1 1.0 QUALIFICATION BY TESTING AND ANALYSIS This EQPR is intended to be used when buying valve positioners, regulators, actuators and spare parts manufactured by GE Masoneilan formerly known as Dresser Flow Control-Masoneilan (hereinafter GE Dresser Masoneilan) for use throughout Ontario Power Generation (OPG) sites in nuclear safety-related applications which require environmental qualification (EQ). Note: Dresser Masoneilan is currently part of GE Energy. OPG clause EQ DRESSFLOB is the applicable EQPR clause for the Safety Relief Valves and Safety Valves manufactured by former Dresser-Consolidated Operations currently GE Consolidated. OPG Engineer (see the Contract Standard for definition of Engineer ) generally assesses and qualifies electrical equipment for use in applications which require Environmental Qualification (EQ) by conducting qualification tests (or referring to qualification tests performed by others) whereas mechanical items are generally qualified by analysis. Once it has been established that an item is environmental qualified, it is necessary to ensure that successive purchases of the item shall be equivalent to the qualified item. In order to do so, it is necessary to clearly establish the critical characteristics of the qualified item so that these can be replicated and verified. These critical characteristics may be shown on one or more of a number of different documents including test reports, qualification reports, the manufacturer s catalogue, manufacturer s drawing(s), bill(s) of material, specification(s) etc. So as not to limit the generality of the foregoing list, the document that describes the critical characteristics that need to be replicated shall be referred to herein as the primary controlled document. For EQ materials it is essential that the primary controlled document clearly identifies the materials of construction as well as other critical characteristics of the item. EPRI NP-5652 and TR may be used as a guideline for a more detailed explanation of critical characteristics. The primary controlled document shall be selected by OPG Engineer and stipulated in OPG purchase order (P.O.) item description. In every instance, the content of the primary controlled document shall be under rigorous revision control. In instances when the primary controlled document defines most but not all critical characteristics, and in instances when the primary controlled document provides configuration or material selection options, the required characteristic or material option(s) shall be stipulated in the OPG purchase order (P.O.) item description. GE Dresser Masoneilan item shown in the OPG P.O. item description has been assessed and environmentally qualified by the OPG Engineer for use in nuclear safety-related applications based on the following primary controlled document(s): Valve Positioner(s) certified to - Dresser Report # TR-012, R01, Environmental Qualification of Dresser 2 Nuclear Control Valve (Item 105) for Wolsong 2, 3, 4. Actuators and spare parts as described in the GE Dresser Masoneilan certified drawings which have been accepted by OPG and are referenced in the P.O. item description. Pressure regulators and spare parts certified to: Dresser Test Report TR-045, R01, Environmental and Seismic Qualification Tests of Dresser Size 160 Actuator and Accessories for Qinshan NGS Unit 1 & 2 or Dresser Test Report TR-039, R00, Environmental and Seismic Qualification of Dresser Item 110, 3 Control Valve SCN543L0771, and Masoneilan North American Operations Dresser Valve & Controls Division Report No , R03, Generic Nuclear Safety-Related Qualification for Model 8005N Electro-pneumatic transducer and 77N-4 Airset per IEEE , IEEE , and IEEE This clause applies to OPG Purchase Orders placed on or after March 20, 2018 Page 1 of 6

2 2.0 CHANGES IN DESIGN AND SUBSTITUTION OF QUALIFIED MATERIALS Changes made to EQ items manufactured and supplied by GE Dresser Masoneilan that were originally tested or previously assessed and environmentally qualified by OPG are to be tracked. GE Dresser Masoneilan shall advise OPG in the event that changes take place in the following areas: design of the product, material/compounds used, manufacturing methods, non-destructive testing procedure, and test reports. Some changes, which may appear insignificant and not deem to be reported to the client (since fit, form, and function are met), can invalidate the equipment nuclear performances. Changes to non-metallic components are of particular interest to OPG. GE Dresser Masoneilan shall ensure contractual provisions and QA program requirements imposed on suppliers and sub-suppliers is such that GE Dresser Masoneilan will be notified of any such changes that may occur within the supply chain. If any of these changes take place then GE Dresser Masoneilan shall notify the OPG Buyer using the Vendor Parts/ Material Substitution Control form N-FORM during bidding or the Concession Application form N-FORM subsequent to contract award. As an alternative to the preceding paragraph, GE Dresser Masoneilan shall notify the OPG Buyer, using the same forms, if for any reason it is unable to provide assurance that there have been no such changes since the item was originally evaluated/ tested for use in a nuclear safety application. The OPG Buyer will forward all such notifications to the Design Authority in OPG. The responsible Design Authority shall make all decisions relating to the need to re-evaluate and/or re-test and