Product Safety Group 1201 South Second Street Milwaukee, WI USA 53204 productsafety@ra.rockwell.com PRODUCT SAFETY ADVISORY Notice of Potential Unsafe Condition Rockwell Automation issues a Product Safety Advisory when it identifies a product(s) anomaly that may potentially cause harm to personnel, capital equipment loss or environmental damage. Immediate action is required as identified below. Select 140M-C*, 140M-D*, 140M-RC*, and 140U-D* Devices May Unexpectedly or Intermittently Conduct Electricity in the OFF or TRIPPED State Reference: 2014-10-002 Date: October 2014 This Product Safety Advisory informs you of a potential anomaly that exists with select Motor Protection Circuit Breakers and Motor Circuit Protectors 140M-C*, 140M-D* and 140M-RC* and select Molded Case Circuit Breakers 140U-D6* manufactured in August 2014 and September 2014. In a small number of instances, when the breaker is in the OFF or TRIPPED state, the identified breakers may unexpectedly or intermittently conduct electricity thru a single phase. This can result in voltage being present on the load side of the breaker with the breaker in the OFF or TRIPPED state. 140M-C 140M-D 140M-RC 140U-D Product Identification The affected products are Motor Protection Circuit Breakers and Motor Circuit Protectors 140M-C*, 140M-D* and 140M-RC* and Molded Case Circuit Breakers 140U-D6 manufactured in August 2014 and September 2014.

PRODUCT SAFETY ADVISORY Page 2 of 6 Potentially affected product will have a date code of IB (August 2014) or JB (September 2014). Refer to Appendix B for a complete listing of potentially affected product by catalog number. The product catalog number and date code may be found on the product carton label or nameplate. For installed product, the product may need to be removed to gain access to the nameplate. Be sure to follow local and industry standard safe electrical practices when performing maintenance activities on this or any other electrical equipment. For new and unopened product, the product catalog number and date code may be found on the carton label. The catalog number is shown on the lower left side just above the bar code label. The date code is shown on the lower right corner and will be one of IB (August 2014) or JB (September 2014). In the example shown to the left, the catalog number is 140M-D8E-C10 and the date code is IB (August 2014). This product falls within the potentially affected date code range and matches the catalog number list in Appendix B, and would be in the potentially affected product population. For used and installed product, the catalog number and date code may be found on the product nameplate. The product nameplate is located on the left side of the product. The catalog number is located at the top left of the nameplate. The date code is shown on the lower right corner and will be one of IB (August 2014) or JB (September 2014). In the example shown to the left, the catalog number is 140M-D8E-C10 and the date code is IB (August 2014). This product falls within the potentially affected date code range and matches the catalog number list in Appendix B, and would be in the potentially affected product population.

PRODUCT SAFETY ADVISORY Page 3 of 6 Description A potential anomaly exists with select Motor Protection Circuit Breakers and Motor Circuit Protectors 140M-C*, 140M- D* and 140M-RC* and select Molded Case Circuit Breakers 140U-D6* manufactured in August 2014 and September 2014. In a small number of instances, when the breaker is in the OFF or TRIPPED state, the identified breakers may unexpectedly or intermittently conduct electricity thru a single phase. This can result in voltage being present on the load side of the breaker with the breaker in the OFF or TRIPPED state. Important - Shock, vibration, or other environmental conditions may cause an affected breaker to unexpectedly conduct electricity after the breaker has been placed in the OFF or TRIPPED state, even after voltage has been verified to be absent on the load side. Temporary Workarounds No workaround has been identified. Customers are advised to replace product within the potentially affected date code range. Correction ATTENTION: Only personnel qualified in the maintenance of electrical products should attempt circuit breaker identification and replacement. Follow industry standard Lock-Out-Tag-Out (LOTO) procedures and wear appropriate PPE when working on this equipment. You must ensure both the line and load sides of the suspect breaker are de-energized prior to performing maintenance or remediation activities. Product identification may require the removal of the product to gain access to the product nameplate. Refer to your product installation guide for instructions on product removal and installation. If you have product within the potentially affected date code range, contact your local Rockwell Automation Distributor to make arrangements for product replacement. Reference this Product Safety Advisory PSA 2014-10-002 when making your request. Rockwell Automation asks that you immediately perform remediation actions. If you believe you may have affected product, you must ensure both the line and load sides of the affected product are de-energized prior to performing remediation or any other maintenance activities on this equipment. Requested Customer Action Rockwell Automation requests you take the following actions: Check if you have a product affected by this Product Safety Advisory. Refer to the Product Identification and Description sections of this document for product identification assistance. Product may be returned to your local Rockwell Automation distributor using standard quality return procedures. All returns should be over packed to prevent shipping damage during transit. If applicable, contact your distributor or local sales office to return the product and request a replacement unit. Reference this Product Safety Advisory 2014-10-002 when making your replacement request.

If your facility participates in the DMI program, you are still responsible for all actions listed in these instructions. If you have questions about product return process, please contact your DMI representative. Contact Technical Support for additional technical information regarding this PSA/PN at (440) 646-3434, however, no ticket # is required to return material affected by this notice. Quality Return - PSA/PN/Program Return Process New/Unopened Box Material: Customers are directed to contact their distributor or local sales office for return/credit of all products affected by this PSA/PN. Customers may be requested to submit a new Purchase Order to obtain replacement product. No ticket # from Technical Support is required to return material affected by this notice. Distributors and local sales offices should follow the Quality Return process (see screen shots below) for Return/credit of all products affected by this PSA/PN. All distributors or local sales offices currently using SAP for order entry must use the SAP Return Order system for return authorization and credit memo issuance. Distributors and local sales offices should follow the standard process to initiate a Remanufacturing or Exchange transaction for all used products affected by this PSA/PN. Repair/Exchange Serviceable Material: All distributors or local sales offices should follow the instructions presentation to initiate a Remanufacturing or Exchange transaction: This presentation is available as an attachment to Rockwell Automation Knowledgebase Article ID 49915 at https://rockwellautomation.custhelp.com/app/answers/detail/a_id/49915. Consumable (No repair or exchange available) Material: Consumable products must be under warranty and powered greater than 24 hours to be classified as a consumable return. A Knowledgebase account is required to access this presentation. Consumable products are not eligible for Remanufacturing or Exchange under the SAP process. Customers are directed to contact their distributor or local sales office for return/credit of all products affected by this PSA/PN.
