4 ew ested Off s in ocity When requesting Time Off in Paylocity, you have the following options: PTO JURY DUTY BANKED FLEX BEREAVEMENT CUSTOMER SERVICE DAY FLOATING HOLIDAY

5 Time Off ocity ode: Y All full- time employees are eligible to take PTO for vacation, illness, medical appointments and personal time off. Time off will be accrued as the follows: Length of Service Less than 1 year of service After 1 year of service After 2 years of service After PTO 3 accrues years of after service 30 days of employment 12 hours PTO can only be taken in 1 hour increments Accrual Rate per Month 10 hours hours hours All employees must submit Time off Requests in Paylocity. Once you submit a Time Off Request, your Manager will be responsible for approving it. Once the Time Off Request has been approved: Your PTO balance will automatically deduct the hours The PTO will automatically be added to your timesheet

6 Y DUTY Employees who are summoned to jury duty must notify their Manager and the HR Manager immediately. A copy of the jury summons must be faxed or ed to the Dearborn, Michigan office. Full- time employees selected for Jury Duty will be paid up to a maximum of 8 working days per calendar year, except as may otherwise be required for exempt employees under applicable law. If Jury Duty exceeds 8 days, please contact Brian Gilbertson in HR. If excused early from Jury Duty, the employee is expected to report back to work on the next scheduled work day. If you are scheduled to go to Jury Duty, please select Jury Duty from the Time Off Types. This will let your Manager know that you are scheduled for Jury Duty. Once your Manager approves, Jury Duty hours will automatically be added to your timesheet.

7 KED FLEX Exempt employees with five or more years of service may be allowed to bank up to 5 flex days per calendar year. An employee with less than 5 years may not bank any days unless last minute scheduling conflicts with a client or project necessitate working that flex day. Your banked day must be approved before working it. Employees work a regular 8 hour day on those Flextour days designated to be banked. All banked flex days are added to an employee s PTO allotment, to be used in accordance with PTO policy. If you are requesting a Banked Flex, please select Banked Flex from the Time Off Types. This will let your Manager know that you have requested a Banked Flex. Once your Manager approves, Banked Flex hours will automatically be added to your timesheet.

8 TOMER VICE DAY Every employee is given two paid Customer Service Days (CSD) per calendar year. These days can be taken any time an employee determines that he or she is not able to live the EDSI values of Show- up, Smile and Support for the day. This could be for sickness or anything that would hinder someone from coming to work at his or her best. These days are not to be taken in increments like PTO, but rather they must be taken as full days. If you would like to use the Customer Service Day(s) on a 9 hour day, please switch your day to an 8 hour day (similar to a Flex Switch). For example, if you take a Customer Service Day on a 9 hour day, please make up that additional hour on another day in that same week. Your total hours for 2 weeks should equal 86

9 AVEMENT Employees who wish to take time off due to the death of an immediate family member should notify their Manager and the HR Manager. Pending approval, employees may have up to three (3) days paid absence. Immediate family member is defined as an employee s spouse, child, mother, father, sister, brother, mother- in- law, father- in- law, brother- in- law, sister- in- law, grandparents, legal guardian, or any dependent as described by the IRS for tax purposes. One (1) paid day of bereavement is available for the passing of a non- immediate family member. Documentation of the service or funeral must be faxed or ed to Robin Redmer in Dearborn. If you are scheduled to take Bereavement, please select Bereavement from the Time Off Types. This will let your Manager know that you are scheduled for a Bereavement Day. Once your Manager approves, Bereavement hours will automatically be added to your timesheet.

12 ew Time Types in ocity sheet On the timesheet, you have the following options to choose for pay type that require approval and documentation: Short- term/long- term Disability Military Leave FMLA Personal Leave Medical Leave Workers Compensation

13 RT- TERM/ G- TERM BILITY EDSI currently provides short and long term disability insurance for its full- time employees. Details of the plan are available upon request. PTO does not accrue when on disability, and EDSI reserves the right to change or eliminate these plans at any time.

14 TARY VE Employees will be granted a leave of absence without pay for military service or reserve duty and training as required by state and federal law. EDSI will comply with all applicable state and federal laws and regulations regarding re- employment and continuation of other benefits. If an employee requires a military leave of absence, he/ she should notify the HR Manager in order to arrange a leave of absence.

15 A In accordance with the Family and Medical Leave Act (FMLA), eligible employees (as defined under the FMLA) may be granted up to 12 weeks of unpaid FMLA leave in a 12- month rolling period for the following reasons: For the birth of the employee s child, and to care for the newborn child For the placement with the employee of a child for adoption or foster care To care for the employee s spouse, child or parent with a serious health condition Because of a serious health condition that makes the employee unable to perform the functions of the employee s job Documents to complete: Provide documentation (Dr. Note) FMLA form Note: FMLA forms are available on the Self- Service Portal in Paylocity under the Company section

16 SONAL VE The Company may grant leaves of absence without pay to employees who must take time off from work duties for personal reasons. Leaves of absence will be granted at the sole discretion of management. A leave of absence may only be taken after all available PTO has been taken. As soon as an employee becomes aware of the need for a personal leave of absence, they should contact his/her Manager and HR Manager. Requests for personal leave will be evaluated based on a number of factors, including anticipated workload requirements and staffing considerations during the proposed period of absence.

17 ICAL VE In situations where an employee is not eligible for FMLA leave (as opposed to situations where an eligible employee exhausts FMLA leave), unpaid medical leaves of absence up to a maximum of 4 weeks may be requested by an employee for absences due to an employee s own illness or disability that extends beyond 5 consecutive days of absence (including a condition relating to pregnancy). All medical leave requests must be accompanied by appropriate medical certification from your physician indicating the condition necessitating your leave request and your projected date of return to work.

18 KERS ENSATION The Company provides worker s compensation insurance for all of its employees in the event of a job- related injury. All injuries and accidents, no matter how minor, should be reported immediately in writing to the employee s Manager and HR Manager.
