Pavement 1 lump 0 0 0 1 sum Concrete approach pavement panels in southeast, northwest, and southwest beyond approach slabs (Photo 17). - The concrete approach pavement in the southbound travel lanes exhibit transverse and diagonal cracks up to 1/4" wide; some of the larger cracks have been previously sealed at southwest (Photo 20). Panels appear to have shifted with gaps and settlement up to 1" wide. A 1' L x 1.5' W x 2-1/2" D mechanical spall exists in Lane 2 due to shifting panels.

This may lead to debris accumulations, heaving, and/or settled pavement.
