1 SAFE WORK METHOD STATEMENT Principal Contractor: ROBERT LIVINGSTONE Project Address: 123 SYDNEY ROAD SYDNEY NSW 2000 High Risk Work Hazards and Risks Identified Control Measures Persons working at a height of more than 2 metres Demolition of a load-bearing structure (or work related to the physical integrity of the structure) Person could fall; falling objects may strike persons or plant / equipment below Person could be crushed; potential damage to plant and equipment; dusts All persons working at a heights of more than 2 metres are to at all times wear a safety harness Scaffolding is to be erected Safety rails are to be installed [For further reference see Code of Practice Safe Work on Roofs Part 2 Residential Building, 2004 WorkCover NSW] Water is to be applied to the demolished area to minimize dust The demolition area is to be isolated within a secure fence Persons involved in the demolition work are to be licensed and wearing appropriate Personal Protective Equipment 1
2 Work involving the disturbance (or likely disturbance) of asbestos Work involving structural alterations or repairs that require temporary support to prevent collapse Work carried out in or near a confined space Work carried out in or near a shaft or trench with an excavation depth of more than 1.5 metres, or in a tunnel Person could sustain a lung disease The structure could collapse; persons could be crushed; potential damage to plant / equipment Dangerous fumes, excessive noise and lack of adequate lighting could affect persons working in confined spaces Person could be buried, or injured whilst entering or exiting the trench Practice How to Manage and Control Asbestos in the Workplace, WorkCover NSW. The installed temporary support is to be examined and signed off by an engineer, and continually monitored. Where dangerous fumes are present, a ventilation system is to be installed. Work is to be carried out in strict accordance with the Code of Practice Confined Spaces, WorkCover NSW. Check with the appropriate authority for the location of underground services (ie. gas, water, communications, electricity) before any digging takes place Secure a suitable barrier / guardrails is to be erected around any excavation If workers are required to be in a trench 1.5 metres or more deep, the sides of the trench are to be battered at an angle of 45 degrees or shored up with a trench support system. Suitable ladders are to be provided for entry and exit from trenches. Practice Excavation Work, WorkCover NSW. 2
3 Work carried out on or near chemical, fuel or refrigerant lines Work carried out in or near energized electrical installations or services Work carried out in an area that may have a contaminated or flammable atmosphere Work involving tilt-up or pre-cast concrete Work carried out on, in or adjacent to a road, shipping lane or other traffic corridor that is in use by traffic other than pedestrians Person could inhale dangerous fumes, be burned, or injured by an explosion Person could be electrocuted Person could be burned and / or injured in an explosion The structure could collapse; persons could be crushed; potential damage to plant / equipment Persons could be hit by moving vehicles; moving vehicles could be hit by plant, equipment or building products Check with the appropriate authority for the location of underground services (ie. gas, water, communications, electricity) Ensure that all work is supervised The worksite is to be protected by an Earth Leakage Device All work is to be supervised by a licensed electrician All chemicals are to be stored safety and be accompanied by a Material Safety Data Sheet There is to be adequate ventilation in confined working areas Strict no smoking rules are to be applied in the vicinity of such an area Workers are to be required to wear appropriate Personal Protective Equipment, including breathing apparatus The installed temporary support is to be examined and signed off by an engineer, and continually monitored. A traffic management plan is to be prepared and submitted to the local council for approval Workers supervising such work are to be adequately trained 3

5 NOTES: 1. If the entire construction project is valued at $250,000 or more, this Safe Work Method Statement ( SWMS ) will form part of the Principal Contractor s Work Health and Safety Management System. 2. The Principal Contractor is to ensure that: (a) the above control measures are implemented prior to high risk construction work commencing; and (b) that all high risk construction work is carried out in accordance with this SWMS; and (c) that all high risk construction work and the control measures implemented are continually monitored. 3. As part of the obligation for consultation, the Principal Contractor is to regularly consult with workers carrying out the high risk construction work identified in this SWMS in relation to the adequacy of the control measures implemented. 4. The Principal Contractor is to ensure that if high risk construction work is not carried out in accordance with this SWMS, that such work is stopped immediately as soon as it is safe to do so, and resumed only in accordance with this SWMS. 5. Any changes to this SWMS must be documented, and the rules that apply to this SWMS will also apply to the amended SWMS. 6. A copy of this SWMS (and any later changes to it) must be readily accessible to any worker who is carrying out, or who is to carry out, high risk construction work. 7. The Principal Contractor must keep a copy of this SWMS: (a) until the high risk construction work to which it relates is completed; or (b) if a notifiable incident occurs in connection with the high risk construction work to which this SWMS relates, for at least 2 years 5
