This Report is being submitted pursuant to requirements of the San Francisco Bay Regional Water Quality Control Board Letter, New Requirements for Reporting Sanitary Sewer Overflows, dated November 14, 2004, as well as the California State Water Resources Control Board ( State Board ), Statewide General Waste Discharge Requirements for Sanitary Sewer Systems (Order No DWQ) issued May 2, 2006 and subsequently modified as to monitoring and reporting requirements by State Board Order No EXEC (the 2008 Modified State Order ), and Order no. WQ EXEC, Amending Monitoring and Reporting Program for Statewide General Waste Discharge Requirements for Sanitary Sewer Systems (effective September 9, 2013). In addition, this report is required by Section IX Findings of Violation and Amended Order for Compliance (Docket No CWA 309(a) ) dated November 24, 2008 (the 2008 Amended Order ) issued to the City, Sausalito Marin City Sanitary District ( SMCSD ) and Tamalpais Community Services District ( TCSD ). For purposes of this Report, the City refers to sewage spills and sanitary sewer overflows from the City sewer mains as "SSOs" and those from private facilities as private lateral sewage discharges "PLSDs". No (0) SSOs and two (2) PLSDs occurred during the reporting period. Location, Cause and Corrective Action for PLSD s July 1, 2018 through September 30, /19/2018 PLSD Cat 2 Map S7 at 1:06 pm the City responded to request to investigate an overflow from a cleanout located at the southwesterly corner of 655 Sausalito Boulevard fronting Sausalito Boulevard. This sewer lateral has overflowed on 4 separate occasions over the last eleven years and the City's regulation of the matter was escalated to nuisance abatement with this most recent PLSD. The property owner has met with City Staff to plan a sewer lateral replacement that does not require access to neighboring property (the existing connection to the public main travels through what was until recently a private common lateral on an adjacent neighbor's property the neighbor recently detached from that common lateral and established an independent connection to the City main). Since being contacted by the City and Marin County Environmental Health Department following the most recent PLSD, the property owner has cleared the sewer lateral and is cooperating in retaining appropriate design professionals for replacement of the lateral. Roots at each 2 foot terra cotta tile joint were documented via inspection by closed circuit televising equipment as the reason for the PLSD. Marking and locating has been completed by the same televising contractor. This PLSD is identified by key number 1 in the attached Tables, and on the attached Figure /28/2018 PLSD Gate 5 Road PLSD Cat 3 reportedly observed by property owner at approximately 9 AM and reported to the City at 10 AM. The sewer lateral that solely serves Unit 55 was observed to have overflowed intermittently from the property access cleanout (ACOP). Building occupants were directed to eliminate wastewater production and the property owner's contractor's attempts to clear the line were unsuccessful on September 28, The property owner's contractor returned on September 29, 2018 at 8 AM and, under City inspection, cleared the lateral using high velocity jetting up the lateral from the City owned sewer manhole on Gate 5 Road by 9 AM. The blockage was caused by what appeared to be rock utility trench backfill. After the line was cleared the sewer lateral was televised. A break in the 10 o'clock position in the lateral serving 55 Gate 5 Road was observed and may have been the result of damage sustained when work was performed by others. The City contacted the Marin Municipal Water District and advised the property owner to expose the location of the break and either repair it or arrange to have it repaired by the responsible party. The sewer overflowed from multi unit building ACOP to the site unpaved and paved surfaces. This PLSD is identified by key number 2 in the attached Tables, and on the attached Figure 2. Other Information Since , the City of Sausalito has had a program requiring the inspection and repair of private sewer laterals upon sale or major remodel of habitable structures. In Sausalito, laterals are privately owned from the structure to the final termination point (including the wye) at the City s main line. Actions that trigger the mandatory inspection are either home (or building) sale or major remodel work (including, but not limited to, an improvement value of $50,000 or more). As the need for repairs is identified on the basis of those inspections, performance of maintenance or repair is required by the City as a 1 Copies of the Sausalito Municipal Code are publically accessible at as is a copy of the City s promulgated administrative interpretation of the relevant code section at

The City s criteria for requiring such maintenance or repair is expressed in the Ordinance and Administrative Interpretation (ibid.) to eliminate infiltration of pollutants into the groundwater, or surface water including Richardson s Bay. This has been a very successful program. The City continues to develop improvements to the existing private lateral program to expedite identification and repair of private facilities to prevent discharge of pollutants to waters of the State. Further, when a PLSD occurs, the City is assertive about requiring inspections and repairs to prevent repeat PLSDs associated with private facilities. Comprehensive cleaning and inspection work was also completed during the Reporting Period in several locations in the City of Sausalito. Those include: 523 feet of private sewer laterals were replaced. 75,235 feet of public sanitary sewer collection system cleaning was performed by mobile combination sewer cleaning vehicle (jetting with vacuum) and by machine rodding. Certification I certify under penalty of law that this document and all attachments are prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gathered and evaluated the information submitted.