the item. Although the OPG Design Authority may solicit GE Dresser Masoneilan s input to the decision, the decision of the OPG Design Authority will be final. GE Dresser Masoneilan is advised that: This item will be installed in a nuclear safety system. That system will be required to operate following a very unlikely but theoretically possible design basis accident. If such an accident occurred the item could be subjected to a harsh environment consisting of chemical and pressure transients, radiation, steam, and elevated temperature. It is possible that the design basis accident could occur at a time when the item has already been subjected to many years of normal operation when it is nearing the end of its useful operating life. This item has been rigorously evaluated /tested, and the OPG Engineer (see the Contract Standard for definition of Engineer ) expects that it will perform its intended safety function following exposure to this harsh environment. The Environmental Qualification Assessment (EQA) has established that the item may not perform its intended safety function if there is a substitution of materials, changes in design, or changes in manufacturing methods. This is especially true for changes or substitutions of non-metallic components used in the construction of this item. Some non metallic components are known to be subject to degradation in a harsh environment and can deteriorate quickly when exposed to steam, heat, or especially when exposed to high levels of radiation. The OPG Engineer accepts that changes in design, sub-suppliers, materials, and manufacturing methods may be necessary as technology and manufacturing processes change with time. The OPG Engineer also accepts that companies may not be able to track all changes that occur over a long period of time. It is The OPG Engineer s objective to minimize unnecessary changes that would necessitate a reevaluation of the item s ability to perform its intended safety function following a Design Basis Accident as even minor changes may dictate the need to retest or re-qualify the item. In any event, it will certainly dictate the need to evaluate the effect of the change, and a revision to the EQA to ensure that the new item will perform the same intended safety function as the item that was previously qualified. The "changes" referred to in this section are interpreted to mean any changes in materials of construction, changes in manufacturing processes that affect interchangeability, changes in form, fit and function, or any changes from the original design or performance requirements. Insignificant changes that do not affect any of these criteria (i.e. that do not result in a change in the item part number) are considered to be outside the scope of this requirement. 3 3.0 QUALITY ASSURANCE PROGRAM REQUIREMENTS For the EQ items that GE Dresser Masoneilan manufacture/supply to OPG, GE Dresser Masoneilan shall implement and maintain a quality assurance program that provides assurance that GE Dresser Masoneilan tracks material substitutions, design changes, sub-supplier changes, manufacturing and non-destructive testing process changes that may occur during the course of time and that the items meet the design requirements and are equivalent to those originally evaluated and environmentally qualified by OPG. In order to provide such assurance, GE Dresser Masoneilan s QA program shall meet the requirements of minimum CSA CAN3-Z , or CSA N QA standards, including the requirement for product traceability. Traceability is required by this contract, and therefore in accordance with CSA CAN3-Z299.1/2/3-85 or CSA N299.1/2/3-16 QA standards, GE Dresser Masoneilan shall assign to each product or service or batch a unique identification/unique identifier such as serial number, date code, batch code to distinguish apparently identical products. GE Dresser Masoneilan s QA Manual as audited by NUPIC and reviewed/accepted by OPG to the requirements of CSA CAN3-Z , or CSA N QA standards along with the requirements specified in the purchase order is acceptable to OPG to meet this requirement. Non-conformances with a disposition to Repair or Use-as-is require the concurrence of all responsible parties, and submission to OPG for acceptance. A valid certificate of authorization issued by by an authorized registrar (i.e. ASME, TSSA, other Canadian provincial jurisdiction etc) shall be required, in accordance with the CSA N and Update No. 2 standard. All the GE Dresser Masoneilan items and spare parts shall be traceable to GE Dresser Masoneilan audited location in Jacksonville, FL, USA. GE Dresser Masoneilan s distributor in Canada shall also implement a quality assurance program that: Provides assurance that there is no substitution of product specified in the purchase orders without prior approval. Such requests are to be submitted to the OPG Engineer for approval using N-FORM-10393, Provides rigorous controls to ensure that batch trace controls are not lost during the process of receiving, storing, packaging, and shipping the subject products. Conveys/transfers information to and from GE Dresser Masoneilan in an accurate and timely fashion. In order to provide the required assurance, GE Dresser Masoneilan Canadian distributor s quality assurance program shall meet the requirements of CSA CAN3-Z or CSA N (distribution activities). 