3 Fatal and Severe Risks Controls Standard 3 INTRODUCTION The Fatal and Severe Risk Standards are an important element of the Laing O Rourke safety management framework. They provide clarity on our expectations and mandatory controls that must be applied across all workplaces where Laing O Rourke Australia operates. The standard applies to all personnel, supply chain partners and visitors involved in Laing O Rourke related activities, including where we partner with other organisations to deliver works. Within our workplaces, there are a number of activities that would have a significant consequence on people if not managed appropriately. Fatal and Severe Risk Controls describe the various minimum mandatory requirements that must be in place and demonstrated to be working effectively to manage these fatal and severe risks. They are supported by Primary Standards within our Safety Management System (SMS). Each workplace with high risk activities is required to establish suitable methods to plan, assess, check and review the implementation and effectiveness of Fatal and Severe Risk Controls. Fatal and Severe Risk Controls will be regularly reviewed to ensure they incorporate important lessons from significant incidents and near misses and changes to our overall work scope. Critical controls are mandatory and must be in place for the effective management of the associated Fatal and Severe Risk. These are controls whose failure would potentially trigger a significant incident. Where critical controls have been identified and assessed as not working effectively, the activity is considered No Go and is not to commence or is to cease immediately. The activity may only restart when the required critical control is effectively implemented. A review of associated activities across the site is to be conducted to ensure the required critical controls are in place. Critical controls are assessed against the referenced criteria within the FSR check tool. All criteria must be met and in place for a critical control to be considered as working effectively. Standard controls are mandatory and support the critical controls and support compliance with the relevant Primary Standards. In assessing standard controls, sites will determine if the controls are in place and working effectively. Where a standard control is not in place, a corrective action is to be developed and implemented for future assessment. Application The Fatal and Severe Risk Controls identified in this document will be used for the following purposes. The controls have been split into two distinct categories critical controls and standard controls each requiring different actions.

4 4 Fatal and Severe Risks Controls Standard Risk Management When assessing and managing risk and applying the hierarchy of controls, elimination of high consequence risk should be the first priority. Where elimination is not feasible the operation must consider the throughout all aspects of the risk management cycle. Workplaces must demonstrate how the Fatal and Severe Risk has been considered and implemented in: Work Winning / Tendering methodologies Safety in Design and Constructability Reviews Project / Workplace Risk Assessments with reference to the program and scope of works Methodologies and / or Work Packs Safe Work Method Statements Planning Self Assessment When developing work packs and methodologies, in consultation with applicable contractors those planning the works will review the planned work using the FSR Check Tool. The frequency of planning self assessments should be established at the workplace based on the forecasted risks for the coming period. Where No-Go s are identified, work will not commence until rectified. Where all critical controls are assessed as Go, the works will be approved to commence as planned. It is important that works are set up for success and that we learn from how successful works are planned and these lessons are shared between work parties, workplaces and projects. Field Checks During high risk working activities must be reviewed as part of Leadership Inspections and by those supervising the work. This is to ensure that all identified criteria are in place and working effectively to manage Fatal and Severe Risks. To assess the implementation of the critical controls, supervisors and engineers should use the Field Checks within the FSR Check Tool. The monitoring frequency will depend on the current activities and the risk profile for the operation. Workers and contractors can also use the Gearbox mobile application to help identify areas of improvement and to promote positive implementation of the critical controls amongst the work teams. Gearbox can be used to facilitate communication between supervisors and workers around Fatal and Severe Risks to keep high consequence risk at the forefront of safety conversations. Where No-Go s are identified works must cease until the deficiencies are rectified. Review and Continuous Improvement Planning and monitoring activity will be recorded through IMPACT. This system will track compliance with Fatal and Severe Risk controls in a standardised format throughout the Australia Hub and allow for workplaces, businesses and Hub to interrogate, review and target areas for improvement. Workplace teams should ensure that results of the planning and monitoring are reviewed regularly and that improvements are communicated. Results will form part of the workplace/contract review process to ensure that all identified criteria are in place and working effectively to manage the Fatal and Severe Risks and that the workplace is learning and continually improving to excel in the management of high consequence risk. 5 Fatal and Severe Risks Controls Standard 5 1 WORKS, EXCAVATIONS UNDERGROUND OVERHEAD SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from temporary works, excavations and underground and overhead services. Temporary Works Coordinator and Temporary Works Supervisor are appointed (suitably qualified). Temporary loadings are applied to the permanent works approved by the designer. Temporary Works are erected, altered or dismantled by a competent person. Change management for temporary works is applied and approved by the designer. A permit to manage excavations is in place. Excavations are controlled to effectively manage access, egress, stability and traffic interactions. Roads and traffic management plans around excavations are designed to ensure plant and people segregation. An inspection regime is completed and recorded on a Temporary Works Control Register. Simultaneous interface of temporary works is controlled. Design/redesign work activities are carried out so the overhead and/or underground services interface is eliminated or reduced so far as is reasonably practicable. Services are positively identified. Live services are de-energised wherever possible and verified as such. Underground and overhead exclusion distances are identified and adhered to. Excavation risks are assessed and geotechnical reports obtained wherever a risk of collapse is identified. Shoring and benching/battering are in place as per the geotechnical design. Exclusion zones for material, spoil and plant surrounding excavations are maintained and clearly delineated. PS 01 Excavation, underground and overhead services. PS 22 Permit to work.

