ADDENDUM NO. 5 ITB-CD-CME-16-10 Southern Well Field Expansion: Equip 5 Wells and New Raw Water Main Phase 2 ISSUE DATE: August 22, 2016 DUE DATE: August 25, 2016 @ 2:00 p.m. SUBJECT: Addendum No. 5 INTENT: This addendum is issued prior to the date bids are due in order to incorporate the following clarifications, additions, omissions, deletions, or changes into the Contract Documents. Except as hereinafter specified, the work shall be in accordance with the drawings and specifications. Bidders are to use the changed quantities of the items listed in this addendum in their proposals, and it shall become a part of the Contract Documents when construction is executed. Bidders are reminded that this addendum must be noted on the Official City Acknowledgment Form when they submit their proposal. A signed copy of the addendum must also be included with the bid submittal. 1. STATEMENTS A. All of the telemetry equipment for this project is hereby removed. This includes the following material: i. Remote Telemetry Unit ii. Radio iii. Antenna Tower Any and all buried conduit from the RTU cabinet to the antenna tower location shall be installed, brought above grade and capped. The Sensus ACT-PAK transmitter shall also be installed. B. Per the EPA, portable/trailer-mounted generators must meet the EPA Tier 4 emission requirements, which will take effect in 2017. In the interim, there are manufacturers that have Tier 4 final certified engines, or Tier 4 Interim Compliant Engines or Tier 3 Flex Credit Engines. The engine for the portable generator must be one of the previously mentioned, and must also have a 48-hour sub-base fuel tank and an aluminum enclosure. Anything else is not acceptable. Location of the control panel and electrical connections is not an issue. Section 16215 has been revised to reflect the above. 2. PROJECT MANUAL A. Section 16215 i. The following paragraphs have been revised: 1.01-A; 1.02-B; 1.02-D; 1.02-F; 1.04-A. Delete this Section in its entirety and substitute a new 16215-R section, which is attached and made part of this Addendum. 3. QUESTIONS & RESPONSES A. Question: Bid form page 00200-12 requires listing the manufacturer used for PVC pipe in section 15063. After reading 15063 we found that there are four different types of PVC listed with different manufacturers named (DR18, SDR21, SCH 40 & SCH 80). Which PVC pipe type is listed on the bid form? B. Question: Are there geotechnical reports available? They are listed in the table of contents but we are unable to find them. Same question for Permits. Response: The geotechnical report and FDEP permit were included within Addendum #3. C. Question: Can you furnish contact information for KW Controls, Inc. Response: Since all of the telemetry has been removed, other than the Sensus flow transmitter and buried conduit, this question does not require an answer. D. Question: Air release valve located at STA 228+60 is does not appear to be at a high point in the line segment. Can this air release be relocated further downstream (Approx. STA 224+40), or should another air release be installed? Response: Bid the project with the air release valve as shown. Any change in location will be made with the Contractor awarded the project. E. Question: Should there be an Air release valve located between STA 235+60, and STA 233+40? Response: Possibly; however, bid the project as designed. F. Question: Is there a local water source for Hydrostatic testing, pigging, and flushing of the pipeline? If there is, where is it located? Response: This question was addressed in Addendum #3. G. Question: A 12 valve is shown at STA 232+45 on piping profile, but not on the Plan view. Is this valve to be included? Response: Yes. The 12 valve will be placed to the east of the 12 6 tee. H. Question: Can well reports, including inner and outer casing material (i.e. DR) be provided? Response: Well completion reports were provided in Addendum #3. The 12 Certa-Lok SDR 17 casing ID is 11.04. I. Question: Please confirm Non-permanent O-Rings are to be fitted in the Certa-lok Pump column couplers. Response: Yes. Non-permanent O-rings will be used in the column couplers. J. Question: Please confirm method of connection for the plain ends of the 8 /6 stainless pipe at the well cap, and the 8 /6 Certa-lok male x PE adapter. Response: The callouts on the drawing are correct. K. Question: The capacities, and discharge size of Grudfos pumps noted for use in the well table on Drawing C-25 appear to be slightly lower than the capacities specified in section 11211. Will the pumps in the well table on Drawing C-25 be acceptable? Response: Yes. L. Question: Components of the well cap that penetrate the base plate (8 SS pipe, 2 SS vent, and 1 ½ SS cable entry) will not fit in the well as shown on Drawing C-25. Please review and confirm wellhead configuration. Response: As part of this addendum, the column pipe for the submersible pumps has been changed to 6 ; this should now provide adequate space to install the miscellaneous small pipe. M. Question: Bid Form Page 00200-8 says Indemnification is $250.00 or 1% of the bid whichever is greater. Bid Item 28 says MAX 1% of bid. Will Bid Item 28 need to be 1% of the sum of Bid Items 1 thru 27? Same question for the Additive Alternate. Response: Both Bid Item Nos. 28 and 44 will use a maximum 1% of the Bid. N. Question: Spec 15062-3 calls for ceramic epoxy lined fittings in all applications except for potable water or reuse water. As the wells are raw water, are ceramic epoxy lined fittings required on this job or are cement mortar lined fittings acceptable? Response: Cement mortar-lined fittings are acceptable. O. Question: Sheets E4 thru E6 show directional drills for the FPL service to these well sites. Are these drills paid for under Bid Item #24A? If not, which Bid Item? Response: Yes. Bid Item #24a has been revised in Addendum #3 to include the drills for the FPL service. P. Question: We have not received the soils report yet & understand it s to come with the next addendum. Spec 02230-3.01 calls for 3 of material to be removed & replaced with compacted backfill while the clearing is being done. Since the clearing is basically done at most of the well sites, does this dirt need to be removed & replaced? Or just to the depth where organics are encountered? Or just follow the soils report recommendations? Response: The dirt at the well sites does not require removal and replacement. Removal of undesirable material and replacement with select fill will occur between STA 234+00 and STA 236+00. Q. Question: Sheet C-24, Well LW-108, has See Note 1 & See Note 4 on the Layout Plan but there are no notes. Which notes are we to reference? Response: A revised Sheet C-24 has been included with this Addendum that lists the pertinent notes for this well site. R. Question: Sheets C-20 thru C-23, Note 1 calls for a herbicide treatment per spec 02281 prior to installing the weed fabric. Is this treatment required under the well pump slabs & driveways per the spec? Response: Yes. S. Question: Please clarify depth of limerock base for asphalt open cut areas? Response: The limerock shall be 8 minimum thickness or twice the thickness of the existing base, whichever is greater. T. Question: Please clarify well site clearing. Is the contractor expected to clean all vegetation within clear limits and only clear 6 trees outside of this area? Response: Any well sites where it states to extend the cleared area, all material will be removed except trees 6 in diameter or greater. U. Question: The Plans do not have referenced notes on LW 108 site plan. Response: A revised Sheet C-24 has been included with this Addendum that lists the pertinent notes for this well site. V. Question: Will the load bank test be Resistive only or Resistive & Reactive? Response: Resistive only.
