1 INVITATION FOR BID RE-ROOFING BUILDING at: Abingdon Child Development Center Early Head Start Roof Replacement 152 Highland Street, Abingdon, Virginia DESCRIPTION OF ITEMS: Notice is hereby given that People Incorporated of Virginia will accept sealed bids for roof replacement at the Early Head Start Center located at 152 Highland Street, Abingdon, Virginia The work will include the removal and disposal of the existing membrane roof and the installation of a new roof to consist of one layer of ½-inch insulation board and.045-mil re-enforced EPDM membrane. All penetrations must be flashed and sealed. Metal flashing must be installed around outer perimeter of the building. Footage is approximately 2300 sq. feet. TERMS AND CONDITIONS: 1. Project Scope: Re-roofing; location is the modular Early Head Start building at 152 Highland Street, Abingdon, Virginia 24210, as further described in Project Specifications section below. 2. Notice to Contractors: Prior to submitting a bid each contractor is expected to: Personally and physically inspect the premises of the proposed work in order to arrive at a clear understanding of the conditions under which all work is to be accomplished. No extra consideration will subsequently be allowed by reason of error or oversight on the part of the contractor upon submittal of bid. If there is other work in progress the contractor shall coordinate his work with all existing adjacent work. Each trade shall afford all other trades every reasonable opportunity for the installation of the work, protection of completed approved work, and for secure storage of materials and equipment. The contractor shall be held responsible for completion of entire work in manner intent for this type of project. 3. Types of Bids Required: A. Bidders may submit both a lump-sum price and unit costs for all items shown on the Bid Proposal Form. B. Bids shall be provided for the basic project C. Conditional bids or voluntary alternates shall not be considered. 4. Time of Completion: The project is to be completed by June 26, Insurance: Contractor shall submit with bid a copy of Worker s Compensation coverage; General Liability coverage and Automobile Liability Coverage. 2 6. Time and Place of Submission of Bids: A. Time: Bid proposals shall be submitted on or before June 5, 2015, at 3:00 p.m. B. Place: All bids shall be submitted at the following location: People Incorporated of Virginia, 1173 West Main Street, Abingdon, Virginia C. Bids will be opened on June 5, 2015, at the hour of 3:05 p.m. D. All information requested of the bidder shall be entered in the appropriate space(s) on form(s) provided herein. Failure to do so may disqualify your offer. E. Submit bid only in a sealed envelope: Project Title (ACDC EHS Roof Replacement) Bidder s name Contractor s license number F. Bids must be submitted by the closing date prior to the time specified to be considered. No , facsimile or telephone bids will be accepted. G. Upon submission of bid documents, all such documents shall become the property of People Incorporated of Virginia. 7. Quantity and Quality of Materials or Services: All materials and supplies furnished in the bid shall be in accordance with the specifications detailed in the Project Specifications, including related building plans, architectural drawings and specifications detailed in those plans and drawings. Materials or supplies, which in the opinion of People Incorporated of Virginia are not in conformity with such specifications, shall be rejected and promptly removed from the premises at the bidder s expense. 8. State Prevailing Wages and Non-Discrimination: This project is subject to the State Prevailing Wage Rates specified in the Contract Document. The Contractor will be obligated to pay no less than the minimum wage rates required by the State of Virginia and to keep records and to make reports as specified. The Contractor also will be obligated not to discriminate in employment practices and to keep records and to make reports as specified. People Incorporated of Virginia will monitor compliance with these requirements. All bidders must comply with Davis-Bacon Act requirements and must state they will abide by these regulations in their Bid Proposal. 9. Award: A combination of the following factors will be considered in awarding this bid. These factors are not necessarily listed in order of priority. A. Prices. B. Quality and conformance to specifications. C. Experience. D. Similar type of projects performed. People Incorporated of Virginia shall be the sole judge in making this determination. 10. Questions concerning specifications or description of work involved should be directed to Byron Farmer, at Questions concerning administrative matters should be directed to Donna Edwards at or Donna Buckland at Withdrawing Bids: No bidder may withdraw his bid for a period of (90) days after the date and time set for the opening thereof; provided, however, that People Incorporated of Virginia may release the bid of unsuccessful bidders after confirming the technical and formal sufficiency of the bid and bid security of the three (3) lowest bidders. In no event shall the bid for security of the three (3) lowest bidders be released before execution with People Incorporated of Virginia of an Agreement with the successful bidder, or the expiration of ninety (90) days, whichever occurs first. 13. Licensing/Qualifications: The Contractor shall possess either a Class A or a Class B license at the time this Contract is awarded. All work shall be performed by a licensed, bonded and insured contractor and his crew. 14. Rejection of Bids: People Incorporated of Virginia reserves the right to reject any and all bids and to waive informalities or irregularities in the best interest of the agency. 15. 4 PROJECT SPECIFICATIONS RE-ROOFING BUILDING at: Abingdon Child Development Center Early Head Start Roof Replacement 152 Highland Street, Abingdon, Virginia Scope of Work: Re-Roof the Early Head Start modular building at 152 Highland Street. Footage is approximately 2300 square feet. This is a prevailing wage job and is subject to the State Prevailing Wage Rates specified in the draft contract attached to this invitation for bid. 2. Qualifications: The Contractor shall possess either a Class A or Class B license at the time this Contract is awarded. All work shall be performed by a licensed, bonded, and insured contractor and crew. 3. Materials, Labor and Equipment: All labor and materials shall be included in the bid proposal. Re-Roofing Project: Furnish all labor and materials necessary for the completion of a watertight roof: 1. Unhook and remove all mechanical equipment prior to roofing (i.e. a/c units, gas and electrical lines, ductwork, condensate lines, fire extinguishers, coping) and store onsite in a secure location. 2. Remove the existing membrane roof and dispose of debris. 3. Inspect wood for dry rot. Notify People Incorporated of Virginia should any exist. Replace at additional prevailing wage cost per man-hour plus materials. 4. Install new roof to consist of one layer of ½-inch insulation board and.045-mil re-enforced EPDM membrane. 5. All penetrations must be flashed and sealed. 6. Metal flashing must be installed around outer perimeter of the building. 7. Check all drains for proper functioning. 8. Make certain that roof is sloped properly to each drain, so ponding water does not occur. 9. Clean up all debris caused by the above work on a daily basis. 10. Guarantee all workmanship for (5) five years. 11. Provide a 10-year NDL warranty from manufacturer. 12. Note that this is a prevailing wage job. Page 4 of 5

8 Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)) The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate). Union Rate Identifiers A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than "SU" or "UAVG" denotes that the union classification and rate were prevailing for that classification in the survey. Example: PLUM /01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers indicates the local union number or district council number where applicable, i.e., Plumbers Local The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate. Survey Rate Identifiers Classifications listed under the "SU" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA /13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier. Survey wage rates are not updated and remain in effect until a new survey is conducted.

9 Union Average Rate Identifiers Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH /29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier. A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/cba rate of the union locals from which the rate is based WAGE DETERMINATION APPEALS PROCESS 1.) Has there been an initial decision in the matter? This can be: * an existing published wage determination * a survey underlying a wage determination * a Wage and Hour Division letter setting forth a position on a wage determination matter * a conformance (additional classification and rate) ruling On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.) and 3.) should be followed. With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations.