6 6 Fatal and Severe Risks Controls Standard FSR 2 WORKS, EXCAVATIONS UNDERGROUND PLANT OVERHEAD EQUIPMENT SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from the interaction of plant, equipment, vehicles and people. Plant is inspected before mobilisation and before use. Operators are verified as competent for the specific item of plant. Plant and people are separated, defined exclusion zones are maintained, and entry to work areas is controlled. Stability and ground bearing pressure are verified for plant setup. Plant and vehicles are parked in a fundamentally stable condition if there is a possibility of run-away. Safe access for pedestrian movements. A Vehicle Movement Plan or equivalent is in place. Heavy vehicles and light vehicles are segregated wherever possible. Rules are established and enacted for plant following plant or overtaking manoeuvres. Dedicated refuelling and servicing rules and locations are established. Plant is maintained and serviced as required in accordance with Original Equipment Manufacturer (OEM) maintenance schedules. The site perimeter is secured and access is controlled against unauthorised entry. Loading/offloading and Chain of Responsibility activities are managed. 7 Fatal and Severe Risks Controls Standard 7 FSR 3 WORKS, EXCAVATIONS UNDERGROUND CRANES OVERHEAD LIFTING SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from the use of cranes and lifting equipment. An appointed person is in place. A Crane Supervisor is appointed. A Crane Lift Plan is in place. Stability and ground-bearing pressure and verified for crane use (including suspended slabs / floors). A commissioning report is in place for tower cranes. Personnel involved in lifting operations are trained and competent. Lifting operations are effectively managed. Lifting gear is certified and fit for use. Soft slings are not in use. A Lifting Operations Plan is in place. Coordination meetings are held for multiple cranes. Free-fall functionality is deactivated. A tower crane anti-collision system is operational where there is radius overlap. Earthmoving equipment is used as a crane is approved by the appointed person. 8 8 Fatal and Severe Risks Controls Standard FSR 4 WORKS, EXCAVATIONS UNDERGROUND WORKING OVERHEAD AT HEIGHTS SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from working at height and using elevated work platforms. Working at Heights is defined as works with the requirement exists to wear a fall arrest/restraint harness. A working at heights permit is in place. Fall and edge protection equipment is fit for use. Personnel working at heights are trained and competent. Controls such as lanyards, screens and barriers are in place to prevent and manage dropped objects. Anchor points and barriers used during working at heights are fit for purpose. Permit to work in place to manage removal of penetration covers, guardrails and gridmesh. Penetrations are effectively managed. Ground conditions are assessed for EWP operations. Physical barriers, barricading and lockouts to prevent entry by persons are installed directly beneath and immediately adjacent to areas where work is occurring above. Operation of plant used for work at heights is effectively managed i.e. EWP, scissor lift, mast climbers, swing stage etc. Working-at-heights activities are identified at the work planning stage. A rescue plan for working at heights is in place and resources are available. Safe access is in place to working at height areas. Anchor points are certified and used while operating boom-type EWPs. EWP and scissor lift operators are verified as competent for specific items of plant. Telescopic EWPs (knuckle and boom) are fitted with secondary anti-crush guarding. PS 04 Work at height. 9 Fatal and Severe Risks Controls Standard 9 FSR 5 WORKS, EXCAVATIONS UNDERGROUND SCAFFOLDING OVERHEAD SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from erecting, altering, dismantling and using scaffolding. Engineer certified design drawings are available for scaffolding over 4 metres in height or of a complex nature. Protection in place around scaffolding. Scaffolding is erected/dismantled by trained and competent (and licensed where required) personnel. Changes in design are documented and approved by the certifying engineer. All required guardrails are in place during erection and dismantling in accordance with the 1m lift requirement. Incomplete scaffold is controlled and entry prohibited. Controls in place manage dropped objects. Formal periodic inspections are carried out by certified scaffolders or qualified persons. A scaffolding handover certificate is produced for all installations and alterations. Safe erection of mobile scaffold. PS 05 Scaffolding.