4.0 INSPECTION AND TEST PLAN AND SPECIAL PROCESS PROCEDURE ACCEPTANCE In accordance with CSA CAN3-Z299.2/3-85 or CSA N299.2/3-16 QA standards, GE Dresser Masoneilan shall prepare the Inspection and Test Plan (I&TP) document as part of the QA program requirements. A copy of the completed I&TP, signed by the authorized QA representative as a minimum, shall be submitted to OPG with the shipment. However, OPG review and acceptance of the I&TP prior to initiating the work is required only when the Inspection and Test Plan Submission Requirement clause QA-I&TP is specified elsewhere in the OPG Contract P.O. This includes the situation for initial procurement of new items not previously manufactured and supplied to OPG. GE Dresser Masoneilan shall make no significant change to the I&TP, critical process procedures, or NDT procedures without notification to, and acceptance from the OPG Engineer for those changes. This notification is to be received at least 4 weeks prior to the commencement of the work to be changed and shall be accompanied by a detailed description of the planned change (including detailed procedures where applicable). In the event that a delay in receiving OPG acceptance for changes will in turn delay completion of the work GE Dresser Masoneilan is to notify the OPG s Buyer at least one week in advance of the expected delay; in order that OPG Buyer can expedite OPG Engineer review and acceptance and thus avoid said delay. This clause applies to OPG Purchase Orders placed on or after March 20, 2018 Page 3 of 6

4 OPG reserves the right to review these during QA program audit or contract specific inspections in order to satisfy itself that manufacturing and inspection activities are appropriately planned, authorized and controlled. GE Dresser Masoneilan critical process procedures and ITP shall be available for audit by OPG source inspectors. Note: Personnel performing inspection, witnessing or monitoring of item critical characteristics for their acceptance must not be those who have performed the work being inspected or report directly to immediate supervisors responsible for producing the work unless specifically permitted otherwise in the I&T Plan and agreed to in writing by the customer before the contract is let. 5.0 PRESSURE RETAINING ITEMS REGISTRATION WITH AUTHORIZED INSPECTION AGENCY Pressure retaining items are to be registered with an Authorized Inspection Agency (AIA). Piping systems shall be registered by technical standards & safety authority (TSSA), located in Ontario. Pressure vessels, storage vessels, pumps, valves, fittings or supports shall be registered as follows: By TSSA located in Ontario, or By Association of Chief Inspectors (ACI) central located in Prince Edward Island. ACI central provides registration for several Canadian provinces, including New Brunswick. As New Brunswick is the only one of these provinces that has nuclear class installations, registration shall be requested for the province of New Brunswick. Fittings are as defined in CSA N and update 2: clause and Table 1. This registration shall be in accordance with CAN/CSA-N and Update No. 2 and the ASME code and class specified on the OPG Design/technical specification referenced in the OPG purchase order (P.O.) item description GE Dresser Masoneilan shall provide evidence of that registration by way of a Canadian Registration Number (CRN) along with the shipment. 6.0 CSA /ESA CERTIFICATION This section is N/A 7.0 SEISMIC QUALIFICATION REQUIREMENTS GE Dresser Masoneilan is to demonstrate the seismic qualification for the item(s) are in accordance with the OPG Design/Technical specification referenced in the OPG purchase order (PO) item description. For repeated orders GE Dresser Masoneilan s Certificate of Conformance statement that the item supplied is equivalent to that originally supplied by GE Dresser Masoneilan (see Section Certification to Accompany Shipment ) is deemed to satisfy the OPG Engineer s requirement for seismic qualification of the item. 8.0 MARKING AND TAGGING Whenever possible, a permanent, corrosion resistant metal tag shall be affixed to each item. Tags are to be permanently attached when appropriate with mechanical fasteners (e.g., screws, rivets, weld or stainless steel wire). Self-adhesive labels and glue are prohibited to ensure that the tag shall not come off during accident conditions. The tag shall be applied in such a manner so as not to affect the function of the item. If it is not possible to attach a tag, GE Dresser Masoneilan may permanently mark, etch or scribe data on the item and spares, whichever is applicable and appropriate usually based on equipment size. This tag and/or appropriate marking on the item shall contain the following information as defined in the item specific design specification. Manufacturer s name and model number/part number Manufacturer s unique identifier such as serial number, batch/lot control number or cure date/ manufacturer date (to ensure traceability) This clause applies to OPG Purchase Orders placed on or after March 20, 2018 Page 4 of 6

