Section G Beef Quality Assurance
The purpose of Section G is to estimate number of operations that are aware of the Beef Quality Assurance (BQA) program and that follow BQA recommendations Page 28
Maine-Anjou
Does the operator follow BQA recommendations for these practices? Page 28 Are they BQA certified? If the operator does not know the answers to question 4 select no

Section I Conclusion
Page 4 Notice how & 2 differ All of the operations in NAHMS had beef cow(s) in January 207. Any record that reports zero beef cows (4), out of business (5), or out of scope (7) will need detailed comments (Page 5) but DO NOT INCLUDE ANY PII on the NAHMS questionnaire.
Reasons for consent refusals (2) or Phase refusal () on the previous question. Check only one and enter in the code box. Page 4
Page 6 DO NOT WRITE IN THE OPERATOR S NAME, PHONE NUMBER, OR ANY PII 0 7 0 0 Please remember to complete the information in the Red box 4xxxx
Very Important Point Remember your job as the enumerator is to ask the questions as they appear on the questionnaire and not to provide veterinary advise Do not get lost in the weeds
Questions Regarding Phase
Phase 2 Phase 2 will take place in late-january 208 through mid-april of 208 and will involve a face-to face interview by a Veterinary Medical Officer and optional participation in biological sample collection Any results from the biological samples can be provided to the operator Those operators that complete Phase 2 will be eligible to have Bovine Viral Diarrhea (BVD) testing conducted on their entire spring calf crop and to have a forage sample tested for nutrient content at no charge Additional information regarding phase 2 can be found on pages 7 and 72 in the Interviews Manual
Who Qualifies for Phase 2 Completed NAHMS Beef Management Survey Phase They had or more beef cows at any point during January, 207 through December, 207 If both qualifications are met ask the operator if they are willing to sign a consent form A signed consent form will allow USDA-NASS to release their contact information to USDA-APHIS so they can participate in Phase 2 A producer will be eligible for Phase 2 if a consent form is signed Phase 2 is not mandatory, consent does not obligate producer
Consent Form Two consent forms will be included with each assignment kit The signed copy will be sent to the office The other will remain with the operator for their reference
Pre-printed label Consent Form Do not make any adjustments to the label on the consent form
Completed by the operator Completed by the enumerator