10 10 Fatal and Severe Risks Controls Standard FSR 6 WORKS, EXCAVATIONS UNDERGROUND ENERGISED OVERHEAD PLANT, SERVICES ISOLATIONS LOCKOUTS INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from energised plant at installation, testing, commissioning, and decommissioning phases of operation. Unless tested for dead, all wires and equipment are considered and treated as live. Isolations for all potential harmful energy sources are identified by competent persons. Isolation (LOTO) training and competency system in place. Isolation procedures are specific to the needs of the worksite, as determined by the risk assessment. Roles are appointed - Authorised Isolator, Permit Issuer and Permit Holder. Isolation permits are in place. Isolation integrity includes physical try test (check for dead). Restricted Access Controls is in place where equipment cannot be fully isolated to zero energy. Isolations are in place for servicing and maintenance work. Suitable isolation devices, locks and equipment are readily available. System in place to manage complex isolations. The transition from construction to commissioning phase is controlled by permits. 11 Fatal and Severe Risks Controls Standard 11 FSR 7 WORKS, EXCAVATIONS UNDERGROUND ELECTRICAL OVERHEAD SAFETY SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from onsite electrical installations and the use of electrical equipment. Unless tested for dead, all wires and equipment are to be treated as live. Personnel undertaking electrical work are trained and competent. Temporary electrical works are installed, tested and commissioned to standard. All circuits and powered equipment have RCD protection. Switchboards are compliant and secured. All energy sources are clearly identified and marked. Live cabling is protected from mechanical damage. Work in and around electrical infrastructure is effectively managed. Current electrical drawings and electrical system data are readily available. Commissioning procedures are readily available. Generators and welders are correctly earthed and staked when required by the Original Equipment Manufacturer (OEM) manual. PS 07 Electrical safety. PS 22 Permit to work.

12 12 Fatal and Severe Risks Controls Standard FSR 8 WORKS, EXCAVATIONS UNDERGROUND CONFINED OVERHEAD SPACES SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from work in confined spaces. Entry controlled by a permit system. A stand by person is in place at all times there are people inside a confined space. Workers, stand by person and supervisors are appropriately trained, certified and competent. Air monitoring program is in place. Emergency Plan is in place. A specific risk assessment is conducted for each confined space. Confined spaces are identified on a register and warning signs are displayed. 13 Fatal and Severe Risks Controls Standard 13 FSR 9 WORKS, EXCAVATIONS UNDERGROUND FORMWORK OVERHEAD SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from erection and dismantling of formwork and false-work. Formwork design is completed. Engineer inspection is completed prior to placing concrete and any discrepancies addressed. Safe placement of concrete. Design of slip and jump forms consider access, personnel and material requirements. Safe erections and stripping. Works are erected in accordance with all relevant drawings and specifications. Formwork and false-work materials are in sound condition. PS 09 Formwork.

14 14 Fatal and Severe Risks Controls Standard FSR 10 WORKS, EXCAVATIONS UNDERGROUND DEMOLITION OVERHEAD SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from demolition work. The demolition methodology is verified. Trained and competent personnel plan and undertake the works. Engineer s sequence of demolition is understood and followed. All services have been disconnected prior to work commencing. Hazardous materials assessment is undertaken prior to work commencing. Asbestos and other hazardous materials is identified and removed by a licensed contractors prior to demolition commencing. Exclusion zones are established. A Demolition Management Plan is developed to AS 2601 and communicated. A licensed demolition contractor is engaged and has the correct licence. Demolition notification is submitted to the regulator as required. Monitoring devices are installed where required dust, noise and asbestos. PS 07 Electrical safety. PS 10 Demolition. PS 11 Asbestos. 15 Fatal and Severe Risks Controls Standard 15 FSR 11 WORKS, EXCAVATIONS UNDERGROUND ASBESTOS OVERHEAD SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from the management, removal and disposal of asbestos. Where asbestos or asbestos containing material (ACM) is identified, an Asbestos Management Plan is developed, communicated and regularly reviewed. Trained and Competent personnel (licensed asbestos removers and company). Air monitoring is in place for all friable / non-friable asbestos removal. Appropriate decontamination facilities are available and material is decontaminated or sealed before removal. Health monitoring is undertaken for workers carrying out asbestos work. All asbestos and ACM s are identified on register. Asbestos is removed by a licensed contractor. If monitoring shows more than 0.02 fibres/ml, work is stopped and the regulator informed. Only asbestos vacuum cleaners with correct filters (HEPA) are used for clean-up; no domestic vacuum cleaners are used. A clearance certificate is issued prior to workplace being reoccupied. PS 11 Asbestos. PS 22 Permit to work.