5 Design pressure at design temperature (as applicable) Pressure Rating GE Dresser Masoneilan should provide the following details either on the metal tag described above or on an additional tag attached to the item by a stainless steel wire in such a manner so as not to affect the function of the item: Canadian Registration Number (CRN) (as applicable) OPG Catalog ID number All marking and tagging shall be clear, unambiguous and indelible. 9.0 PROTECTION FROM DEGRADATION DURING SHIPMENT OR STORAGE All orifices shall be plugged and/or sealed as applicable, to prevent ingress of foreign materials, dust, dirt, etc. The items shall be individually packaged and protected from degradation during shipment in accordance with ASME NQA-1a-2009, Part II, Subpart 2.2 Level B. Where sealed packaging is used, the seal is to be such that the item can be removed for inspection and resealed in the same package for long term storage. Packaging requirements for level B items are specified in NQA-1a-2009, Part II, Subpart 2.2, section 302.2, which refers to Level A packaging specification defined in section with the exclusion of requirement 302.1(a) (b) Items shall have been inspected for cleanness immediately before packaging. Dirt, oil residue, metal chips, or other forms of contamination shall have been removed by approved cleaning methods. Any entrapped water shall have been removed (c) Items not immediately packed shall be protected from contamination (d) Items requiring protection from water vapor, salt air, dust, dirt or other forms of contamination penetrating the package, shall be packaged with a barrier (e) Items that require protection from damage during shipping and handling shall be packaged in containers or crates (see para 307 of NQA-1a-2009, Part II, Subpart 2.2) (f) Items that can be damaged by condensation trapped within the package shall be packaged with approved desiccant inside the sealed waterproof and vapor-proof barrier or by an equivalent method (g) All openings into items shall be capped, plugged or sealed. Weld end preparations shall be protected against corrosion and physical damage (h) Items packed in containers shall be blocked, anchored, braced or cushioned to prevent physical damage to the item or barrier (i) Items and their container shall be identified by marking. Where packaging is intended to protect the items and spare parts from degradation during storage, the packaging individual cartons or the packaging should be clearly marked with the following: Each individual carton or package is to be identified with the following: GE Dresser Masoneilan s part and model numbers. Date of manufacture. Item shelf life (if applicable) or the date on which the shelf life will expire. OPG s Purchase order number. OPG s Catalogue Id. number. If possible, packaging is to be clearly marked with the following notations: This package contains an EQ item. Do not discard the packaging until installation in the intended application is complete. If this item is to be returned to stock it shall be returned in its original packaging. Store in ASME NQA 1a-2009 level B warehouse conditions until issued for installation/use. Individual packaging shall provide protection from ultraviolet (UV) radiation. Where sealed packaging is used, the seal is to be such that the spare parts can be removed for inspection and resealed in the same package for long This clause applies to OPG Purchase Orders placed on or after March 20, 2018 Page 5 of 6

6 term storage. In situations where GE Dresser Masoneilan cannot provide the spare parts packaging with UV protection, a second set of unused labels shall be provided with the marking as detailed above. GE Dresser Masoneilan is to include a shelf life statement, when applicable, with every purchase order indicating the shelf life of each item (Catalog ID Number) on the purchase order. Shelf life for every item should be assigned by GE Dresser Masoneilan under their quality assurance program as reviewed and accepted by OPG and in accordance with industry recommended shelf life values for similar components (e.g., EPRI, Mil Spec, etc.). Shelf life can be determined by using EPRI methodology: EPRI Report NP6408, Guidelines for Establishing, Maintaining and Extending the Shelf Life Capability of Limited Life Items (NCIG-13).: Typical Data from EPRI Report: NP6408, App. B, List of Generic Shelf Lives (Partial List) Material Shelf life in years (EPRI NP6408 May 1992) EPDM/EPR 60 Neoprene 36 Nitrile 14 Nomex 60 Nylon 25 Teflon 60 UHMWPE 14 Viton 49 OPG Receiving Inspector should not accept the item for delivery if the remaining shelf life is less than 70% of the available shelf life at the time of delivery without prior written consent CERTIFICATION TO ACCOMPANY SHIPMENT GE Dresser Masoneilan shall supply a certificate, which shall accompany the shipment of the item or spare part. This certificate shall be signed by GE Dresser Masoneilan s QA authority and shall certify that: The item has been manufactured in accordance with GE Dresser Masoneilan QA program as audited/accepted by OPG and any additional requirements of the OPG P.O. (refer to Section 3.0). The item supplied both meets the design requirements and is equivalent to the item originally manufactured by GE Dresser Masoneilan that is described in the primary controlled documents referenced in Section 1.0 above. Note: If a change or substitution of material, design, compounds used in construction, manufacturing method(s) or nondestructive method that will affect the item s qualification is proposed or identified, that change shall be reviewed and accepted by the OPG Engineer in writing prior to shipment. Evidence of that acceptance shall accompany the certification statement and the wording of the statement shall be suitably modified to reflect the change and the approval. The certificate shall also clearly indicate the following: GE Dresser Masoneilan s name, model or part number. The serial number(s) or batch number (as applicable). OPG s purchase order number and Catalog ID number.
