1 JOB SAFETY ANALYSIS GUIDE Version Date Comments 1 September, 2016 Initial 2 June, 2017 Update procedure A. INTRODUCTION Job Safety Analysis (JSA) is a procedure that focuses on individual job tasks to identify hazards and prevent injury or exposure by employing appropriate engineering, administrative, physical controls, as well as personal protective equipment (PPE). The JSA process focuses on the relationship between the worker, the task, the tools, and the work environment. JSA document steps that must be taken to eliminate or reduce workplace hazards to an acceptable risk level. This Guide is based upon Occupational Safety and Health Administration (OSHA) publication 3071 Job Hazard Analysis and 29 CFR 1910 Subpart I, Personal Protective Equipment. B. SCOPE University supervisors in consultation with EHS are responsible for determining a list of tasks performed by their employees. The JSA process requires employee involvement, review of any hazards associated with work tasks, and identification of controls that can mitigate or reduce hazards. The Environmental Health and Safety Office (EHS) will provide assistance developing the JSAs and supervisors are responsible for training, implementation and revision of the JSA Form. Work tasks which have JSA priority are those that: Have procedures that have the potential to be immediately dangerous to life and health; That can or have resulted in employee injury or exposure; Required lockout/tagout, confined space entry, fall protection, or personal protective equipment (PPE) to perform the task safely; Require the operation of a vehicle (other than an automobile), tool, or equipment capable of causing injury or exposure to an employee or bystanders; and Involve two or more employees, special procedures, or unique process in order to complete the task. C. RESPONSIBILITES EHS is responsible for: Providing training to supervisors on how to complete the Job Safety Analysis Form. Maintaining the JSAs Tracking Log. Assigning JSA numbers and approving submitted JSAs. Responding to questions in the field relating to JSAs. Environmental Health & Safety Office 1

2 Notifying supervisors when JSAs have been approved. Following-up with supervisors to ensure all necessary PPE, and, or necessary equipment such as LOTO devices identified in the JSAs are available for employee safety. Posting completed JSAS onto MyMasonJSA web portal. Supervisors are responsible for: Identifying hazardous tasks performed under their work area/ supervision. Completing Job Safety Analysis Form for tasks deemed hazardous in their work area/activities. Reviewing Job Safety Analysis Form periodically with employees. Maintaining Job Safety Analysis Form and making them available to employees. Submitting completed Job Safety Analysis Forms to EHS Utilizing JSAS for tool-box safety talk topics, new employee, or hands-on training. Documenting training, each employee should review all JSA relating to their job tasks. Employees are responsible for: Reviewing Job Safety Analysis Form prior beginning a job task. Observing the safety procedures and PPE requirements documented in JSAs. D. JSA Procedure 1. Obtain Job Safety Analysis Form (Appendix A). The Job Safety Analysis Form is available online at or from EHS upon request. 2. Select the work to perform the JSAs based upon priority and actual or potential hazards. 3. Observe the work process to identify work procedures and safety hazards. 4. Fill out all sections of the Job Safety Analysis Form (see section F. JSAs Form Instructions) 5. Send completed Job Safety Analysis Form to EHS at safety@gmu.edu. 6. Approve any EHS changes to JSAs Forms prior to making final. 7. File Job Safety Analysis Form in a conspicuous location for employees to access and reference as necessary. E. Sequence of Job Steps Potential Hazard Hazard Type PPE & Control Measures List all applicable personal protection equipment necessary to perform the task. Check yes, or no, if LOTO procedures are required, if you are uncertain Check yes, or no, if a permit is required for entry, if you are uncertain Check yes, or no, if fall protection is required, if you are uncertain Always start and finish with a safe set up, and include all tools and PPE required for the job in the first step. Usually a task can be completed in about 7-10 steps. Don t forget to include housekeeping as the last step Identify any hazards in this column such as slippery surface, air contaminants, strains and sprains, etc. Select from the Hazard Type box above that relates to that step. List all control measure and PPE needed to perform the step safely. F. REVIEW AND MODIFICATIONS EHS Safety Officers will review each submitted Job Safety Analysis Form for accuracy and thoroughness. Once the JSAS is reviewed by EHS, EHS will assign a JSA ID number and make available on MyMason JSA web portal. Incomplete JSAs will be returned to the supervisor with questions or comments. In some cases, EHS will review the task being performed to approve the JSAs. Completed JSAs require review periodically, or if there are changes to the workplace. If the Job Safety Analysis Form is updated, or new tasks are documented using the JSAs process, updated Job Safety Analysis Forms must be submitted to EHS at safety@gmu.edu. G. RECORD KEEPING Supervisors are responsible for maintaining Job Safety Analysis Forms in an accessible location for employees to reference, and training, as needed. When feasible, JSAs will be posted near the hazards in the workplace. All approved JSAs can be found online at MyMason.gmu.edu. To access JSAs: 1. Log into MyMason.gmu.edu using your university NetID and password 2. Select the Organization tab at the top right 3. Select Job Safety Analysis 4. Choose the JSA from the work tasks H. EHS INSPECTION EHS will coordinate a review of Job Safety Analysis Forms on a routine basis with supervisors. Job Safety Analysis Forms will be updated as necessary whenever an incident or injury occurs. PPE and equipment may also be reviewed as a part of the inspections process to ensure employees have the necessary tools identified to perform work safely. Environmental Health & Safety Office 3

5 Forms must be submitted to for review. EHS will post approved JSA on the MyMason JSA web portal. JSAs must be reviewed periodically by supervisors and forwarded to EHS for approval All employees must be trained and have access to JSAs that are applicable to their work tasks. Any exposure to hazardous materials or injuries must be reported to Human Resources and Payroll Worker s Compensation using an Employer s First Report of Accident Form. All accidents, incidents, property damages, or emergencies that do not result in employee injuries or exposures must be reported to EHS using the university Incident Report Form.