16 16 Fatal and Severe Risks Controls Standard FSR 12 WORKS, EXCAVATIONS UNDERGROUND PRECAST OVERHEAD TILT-UP SERVICES CONCRETE INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from precast and tilt up concrete work. There is a certified design for precast elements and tilt up concrete. A documented safe erection system. An approved propping and stripping methodology is available. Changes to panel and support system design are approved by the designer. The lifting clutch is approved by the panel or element manufacturer and matches the cast in lifting point. Removal of temporary bracing is effectively managed. Exclusion zones are in place during erection. Personnel are trained and competent. Concrete strength test results are kept on site for panels and footing slabs. Deadman screw anchors/rock anchors are inspected. 17 Fatal and Severe Risks Controls Standard 17 FSR 13 WORKS, EXCAVATIONS UNDERGROUND WORK OVERHEAD OVER WATER SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from work over or adjacent to water and diving work. Risk of falling into water is managed. Personal floatation devices are worn over or adjacent to water. Emergency plan and equipment in place. Personnel are trained and competent and where required, licensed for diving work and vessel operation (e.g. boatswain s or coxswain s qualifications). Approved life buoys and lifelines are secured and correctly positioned. Dive safety logs are kept for each dive. Diving first aid equipment and trained personnel are available. 18 18 Fatal and Severe Risks Controls Standard FSR 14 WORKS, EXCAVATIONS UNDERGROUND RAIL OVERHEAD OPERATIONS SERVICES INTENT: To eliminate or minimise the risks of fatalities, injuries and incidents arising from rail operations. Safeworking systems applied to protect personnel and plant from trains and track machines on the railway line work is being performed. Safeworking systems applied to protect personnel and plant from trains and track machines on the adjacent railway line. Worksite delineation applied to protect workers from other railway operations conducted nearby. Safe systems of work applied when plant and personnel required to work in electrified area or close to services. All rollingstock to be operated in association with the planned works conforms to the standards relevant to the railway they are to operate on. Fitness for work is verified for all persons on site as well as required rail safety worker competencies held. Effective communication in place Protection Officer, Rolling Stock Operators, work crews and Network Control. Have processes been determined that allows the rolling stock access to the network owners railway for all railway operations to be undertaken? Has it been determined in contractual documents or other recorded documents whose rail accreditation will apply and whose safety management system will apply? PS 14 Rail operations. 19 Fatal and Severe Risks Controls Standard 19 FSR 15 TRAFFIC CONTROL INTENT: To eliminate or minimise the risk of fatalities, injuries and incidents arising from working in close proximity to passing traffic and other road users. Planning and staging of works must apply the safety hierarchy of controls and seek opportunities to eliminate the interface with live traffic flows, consideration should be given to the use of side-tracks, safety barriers, road closures and/or detours Road Occupancy Licences and Speed Zone Authorisations are in place A competent road safety traffic management representative is appointed to oversee the application of traffic control Traffic Management Plan(s) in place, adhered to and monitored by competent traffic management personnel Work areas are isolated from passing traffic with approved safety barrier treatments where road speeds are equal to or greater than 60km/h Traffic controllers must be replaced by automatic/remote devices unless stipulated by the road authority or traffic signal operations limitations do not make it feasible Traffic controllers must demonstrate experience for specific traffic situations and be adequately resourced Truck mounted attenuators are used as buffer vehicles in conjunction with speed reductions on multi-lane roads, for mobile works or when setting up and removing temporary traffic controls Physical protection and safe escape routes are in place for traffic control personnel interacting with live traffic flows Designated construction access points are provided and managed to prevent risk to workers or road users An audit and inspection regime is dedicated to the traffic control system to ensure compliance with traffic control plans and the controls applied in the field Works are planned and staged to minimise road occupancy and reduce conflict points with the existing road network Exclusion zones are applied behind safety barriers to protect workers from entering the deflection zones of the barriers Traffic controls, including signage and devices are in place to effectively warn, inform and guide road users through the work area The use of electronic sign posting should be applied, including variable message signs and variable speed limit signs which should be linked to speed monitoring systems with feedback to the road user Vehicle movement plans are in place for all construction vehicle movements PS 15 Traffic Management
